Diagnosis <strong>of</strong> <strong>Tuberculosis</strong> Disease<br />

The diagnosis <strong>of</strong> TB disease is <strong>of</strong>ten overlooked because <strong>of</strong> the failure to consider it<br />

among possible diagnoses. While a definitive diagnosis may involve the addition <strong>of</strong><br />

laboratory and radiographic findings, a high degree <strong>of</strong> suspicion can be based on<br />

epidemiology, medical history, and physical examination. In considering TB disease, it is<br />

also important to consider factors that may affect the typical presentation <strong>of</strong> TB, such as<br />

the patient’s age, nutritional status, and coexisting diseases.<br />

An individual who is suspected <strong>of</strong> having TB disease requires a complete medical<br />

evaluation, including the following:<br />

Medical history, including exposure, symptoms, previous treatment for TB, and risk<br />

factors<br />

Human immunodeficiency virus (HIV) screening<br />

Physical examination<br />

Tuberculin skin test or interferon gamma release assay<br />

Chest radiography<br />

Bacteriologic examination<br />

When a suspected case <strong>of</strong> pulmonary TB is identified, refer to Table 4 for guidelines for<br />

the initial steps <strong>of</strong> TB case detection in five clinical scenarios encountered by primary<br />

care providers and emergency physicians. 24<br />
