experience of <strong>in</strong>dividual patients, however, but <strong>in</strong> iteration become more than <strong>the</strong><br />

sum of <strong>in</strong>dividual improvements with<strong>in</strong> <strong>the</strong> community comprised of <strong>the</strong>se <strong>in</strong>dividuals<br />

previously suffer<strong>in</strong>g hypothyroidism. Some of <strong>the</strong>se functional consequences<br />

may be surpris<strong>in</strong>g when considered collectively even if predictable when viewed<br />

<strong>in</strong>dividually s<strong>in</strong>ce correction of hypothyroidism allows <strong>in</strong>creased energy output <strong>and</strong><br />

development, but also dem<strong>and</strong>s <strong>in</strong>creased caloric <strong>in</strong>put <strong>and</strong> creates remarkably <strong>in</strong>creased<br />

fertility <strong>and</strong> enhanced resource consumption.<br />

The measurements of hypothyroidism, goiter <strong>and</strong> fertility <strong>in</strong> <strong>the</strong> neighbor<strong>in</strong>g<br />

Efe <strong>and</strong> Lese population showed that <strong>the</strong>y were similarly affected with hypothyroidism<br />

(Table 3). 25 Fertility was low <strong>in</strong> both <strong>the</strong> study population at <strong>the</strong> outset of <strong>the</strong><br />

survey <strong>and</strong> <strong>in</strong> <strong>the</strong> Pygmy population as exhibited <strong>in</strong> <strong>the</strong> Table 4. 40 Fertility is multifactorial,<br />

with component parts be<strong>in</strong>g <strong>the</strong> rate of secondary <strong>in</strong>fertility based <strong>in</strong> pelvic<br />

<strong>in</strong>fectious disease, caloric sufficiency with adequate nutrients to accumulate a fat<br />

surplus <strong>in</strong> women sufficient to commence <strong>and</strong> susta<strong>in</strong> ovulation, <strong>and</strong> o<strong>the</strong>r factors<br />

such as duration of breastfeed<strong>in</strong>g. 41 There is no evidence that <strong>the</strong> rate of pelvic <strong>in</strong>flammatory<br />

disease or breastfeed<strong>in</strong>g practices changed dur<strong>in</strong>g <strong>the</strong> study <strong>in</strong>terval <strong>in</strong><br />

<strong>the</strong> population under surveillance, but fertility certa<strong>in</strong>ly <strong>in</strong>creased.<br />

Not only did fertility <strong>in</strong>crease, but maternal <strong>and</strong> <strong>in</strong>fant mortality decreased, result<strong>in</strong>g<br />

<strong>in</strong> an overall <strong>in</strong>crease <strong>in</strong> <strong>the</strong> population. Each of <strong>the</strong>se population members<br />

were also <strong>in</strong>creased consumers, s<strong>in</strong>ce caloric <strong>in</strong>take adequate for <strong>the</strong> m<strong>in</strong>imal energy<br />

output of a cret<strong>in</strong> is not adequate to susta<strong>in</strong> a person with normal thyroid status <strong>and</strong><br />

normal metabolic dem<strong>and</strong>s. The caloric requirement <strong>in</strong>creased by at least a third for<br />

each of <strong>the</strong> <strong>in</strong>dividuals treated when <strong>the</strong>y were relieved of hypothyroidism, <strong>and</strong> <strong>the</strong><br />

<strong>in</strong>creased number of <strong>the</strong>se enhanced consumers has meant greater dem<strong>and</strong> on <strong>the</strong><br />

environment with<strong>in</strong> <strong>the</strong> study area. Food production from that which is grown or<br />

ga<strong>the</strong>red was marg<strong>in</strong>al at <strong>the</strong> time of <strong>the</strong> orig<strong>in</strong>al survey <strong>and</strong> not remarkably <strong>in</strong>creased<br />

despite efforts to do so follow<strong>in</strong>g medical <strong>in</strong>tervention <strong>and</strong> <strong>the</strong> notable population<br />

<strong>in</strong>crease dur<strong>in</strong>g <strong>the</strong> period of <strong>the</strong> study.<br />

The effects of medical <strong>in</strong>tervention on <strong>the</strong> society as a whole have tempered <strong>the</strong><br />

conclusions of an overall success <strong>in</strong> terms of <strong>in</strong>dividual improvement follow<strong>in</strong>g<br />

medical <strong>in</strong>tervention. As dramatic as <strong>the</strong> measurements of morphologic markers <strong>in</strong><br />

<strong>in</strong>dividuals, <strong>the</strong> functional consequences were no less impressive both for <strong>in</strong>dividu-
