roidism is maladaptive, how is it that it has been conserved? And, particularly s<strong>in</strong>ce<br />

<strong>the</strong> modern advent of global health programs for prevention of serious disease, why<br />

cannot this disorder be eradicated? S<strong>in</strong>ce such a goal has been stated <strong>and</strong> acted upon<br />

with a timetable set for elim<strong>in</strong>ation of hypothyroidism, what are <strong>the</strong> likely possibilities<br />

that it will be successful, <strong>and</strong> what are <strong>the</strong> probable consequences if it should<br />

succeed or fail?<br />

How Can Hypothyroidism Be Studied?<br />

Prior to any underst<strong>and</strong><strong>in</strong>g of how deficiencies <strong>in</strong> <strong>the</strong> environment <strong>and</strong> human<br />

disorders may be related, <strong>the</strong> best approach is to def<strong>in</strong>e, describe <strong>and</strong> measure aspects<br />

of <strong>the</strong> environment <strong>and</strong> <strong>the</strong> affected society before evaluat<strong>in</strong>g <strong>the</strong> role of one<br />

isolated factor such as iod<strong>in</strong>e <strong>in</strong> human growth <strong>and</strong> potential. Underst<strong>and</strong><strong>in</strong>g how<br />

much deficit is due to <strong>the</strong> identified micronutrient <strong>and</strong> how repletion might change<br />

both <strong>in</strong>dividuals <strong>and</strong> <strong>the</strong> society might come later with a cumulative assessment of<br />

<strong>the</strong> changes <strong>in</strong> form <strong>and</strong> function.<br />

From <strong>the</strong> perspective of my <strong>in</strong>terest <strong>in</strong> <strong>the</strong> biological, environmental <strong>and</strong> cultural<br />

adaptations of remote populations <strong>and</strong> with a professional background <strong>in</strong> surgical<br />

endocr<strong>in</strong>ology, I had sought out <strong>the</strong> most significant endemias of<br />

hypothyroidism to attempt to address <strong>the</strong> questions of <strong>the</strong> endemias’ characterization,<br />

why hypothyroidism is conserved, <strong>and</strong> <strong>the</strong> <strong>in</strong>dividual <strong>and</strong> social consequences<br />

of hypothyroidism <strong>and</strong> its correction. One of <strong>the</strong> most severe endemias so far recorded<br />

on earth is <strong>the</strong> study region of this <strong>in</strong>vestigation <strong>in</strong> Central Africa. The area<br />

selected for this study of endemic hypothyroidism is <strong>in</strong> <strong>the</strong> upper reaches of what<br />

was called “Bas-Uele”, first recognized by colonial district officers when this area<br />

was “Congo Belge” <strong>and</strong> <strong>the</strong>n reported by several Belgian <strong>in</strong>vestigators through prelim<strong>in</strong>ary<br />

surveys. 18<br />

When an isolated factor <strong>in</strong> this region is selected as <strong>the</strong> <strong>in</strong>dependent variable <strong>in</strong><br />

a study of a population, it is possible that o<strong>the</strong>r events <strong>in</strong> <strong>the</strong> environment may be<br />

chang<strong>in</strong>g as well i.e., <strong>the</strong> factor cannot be completely isolated. To identify as completely<br />

as possible covariables <strong>and</strong> to <strong>the</strong> degree possible exam<strong>in</strong>e untreated controls<br />

with<strong>in</strong> <strong>the</strong> study population adds strength to <strong>the</strong> associations depend<strong>in</strong>g from <strong>the</strong><br />

<strong>in</strong>dependent variable—<strong>in</strong> this <strong>in</strong>stance, <strong>the</strong> micronutrient, iod<strong>in</strong>e.<br />

If observations made <strong>in</strong> a population are compared with reported historic data <strong>in</strong><br />

<strong>the</strong> same population, <strong>the</strong>re is no control for <strong>the</strong> cofactors that may be brought <strong>in</strong><br />

with <strong>the</strong> <strong>in</strong>dependent variable. A parallel social group <strong>in</strong> <strong>the</strong> same region <strong>and</strong> an<br />

<strong>in</strong>ternal untreated control group with<strong>in</strong> <strong>the</strong> same population studied contemporaneously<br />

constitute a better control for <strong>the</strong>se unknown cofactors’ confound<strong>in</strong>g <strong>in</strong>fluence.<br />

The same <strong>in</strong>vestigators observed different ethnic groups <strong>in</strong> a different geographic<br />

area <strong>in</strong> <strong>the</strong> population survey. In <strong>the</strong>se o<strong>the</strong>r populations, <strong>the</strong> micronutrient iod<strong>in</strong>e<br />

was not repleted even though o<strong>the</strong>r medical <strong>in</strong>tervention was <strong>in</strong>troduced for services<br />

to <strong>the</strong> o<strong>the</strong>r control population. This second population was compared with<br />

<strong>the</strong> study group under longitud<strong>in</strong>al surveillance.<br />

Where Was This Study Conducted?<br />

This study was conducted <strong>in</strong> an Az<strong>and</strong>e population through longitud<strong>in</strong>al study<br />

of profoundly iod<strong>in</strong>e deficient populations <strong>in</strong> <strong>the</strong> Uele region of nor<strong>the</strong>ast Congo<br />

(at <strong>the</strong> time, designated “Zaire”). This region, def<strong>in</strong>ed below as “Sasal<strong>and</strong>” (because<br />

of <strong>the</strong> hereditary chiefta<strong>in</strong>ship of <strong>the</strong> region bear<strong>in</strong>g <strong>the</strong> Az<strong>and</strong>e family name “Sasa”),<br />

is <strong>in</strong> a remote part of Central Africa <strong>in</strong> which little study or <strong>in</strong>tervention had previously<br />

been carried out or reported.
