Tra<strong>in</strong><strong>in</strong>g Surgeons <strong>in</strong> <strong>the</strong> Develop<strong>in</strong>g <strong>World</strong><br />

403<br />

Attend<strong>in</strong>g National <strong>and</strong> International Medical Conferences<br />

Ideally, surgery residents should attend one or more medical conferences a year.<br />

Although <strong>the</strong> limit<strong>in</strong>g factor will usually be f<strong>in</strong>ancial, Program Directors should do<br />

everyth<strong>in</strong>g <strong>the</strong>y can to make it happen.<br />

Attend<strong>in</strong>g regional <strong>and</strong> <strong>in</strong>ternational conferences enables residents to network<br />

with o<strong>the</strong>r physicians, ga<strong>in</strong> experience <strong>in</strong> critiqu<strong>in</strong>g papers that are presented, present<br />

<strong>the</strong>ir own papers, <strong>and</strong> learn from o<strong>the</strong>rs. It also legitimizes well-conceived <strong>and</strong> organized<br />

surgical programs <strong>and</strong> allows o<strong>the</strong>r physicians to take <strong>the</strong> measure of surgeons<br />

<strong>in</strong> tra<strong>in</strong><strong>in</strong>g. It may even encourage o<strong>the</strong>r surgeons to help <strong>in</strong> some way to improve a<br />

tra<strong>in</strong><strong>in</strong>g program <strong>and</strong> improve its st<strong>and</strong><strong>in</strong>g <strong>in</strong> <strong>the</strong> medical community.<br />

Conclusion<br />

Some surgeons work<strong>in</strong>g <strong>in</strong> <strong>the</strong> develop<strong>in</strong>g world have concluded that <strong>the</strong> obstacles<br />

to tra<strong>in</strong><strong>in</strong>g adequate numbers of qualified surgeons for <strong>the</strong>ir countries are<br />

<strong>in</strong>surmountable. As a result most have quietly tra<strong>in</strong>ed nurses to operate. O<strong>the</strong>rs<br />

have tra<strong>in</strong>ed doctors to operate but have felt <strong>in</strong>capable of tra<strong>in</strong><strong>in</strong>g <strong>the</strong>m to <strong>the</strong>ir own<br />

level of competence without help. O<strong>the</strong>rs have simply been <strong>the</strong> best surgeons possible<br />

without tra<strong>in</strong><strong>in</strong>g anybody. Because of an ongo<strong>in</strong>g <strong>and</strong> <strong>in</strong>creas<strong>in</strong>gly severe shortage<br />

of surgeons none of <strong>the</strong>se choices have done much to make quality surgical care<br />

more available or affordable to <strong>the</strong> average citizen <strong>in</strong> <strong>the</strong> develop<strong>in</strong>g world.<br />

Despite <strong>the</strong> apathy or even antipathy of governments or of <strong>the</strong> surgical community<br />

as a whole, we believe that <strong>in</strong> countries where <strong>the</strong>re critical shortages rema<strong>in</strong> a<br />

surgeon work<strong>in</strong>g alone can <strong>and</strong> should reproduce himself. He is more likely to be<br />

successful if he networks with o<strong>the</strong>r <strong>in</strong>terested surgeons both with<strong>in</strong> <strong>and</strong> beyond <strong>the</strong><br />

borders of his country of service <strong>and</strong> if he associates his effort with an <strong>in</strong>ternational<br />

surgical society dedicated to surgical education. The establishment of <strong>the</strong> Pan-African<br />

College of Christian Surgeons is an example of surgeons work<strong>in</strong>g <strong>in</strong> hospitals throughout<br />

<strong>the</strong> cont<strong>in</strong>ent of Africa creat<strong>in</strong>g an association to develop surgical residencies <strong>in</strong><br />

exist<strong>in</strong>g hospitals. These efforts to <strong>in</strong>crease <strong>and</strong> upgrade surgical education <strong>in</strong> <strong>the</strong><br />

develop<strong>in</strong>g world deserve <strong>the</strong> enthusiastic support of both private <strong>and</strong> government<br />

surgeons <strong>and</strong> of national <strong>and</strong> <strong>in</strong>ternational surgical societies.<br />
