The surgeon oversees anes<strong>the</strong>sia while operat<strong>in</strong>g. If additional bodies are necessary<br />

<strong>in</strong> <strong>the</strong> operat<strong>in</strong>g room, nearly anybody can learn quickly how to bag a patient<br />

or hold a retractor. It has been encourag<strong>in</strong>g to witness <strong>the</strong> speed at which people<br />

learn <strong>the</strong>se new tasks.<br />

Anes<strong>the</strong>sia<br />

Sp<strong>in</strong>al anes<strong>the</strong>sia with xyloca<strong>in</strong>e <strong>and</strong> bupivica<strong>in</strong>e <strong>and</strong> regional blocks are frequently<br />

utilized because <strong>the</strong>y require m<strong>in</strong>imal patient monitor<strong>in</strong>g. When giv<strong>in</strong>g<br />

sp<strong>in</strong>al anes<strong>the</strong>sia, <strong>the</strong> patient is loaded with 500 mL of <strong>in</strong>travenous fluid <strong>and</strong> an<br />

alpha-agonist vasocontrictor should be kept available for rapid adm<strong>in</strong>istration. The<br />

surgeon can monitor <strong>the</strong> patient’s airway by leav<strong>in</strong>g his or her head uncovered. If<br />

general anes<strong>the</strong>sia is required, a comb<strong>in</strong>ation is typically chosen from a selection of<br />

thiopental, ketam<strong>in</strong>e, succ<strong>in</strong>ylchol<strong>in</strong>e, e<strong>the</strong>r <strong>and</strong> halothane.<br />

Rural Camps<br />

When sett<strong>in</strong>g up surgical camps <strong>in</strong> rural areas, a team of three <strong>in</strong>dividuals is also<br />

sufficient. A surgeon <strong>and</strong> an assistant perform <strong>the</strong> cases, while <strong>the</strong> nurse is typically<br />

<strong>in</strong> charge of equipment <strong>and</strong> acts as a circulator. In addition, a local health worker<br />

may be used for monitor<strong>in</strong>g of <strong>the</strong> patient.<br />

Methods of Cop<strong>in</strong>g with Deficits <strong>in</strong> Equipment<br />

Equipment shortages can be h<strong>and</strong>led by reus<strong>in</strong>g everyth<strong>in</strong>g possible. Described<br />

here are methods we have used for sterilization of equipment at <strong>the</strong> Lady Will<strong>in</strong>gdon<br />

Hospital <strong>and</strong> dur<strong>in</strong>g our surgical camps.<br />

Gloves<br />

After use, gloves are thoroughly washed <strong>and</strong> allowed to dry <strong>in</strong> <strong>the</strong> sun (Fig. 1).<br />

<strong>the</strong>y are <strong>the</strong>n soaked <strong>in</strong> a steriliz<strong>in</strong>g solution <strong>and</strong> autoclaved for ten m<strong>in</strong>utes. The<br />

gloves used dur<strong>in</strong>g surgical procedures are made of relatively thick latex, similar to<br />

orthopedic gloves; th<strong>in</strong>ner latex gloves do not withst<strong>and</strong> <strong>the</strong> autoclav<strong>in</strong>g process.<br />

Even when thick gloves are used, it is important to <strong>in</strong>spect <strong>the</strong>m for <strong>in</strong>tegrity follow<strong>in</strong>g<br />

sterilization.<br />

Gauze<br />

Gauze can also be resterilized <strong>and</strong> reused. It is washed, bleached, refolded <strong>and</strong><br />

<strong>the</strong>n autoclaved. If it is too shabby to use as a gauze sponge it can be used for o<strong>the</strong>r<br />

purposes. For example, it can be transformed <strong>in</strong>to a wad by ty<strong>in</strong>g it <strong>in</strong>to a ball, <strong>and</strong><br />
