01/06/2012 : Online digitally signed tenders are invited from the General Insurance
Companies for selection of Insurance Company / Companies to implement Centrally
Sponsored “Livestock Insurance Scheme” as per the guidelines of the scheme for the
period 2012 - 2014. The scheme will be in operation in Nagpur, Bhandara, Gondiya,
Wardha, Yavatmal, Jalna, Aurangabad, Beed, Nanded, Latur, Jalgaon, Nashik,
Ahamadnagar, Pune, Satara, Sangali, Solapur & Kolhapur of Maharashtra. However, the
number of districts might be increased as per the guidelines of Government of India. The
Insurance Company must have office at these district head quarters for insurance
coverage & claim settlement with sufficient insurance agents at blocks/ Talukas which is
mandatory. The Insurance coverage is presently restricted to two high yielding cows &
buffaloes per animal owner for general death as well as Permanent Total Disabled (PTD)
cases for one or three years period, however, the number animals per beneficiary are
likely to be increased. The 50% premium share will be paid by this office & 50%
premium will be born by animal owner which should be collected by the Insurance
concerned companies.
The tender details, terms and conditions may be purchased, downloaded, prepared from
04.06.2012 to 19.06.2012 & submitted online till 19.06.2012 upto 17.00 hrs. on the
department’s e-Tendering Portal http://adf.maharashtra.etenders.in and submit payment for
website Registration charges or Rs. 5000/- (Rs Five thousand Only) in the form of Demand draft
drawn in the name of “Chief Executive Officer, Maharashtra Livestock Development Board,
Akola” drawn from any Nationalized Bank payable at Akola. This amount is not refundable.

TERMS & CONDITIONS FOR SUBMITTING THE TENDER FOR PREMIUM RATES
FOR LIVESTOCK INSURANCE (2012 - 2014)

1. How to get Tender forms:
The Tender Form along with Terms & Conditions can be downloaded from
http://adf.maharashtra.etenders.in w.e.f. 04.06.2012 to 19.06.2012. by payment of Rs. 5000/-(Rs
Five thousand Only) as the cost of website registration charges in the form of Demand Draft.
This amount is non-refundable. The Demand Draft should be drawn in favour of “Chief
Executive Officer, Maharashtra Livestock Development Board”, payable at Akola. Failure to
submit the cost of website registration charges within stipulated time will disqualify the tender.
2. EARNEST MONEY DEPOSIT (EMD)-
a. Earnest Money Deposit should be paid Rs. 6,00,000/- in the form of Demand Draft of
any Nationalized Bank. Cheque and cash will not be accepted at any stage and in that
case tender form will be rejected.
b. The Government institutes will be exempted from payment of EMD.
3.LAST DATE AND PLACE OF SUBMISSION OF COMPLETED TENDER FORMS –
Completed Tender Form along with the tender rate should be prepared on department’s etendering
Portal on or before (SCHEDULED DATE) & submitted on same portal on or before
(SCHEDULE DATE) duly filled in and digitally signed by concerned authority. e-tendering
system won’t accept any tender after the deadline for preparation & will allow only those tenderer
who have completed their online bid preparation for submission, Tender received in other format
will not be accepted.
4. MANNER OF SUBMISSION OF COMPLETED TENDER FORMS-
A) Tenderers should submit tenders in two envelopes system (two bid system).
Accordingly, the tender form dully filled for the rates quoted for items should be mentioned
in online form given in Commercial Bid (C1). All other required documents such as
Technical documents as mentioned in the check-list, demand draft along with a copy of terms
and conditions duly signed and all documents supporting the Eligibility criteria for
participation in tender process (Refer GENERAL TERMS & CONDITIONS FOR
SUBMITTING TENDER) etc. should be uploaded in forms given under Technical Envelope
(T1).
B) It may be noted that while opening the tender online on above mentioned e-Tendering
Portal, Technical Envelope (Envelope No.1) will be opened in the first instance. Commercial
Bid C1 will be opened on above mentioned e-Tendering Portal only after ascertaining
whether the tenderer has fulfilled all requisites in Envelope No.1. The tenderer not fulfilling
the above requirements will be rejected and no further claim will be entertained.
Technical Bid (T 1) :- Technical information (as per Checklist) in the form of following
documents. All the documents uploaded in Technical Bid (T1), should be scanned in PDF
Format having duly certified/attested by Gazatted Officer or Notary and the originals should
be kept ready for submission on demand.
1. The cost of website registration charges Rs. 5000/-
2. Earnest money Deposit of Rs.6,00,000/- in the form of uncrossed Demand Draft drawn in
favor of “Chief Executive Officer, Maharashtra Livestock Development Board”, payable
at Akola.
3. Copy of Rate Contract, if existing with any other government/ Board/ corporation.
4. All Documents attested by Gazetted Officer/ Notarized.
5. Declaration form, all pages of the tender form including Terms and conditions duly
signed.

