In this region, endemic hypothyroidism is both highly prevalent <strong>and</strong> profound<br />

<strong>in</strong> its consequences on <strong>the</strong> juvenile <strong>and</strong> adult members of this affected community.<br />

This society <strong>and</strong> its members are as severely affected as any group on earth accord<strong>in</strong>g<br />

to <strong>the</strong> st<strong>and</strong>ards of <strong>the</strong> International Council for Control of Iod<strong>in</strong>e Deficiency Disorders<br />

(ICCIDD) <strong>and</strong> should, <strong>the</strong>refore, be significant beneficiaries of The <strong>World</strong><br />

Health Assembly resolution at <strong>the</strong> 43rd assembly <strong>in</strong> May of 1990 to elim<strong>in</strong>ate iod<strong>in</strong>e<br />

deficiency disease by <strong>the</strong> year 2000. Before implementation of this goal, however,<br />

a survey of a very significant endemia such as that described <strong>in</strong> this Uele region<br />

under study <strong>in</strong> <strong>the</strong> Congo could assess not only <strong>the</strong> impact of <strong>the</strong> disorder with<strong>in</strong><br />

<strong>the</strong> population but also what might be expected to result from successful <strong>in</strong>tervention<br />

<strong>in</strong> treat<strong>in</strong>g this disorder.<br />

Micronutrient repletions might not be <strong>the</strong> only th<strong>in</strong>g go<strong>in</strong>g on <strong>in</strong> <strong>the</strong> society<br />

dur<strong>in</strong>g <strong>the</strong> period of this observation. But some estimation can be made as to whe<strong>the</strong>r<br />

<strong>the</strong> disease, or its treatment by iod<strong>in</strong>e repletion alone unmitigated by any o<strong>the</strong>r<br />

<strong>in</strong>troduction of micro- or macronutrient supplementation, might be maladaptive<br />

while not<strong>in</strong>g any o<strong>the</strong>r concurrent programs <strong>in</strong> o<strong>the</strong>r micronutrient enrichment,<br />

macronutrient expansion, <strong>and</strong> general development assistance. In addition to iod<strong>in</strong>e<br />

repletion, rais<strong>in</strong>g <strong>the</strong> metabolic rate by iod<strong>in</strong>e correction of hypothyroidism<br />

might be mitigated by an <strong>in</strong>crease <strong>in</strong> caloric <strong>in</strong>take concomitant with iod<strong>in</strong>e adm<strong>in</strong>istration.<br />

In some isolated societies such as a few of those represented <strong>in</strong> Central Africa,<br />

<strong>the</strong> Equatorial ra<strong>in</strong> forest is an environment <strong>in</strong> which water, fuel energy <strong>and</strong><br />

calorie-dilute food stock are adequate <strong>and</strong> accessible, yet human energy resources<br />

are relatively less well developed. This has led <strong>the</strong>se cultures to subsist on what can<br />

be extracted or produced locally without a good deal of <strong>the</strong>ir consumption be<strong>in</strong>g<br />

based on tradegoods that might be brought <strong>in</strong> from elsewhere, s<strong>in</strong>ce few exportable<br />

unique resources are available <strong>in</strong> <strong>the</strong>se environments.<br />

Extricative <strong>in</strong>dustry is very limited, s<strong>in</strong>ce <strong>the</strong> forest does not recover after timber<strong>in</strong>g,<br />

no pastur<strong>in</strong>g or agronomy is productive because of <strong>the</strong> poverty of <strong>the</strong> soil<br />

<strong>and</strong> abundance of predators, m<strong>in</strong>eral resources are absent here compared with <strong>the</strong><br />

wealthier “Copper Belt” region elsewhere <strong>in</strong> Zaire <strong>in</strong> <strong>the</strong> Kasai Prov<strong>in</strong>ce, <strong>and</strong> fruits,<br />

rubber or o<strong>the</strong>r exotica have not thrived when <strong>in</strong>troduction was attempted.<br />

There are only two native substances <strong>in</strong> <strong>the</strong> environment for which a world<br />

market exists—one animal <strong>and</strong> one vegetable, extracted from <strong>the</strong> root of an <strong>in</strong>digenous<br />

tree. The first is “White Gold” <strong>the</strong> ivory from elephant poach<strong>in</strong>g that has<br />

been <strong>the</strong> highest “value added” <strong>in</strong>dustry, <strong>and</strong> <strong>the</strong> second is “rauwolfia” <strong>the</strong> root extract<br />

formerly used to produce anti-hypertensives. The <strong>in</strong>ternational pressures to<br />

devalue <strong>the</strong> ivory trade had resulted <strong>in</strong> <strong>in</strong>creased <strong>in</strong>tensity of poach<strong>in</strong>g younger bulls<br />

<strong>and</strong> elephant cows to keep up with <strong>the</strong> fall<strong>in</strong>g prices as <strong>the</strong> herd was decimated;<br />

rauwolfia has been preempted by newer generations of syn<strong>the</strong>tic anti-hypertensives<br />

<strong>and</strong> rauwolfia harvest<strong>in</strong>g is now obsolete. The collapse of <strong>the</strong>se two external markets<br />

has returned <strong>the</strong> population to <strong>the</strong> subsistence that has always been its ma<strong>in</strong>stay,<br />

with <strong>the</strong> very rare export of <strong>the</strong> only rema<strong>in</strong><strong>in</strong>g forest product, palm oil, <strong>in</strong>adequate<br />

<strong>in</strong> yield to ma<strong>in</strong>ta<strong>in</strong> trade routes through which it can be transported.<br />

This pattern of subsistence, particularly if marg<strong>in</strong>al, has favored less complex<br />

societies that appear to be less economically developed by Western st<strong>and</strong>ards. This<br />

underdevelopment could result from deficiencies <strong>in</strong> natural resources or <strong>in</strong> human<br />

capital <strong>and</strong> energy resources whe<strong>the</strong>r <strong>in</strong>ternal or external to <strong>the</strong> society.<br />
