<strong>and</strong> electrolyte resuscitation have been achieved. If <strong>the</strong>re is no free air, a diagnostic<br />

open peritoneal lavage can be done under local anes<strong>the</strong>sia <strong>in</strong> <strong>the</strong> midl<strong>in</strong>e one-<strong>in</strong>ch<br />

below <strong>the</strong> umbilicus after dra<strong>in</strong><strong>in</strong>g <strong>the</strong> bladder by ca<strong>the</strong>ter to rule-out or treat a<br />

distended bladder <strong>and</strong> avoid damage to <strong>the</strong> bladder. If pus or <strong>in</strong>test<strong>in</strong>al contents are<br />

returned after lavage with a litre of warm sterile sal<strong>in</strong>e, explore <strong>the</strong> abdomen or refer<br />

<strong>the</strong> patient for exploration immediately. If perforated peptic ulcer is suspected, start<br />

<strong>the</strong> <strong>in</strong>cision <strong>in</strong> <strong>the</strong> upper abdomen. O<strong>the</strong>rwise use a long midl<strong>in</strong>e <strong>in</strong>cision to give<br />

full access to <strong>the</strong> abdomen.<br />

At exploration, exam<strong>in</strong>e <strong>the</strong> entire abdomen <strong>and</strong> subphrenic regions <strong>and</strong> <strong>the</strong><br />

entire length of <strong>the</strong> <strong>in</strong>test<strong>in</strong>es. To treat a perforated duodenal ulcer, a patch of attached<br />

omentum is sutured over <strong>the</strong> perforation without attempt<strong>in</strong>g to suture <strong>the</strong><br />

hole closed. A similar method may be used <strong>in</strong> early typhoid perforation (usually of<br />

term<strong>in</strong>al ileum as <strong>in</strong> Fig. 19) if <strong>the</strong>re are no more than three sites of perforation <strong>and</strong><br />