Commercial Bid (C1 ) :- The Commercial Bid C1 as mentioned above should contain rates.
Rates quoted in prescribed tender form or uploaded on separate paper will not be considered and
the tender form will be rejected. The rates should be quoted as per stipulated format. The Chief
Executive Officer, Maharashtra Livestock Development Board, Akola reserves the right to call
for break up of rate wherever necessary.
The Technical Bid T1 and Commercial Bid C1 should also be digitally signed by
tenderer before completion of Online Bid Preparation on or before (SCHEDULED
DATE) and again submitted online on or before (SCHEDULED DATE) of Online Bid
Submission. e-Tendering System won’t allow any tenderer to participate in tender after
stipulated date & time. Similarly no request for extension of time will be considered.
The firm who make any undue effort to bring the pressure from outside or from any
departmental authority will be liable for outright rejection.
Tenders not submitted as per the above-prescribed manner will be treated as invalid.
5.COMPANY PERFORMANCE: If any company has not given satisfactory performance
during previous years, then Chief Executive officer, Maharashtra Livestock Development Board,
Akola reserves the full right to reject its tender or part thereof.
6. REGISTRATION: The copy of valid Registration certificate must be enclosed in Technical
T1.
7. COMPANY OFFICE & NETWORK: The Insurance company must have offices at district
head quarters with Network in the above mentioned districts for correspondence about insurance
policies issued, ear tagging-Retagging & claim settlement. For this purpose insurance companies
must appoint one development officer at district head quarters & minimum four insurance agents
at Taluka/ Block level. It is mandatory to mention name, address, phone no. & authorization
letter for district & block level officer & insurance agents of company.
8. CHANGE OF INSURANCE COMPANY: In the event of the order being placed to the
successful tenderer and if the tenderer fails to quote the premium rates & not performing
according to the terms and conditions envisaged, then Chief Executive Officer, Maharashtra
Livestock Development Board, Akola will be at liberty to engage another Insurance
Company/Companies in his sole discretion which he thinks fit.
9. DOCUMENTS: The tender documents should be written legible and free from errors, over
writings scratches etc.
a) No tenders will be accepted after stipulated date & time. Similarly no request for extension of
time will be considered.
b) The firm who make any undue effort to bring the pressure from out side or from any
departmental authority will be liable for outright rejection.
c) Tenders not submitted as per the above prescribed manner will be treated as invalid.
10. LAST DATE FOR SUBMISSION & OPENING OF TENDER FORM:
Online e-tender should be submitted on e-tender Portal http://adf.maharashta.etenders.in on or
before 19.06.2012 up to 17.00 hrs. The technical bid tender will be opened possibly on
21.06.2012 at 13.00 hrs and Commercial bid shall be possibly on 22.06.2012 at 14.00 Hrs. in the
“Office of Regional Manager, Frozen Semen Laboratory”, 5, Mula Road, Khadki, Pune 411003.
The tender form will not be accepted after stipulated date & time & hence no request for
extension of time will be considered.
11. PREMIUM RATES: The tenderer should quote premium rates separately for General Death
and Permanent Total Disability (PTD) for one & three year period in commercial bid C1. These

premium rates once quoted will not be increased on account of increase in Govt taxes or due to
escalation of any costs during the contract period.
12. INCOMPLETE TENDER: The tender is liable to be ignored if complete information is not
furnished or if any or all the conditions prescribed herein are not fulfilled or if any item is
overwritten or corrected.
13. PENALTY: i) If advance premium received is not refunded within stipulated time at the end
of contract period then 2% interest per month will be charge as penalty.
ii) If claims are not settled in stipulated time of 15 days as per agreement then 2% of sum insured
per month will be charged as penalty.
iii) Services of Govt. field veterinary staff/ Officers should not be utilized for collection of
premium from farmers/ livestock owners & if found, the concerned company will be penalized
@15% of the premium collected from the farmers & will be deducted from the next installment to
be given by MLDB.
14. INSURANCE COVERAGE: The insurance coverage is presently restricted to high milk
yielding cows and buffaloes with restriction of maximum two animals per animal owner of the
Nagpur, Bhandara, Gondiya, Wardha, Yavatmal, Jalna, Aurangabad, Beed, Nanded, Latur,
Jalgaon, Nashik, Ahamadnagar, Pune, Satara, Sangali, Solapur & Kolhapur of Maharashtra.
However, the number of districts might be increased as per the guidelines of Government of
India. The Insurance coverage is presently restricted to two high yielding cows & buffaloes per
animal owner for general death as well as Permanent Total Disabled (PTD) cases for one or three
years period, however, the number animals per beneficiary is likely to be increased.
15. INSURANCE POLICIES:
i) Insurance company will make district wise master policy for contract period & copy will be
given to concerned district & MLDB.
ii) Insurance company will issue insurance policy to policy owner along with information about
claim procedure, retagging procedure etc. This document should be in Marathi language. No
other documents will be demanded by the insurance company without the permission of MLDB.
16. CLAIM AMOUNT: The Insurance claim amount in case of death will be equal to sum
insured & 75% for PTD cases. The Insurance company should settle the claim on time and should
send the claim amount to policy holder i.e. cattle owner.
17. SETTLEMENT CLAIMS: The Insurance Company will be committed for settlement of
claim at district level only within 15 days from the date, the claim is received by company which
is mandatory. The documents for claim settlement will include:
i) First information report about death of animal to be reported by animal owner on toll free or
contact no provided by insurance company
ii) Insurance policy.
iii) Claim form
iv) Post Mortem Report with intact ear tag.
v)No other documents will be demanded by the insurance company. In case of investigation of
claim is required then it will be done within 7 days of documents received from animal owner.
vi)If death of animal is reported within 48 Hrs, but the documents are submitted late due to
genuine & legal difficulties & confirmed by the Govt. Veterinarian then the claim will not be
denied on the ground of delay in submission of necessary documents by animal owner.
The cost of ear-tagging/ re-ear-tagging & photo identity (if insisted by insurance
company) of animal will be borne by the insurance company.

