<strong>the</strong> dietary vegetable products that conta<strong>in</strong> <strong>the</strong>se thiocyanates through a shift of<br />
dietary starch staples or chang<strong>in</strong>g <strong>the</strong> way some foodstuffs are prepared to reduce<br />
goitrogen content. 4 A third cause of hypothyroidism among those factors known or<br />
suspected <strong>in</strong>clude dietary deficiency of cofactors necessary for reduction of iodide,<br />
<strong>in</strong>clud<strong>in</strong>g a trace element antioxidant, selenium, ei<strong>the</strong>r present <strong>in</strong> a varied balanced<br />
diet or capable of fortification, like iod<strong>in</strong>e, <strong>in</strong> a dietary supplement. 5<br />
Where Is Hypothyroidism Found <strong>and</strong> Why?<br />
Geographic features that correlate with regional iod<strong>in</strong>e deficiency <strong>in</strong>clude soils<br />
of volcanic orig<strong>in</strong> with low iod<strong>in</strong>e (eg, some Indonesian Isl<strong>and</strong>s). 6,7 Soils leached of<br />
iod<strong>in</strong>e by glacial action (eg, Great Lakes, Alp<strong>in</strong>e central Europe, Andes, <strong>the</strong><br />
Himalayas), or leached by periodic <strong>in</strong>undation <strong>and</strong> elution of water-soluble nutrients<br />
(eg, equatorial <strong>and</strong> tropical ra<strong>in</strong> forest) are poor <strong>in</strong> iod<strong>in</strong>e content. In <strong>the</strong> ex-<br />
Zaïre (now renamed <strong>the</strong> Democratic Republic of <strong>the</strong> Congo) study area, <strong>the</strong> comb<strong>in</strong>ation<br />
of lava substrate <strong>and</strong> cyclic <strong>in</strong>undation (leach<strong>in</strong>g) is an explanation for <strong>the</strong><br />
iod<strong>in</strong>e-poor environment. 8 Additionally, this remote Central Africa location is far<br />
removed from natural sources such as iod<strong>in</strong>e-rich seafoods or commercially fortified<br />
products such as iodized salt.<br />
The extent of hypothyroidism is a serious global public health problem. 9 Nearly<br />
a billion people, or a fifth of <strong>the</strong> planet’s <strong>in</strong>habitants, are affected—<strong>the</strong> majority of<br />
<strong>the</strong>m <strong>in</strong> marg<strong>in</strong>al deficiency that <strong>in</strong>hibits <strong>the</strong>ir reach<strong>in</strong>g <strong>the</strong>ir full potential (75%),<br />
but many manifest this hypothyroidism through goiter (20%—or a number approach<strong>in</strong>g<br />
<strong>the</strong> size of <strong>the</strong> US population) <strong>and</strong> a tragic residual of cret<strong>in</strong>ism (3.5<br />
million) who might never develop but who are suffer<strong>in</strong>g a permanent preventable<br />
retardation. 10 In fact, iod<strong>in</strong>e has been called <strong>the</strong> “m<strong>in</strong>d-protector”. 11 The population<br />
affected by hypothyroidism is among <strong>the</strong> poorest of <strong>the</strong> peoples of <strong>the</strong> globe, distributed<br />
<strong>in</strong> those tropical, mounta<strong>in</strong>ous or volcanic regions which are least developed<br />
with populations with <strong>the</strong> least potential for develop<strong>in</strong>g <strong>the</strong> resources of <strong>the</strong>se<br />
environments. 12,13 These foci of “geologically <strong>in</strong>duced goiters” 14 are places where<br />
geologic features allow prediction of probable hypothyroidism; <strong>in</strong> <strong>the</strong> world today,<br />
<strong>the</strong>se areas are found nearly uniformly to be lesser developed regions of <strong>the</strong> world.<br />
This disadvantage is based at least <strong>in</strong> part on <strong>the</strong> lack of energy, ambition <strong>and</strong> resourcefulness<br />
which are characteristic of <strong>the</strong> hypometabolism of hypothyroidism,<br />
sapp<strong>in</strong>g <strong>the</strong> human resources, whatever <strong>the</strong> limited potential of <strong>the</strong> natural resource<br />
environment. 15-17<br />
The <strong>World</strong> Health Assembly has recognized this co<strong>in</strong>cidence of iod<strong>in</strong>e deficiency<br />
disease <strong>in</strong> <strong>the</strong> underdeveloped regions of <strong>the</strong> world <strong>and</strong> at its 43rd assembly <strong>in</strong> May<br />
of 1990 established <strong>the</strong> goal of elim<strong>in</strong>ation of iod<strong>in</strong>e deficiency disease by <strong>the</strong> year<br />
2000. 3 The ICCIDD (International Council for <strong>the</strong> Control of Iod<strong>in</strong>e Deficiency<br />
Disorders) has been work<strong>in</strong>g toward <strong>the</strong> achievement of this global goal, now nearly<br />
approach<strong>in</strong>g <strong>the</strong> deadl<strong>in</strong>e of <strong>the</strong> timetable for its accomplishment.<br />
Consequences of Hypothyroidism<br />
The consequences of hypothyroidism are severe. The causes are known <strong>and</strong> correctable.<br />
Why is it that this disease has persisted? It seems by some co<strong>in</strong>cidence that<br />
it most severely affects <strong>the</strong> poorest of <strong>the</strong> world’s populations <strong>in</strong> <strong>the</strong> areas that are<br />
most resource-depleted, <strong>and</strong> this fact would tend toward a question of whe<strong>the</strong>r this<br />
association was cause or effect. It is true that poverty <strong>and</strong> hypothyroidism appear<br />
congruent on <strong>the</strong> world’s map of geographic medic<strong>in</strong>e. If <strong>the</strong> disease of hypothy-<br />
