1.4 HEALTH DISCOURSES<br />

Study<strong>in</strong>g health issues <strong>in</strong> <strong>the</strong> African context <strong>in</strong>volves deal<strong>in</strong>g with medical diversity.<br />

Therapeutic pluralism has always been part <strong>and</strong> parcel <strong>of</strong> <strong>the</strong> African context (Orley 1980:127;<br />

Feierman & Janzen 1992:4; Good 2004:10), although <strong>the</strong> presence <strong>of</strong> Western biomedic<strong>in</strong>e has<br />

confidently dom<strong>in</strong>ated <strong>the</strong> medical scene <strong>in</strong> such a way that it seemed <strong>the</strong> only authorized option<br />

for health-seekers. This disparity <strong>in</strong> <strong>the</strong> range <strong>of</strong> health options came <strong>in</strong>to existence under<br />

colonial rule <strong>and</strong> missionary <strong>in</strong>fluence, but for a few decades now some changes have been<br />

noticeable <strong>in</strong> <strong>the</strong> African <strong>heal<strong>in</strong>g</strong> l<strong>and</strong>scape: <strong>the</strong> dist<strong>in</strong>ct boundaries between <strong>in</strong>digenous <strong>heal<strong>in</strong>g</strong><br />

practices <strong>and</strong> Western biomedical treatment are dim<strong>in</strong>ish<strong>in</strong>g, due to <strong>in</strong>creas<strong>in</strong>g collaboration<br />

between <strong>the</strong> two health discourses (Neumann 1982:217; Khamalwa 2006:86; Amutabi<br />

2008:159). In addition to <strong>the</strong> grow<strong>in</strong>g complementarity between allopathic medic<strong>in</strong>e <strong>and</strong> o<strong>the</strong>r<br />

health traditions, <strong>the</strong> importance <strong>of</strong> lay persons <strong>in</strong>volved <strong>in</strong> <strong>the</strong> <strong>heal<strong>in</strong>g</strong> process is <strong>in</strong>creas<strong>in</strong>gly<br />

recognized (cf. Rasmussen 2008:11). Not only do healers <strong>and</strong> specialists articulate <strong>and</strong> transmit<br />

<strong>the</strong> various health ideas <strong>of</strong> <strong>the</strong>ir particular discourse, but lay persons also play <strong>the</strong>ir part <strong>in</strong><br />

treat<strong>in</strong>g diseases <strong>and</strong> <strong>in</strong> produc<strong>in</strong>g medical knowledge. Tak<strong>in</strong>g <strong>the</strong>se developments <strong>in</strong>to<br />

consideration, it seems obvious that <strong>the</strong> range <strong>of</strong> health ideas <strong>and</strong> practices <strong>in</strong> Africa is<br />

<strong>in</strong>creas<strong>in</strong>g, <strong>and</strong> that African <strong>heal<strong>in</strong>g</strong> perspectives are cont<strong>in</strong>uously be<strong>in</strong>g syn<strong>the</strong>sized. The fusion<br />

<strong>of</strong> <strong>the</strong> many health concepts is not a haphazard process, but happens accord<strong>in</strong>g to <strong>the</strong> logics <strong>of</strong><br />

experience <strong>and</strong> response based on <strong>the</strong> available resources with<strong>in</strong> a specific context. When<br />

consider<strong>in</strong>g <strong>the</strong> context <strong>of</strong> Sou<strong>the</strong>rn Africa, <strong>the</strong> evolution <strong>of</strong> health ideas takes place with<strong>in</strong> <strong>the</strong><br />

scope <strong>of</strong> different but <strong>in</strong>terrelated discourses.<br />

At this po<strong>in</strong>t, it is necessary to expla<strong>in</strong> <strong>in</strong> greater depth what is actually meant by ‘discourse’,<br />

s<strong>in</strong>ce <strong>the</strong> term is used <strong>in</strong> numerous ways <strong>and</strong> <strong>in</strong> various discipl<strong>in</strong>es. ‘Discourse’ carries multiple<br />

mean<strong>in</strong>gs, <strong>and</strong> <strong>of</strong>ten even a syn<strong>the</strong>sis <strong>of</strong> mean<strong>in</strong>gs. So, <strong>in</strong> try<strong>in</strong>g to avoid <strong>the</strong> pitfall <strong>of</strong> us<strong>in</strong>g <strong>the</strong><br />

term without clarify<strong>in</strong>g how ‘discourse’ is understood <strong>in</strong> this research, <strong>the</strong> follow<strong>in</strong>g paragraphs<br />

address <strong>the</strong> mean<strong>in</strong>g <strong>of</strong> <strong>the</strong> term as applied <strong>in</strong> <strong>the</strong> rema<strong>in</strong>der <strong>of</strong> this research.<br />

1.4.1 Discourse <strong>the</strong>ory<br />

The use <strong>of</strong> <strong>the</strong> term discourse <strong>in</strong> current research (particularly <strong>in</strong> <strong>the</strong> field <strong>of</strong> cultural <strong>the</strong>ory,<br />

l<strong>in</strong>guistics <strong>and</strong> social psychology <strong>and</strong> critical discourse analysis) is mostly def<strong>in</strong>ed by <strong>the</strong> work<br />

<strong>of</strong> Michel Foucault. Although his approach did not result <strong>in</strong> a neatly worked out <strong>and</strong> systematic<br />

def<strong>in</strong>ition <strong>of</strong> discourse, Foucault regarded ‘discourse’ as a prom<strong>in</strong>ent issue <strong>of</strong> attention, because<br />