18. PUBLICITY: Insurance company must ensure wide publicity in Marathi language by way of
leaflets and posters, in addition to Radio, Television, News paper advertisements etc. The
proposal/ claim form should be made available in Marathi. The Insurance company should
display scheme information including claim procedure in all Gram-panchayat offices, Veterinary
Institutes and ensure wide publicity to achieve the targets as per agreement and guidelines of
Govt. of India.
19. PERFORMANCE GUARANTEE: Insurance Company will have to give performance
Guarantee during the period of scheme in operational area till the expiry period of insurance
coverage and claim settlement.
20. INSURANCE EXPERIENCE: The Insurance Company should have at least three years
experience in the field of Cattle Insurance preferably where the scheme is being implimented, the
attested copies of related documents should be enclosed.
21. INSURANCE AGENT: The insurance company/ companies should ensure sufficient agents
at the block level, so as to provide services at all Govt. veterinary institutes & to the concerned
involved in the insurance coverage for the purpose of identification of animals, collection of
premium, timely issue of policies, ear- tagging / retagging, prompt claim settlement procedure.
For this purpose, insurance company should pay 15% commission to their agents at company cost
as per the directives of the scheme given by the Govt. of India. No Govt. employee will be
entrusted for collection of premium from farmers which will come under penalty.
22. ADVANCE PAYMENT: The 50% premium share will be borne by govt. and if required
could be paid in advance to the insurance company. However, insurance company will have to
give complete information in the prescribed format duly certified by concerned Dist. Dy.
Commissioner of A.H. at the time of recoupment as per the guidelines of the scheme & will be
mentioned in the MOU/ agreement.
23. MOU / AGREEMENT: Selected Insurance company(s) will have to make MOU/Agreement
as per the directives given in the insurance scheme of Govt. of India within 10 days of acceptance
of tender.
24. MONTHLY REPORTING: The Insurance Company will have to submit monthly report of
physical & financial statement in the prescribed format along with individual policy details of
each district and claim report on or before 5 th of each month which is mandatory.
In case of any breach of terms and conditions of the contract & if dispute is not resolved
within stipulated time with Chief Executive Officer, Maharashtra Livestock Development Board,
Akola, the matter will be referred to Insurance Regulatory and Development Agency, as per the
guidelines of the scheme.
25. OPENING OF TENDERS:
Opening of Technical bid of tenders shall be possibly on 21.06.2012 at 13.00 Hrs. and
Commercial bid shall be possibly on 22.06.2012 at 14.00 Hrs. in the office of the Regional
Manager, Frozen Semen Laboratory, Mula Road, Khadki, Pune-03. Every bidder is at liberty to
remain present or authorize one representative to be present at the opening of the tender at the
time and date specified.
26. ACCEPTANCE OF TENDERS:
Chief Executive Officer, Maharashtra Livestock Development Board, Akola reserves the right of
accepting the whole or any part of the tender or to refuse all tenders without assigning any reason.

27. SETTLEMENT OF DISPUTES:
a. All disputes with Insurance Company shall be settled by the Chief Executive Officer,
Maharashtra Livestock Development Board, Akola. However, the disputes between the
insurance company and the Chief Executive Officer, Maharashtra Livestock Development
Board, Akola or the appeal against the decision of Chief Executive Officer, Maharashtra
Livestock Development Board, Akola in disputes referred to him shall lie with the
Secretary (ADF) Mantralaya, Mumbai-32/ Chairman, Maharashtra Livestock Development
Board, Akola
b. Judicial proceedings, if any, can be started only in Courts of the State of Maharashtra at
Akola Court only.
c. Chief Executive Officer, Maharashtra Livestock Development Board, Akola will not be
held responsible for any claim demands and/ or judgement of any nature brought against
the M.L.D.B. Akola arising out of the services rendered by the insurer and it’s staff under
this contract. Default in settlement of claim or any types of deficiency in services on the
part of insurance company could be brought to the notice of Insurance Regulating and
Development Authority, which is nodal authority in the country. In case of dispute in
implementation of this scheme the opinion of Chief Executive Officer, Maharashtra
Livestock Development Board, Akola will be final and will be binding to the insurance
companies.
28. REMOVAL OF DIFFICULTY:
The dates quoted in the tender form are subject to change in the event of any holiday abruptly
declared by Govt. Chief Executive Officer, Maharashtra Livestock Development Board, Akola
will take appropriate decisions to remove difficulties arising due to ambiguity in provisions made
in the tender document.
