Metabolic Maladaptation<br />

Abstract<br />

Endemic hypothyroidism has been studied <strong>in</strong> a Central African population <strong>in</strong><br />

remote Congo (ex-Zaire) to determ<strong>in</strong>e <strong>the</strong> prevalence, severity, causes <strong>and</strong> potential<br />

control of this disorder, with questions as to why this disease is conserved <strong>and</strong> whe<strong>the</strong>r<br />

it confers any adaptive advantage <strong>in</strong> this resource-constra<strong>in</strong>ed environment. Iod<strong>in</strong>e<br />

deficiency, cassava goiterogens <strong>and</strong> selenium deficiency were found to be <strong>the</strong> factors<br />

implicated <strong>in</strong> <strong>the</strong> severe hypothyroidism expressed <strong>in</strong> congenital cret<strong>in</strong>ism <strong>and</strong> high<br />

goiter <strong>in</strong>cidence <strong>in</strong> this isolated population, which cont<strong>in</strong>ues under observation follow<strong>in</strong>g<br />

medical <strong>in</strong>tervention.<br />

Profound hypothyroidism was encountered <strong>in</strong> whole village populations as measured<br />

by serum TSH determ<strong>in</strong>ations rang<strong>in</strong>g from very high to over 1000 I U, <strong>and</strong><br />

thyrox<strong>in</strong> levels rang<strong>in</strong>g from low to undetectable; cret<strong>in</strong>ism rates were as high as<br />

11% <strong>and</strong> goiter <strong>in</strong>cidence approached 100%. Assessment of endocr<strong>in</strong>ologic status,<br />

caloric requirement, energy output, fertility <strong>and</strong> ecologic factors was carried out<br />

before <strong>and</strong> dur<strong>in</strong>g iod<strong>in</strong>e repletion by depot <strong>in</strong>jection.<br />

Hypothyroidism was corrected <strong>and</strong> cret<strong>in</strong>ism elim<strong>in</strong>ated <strong>in</strong> <strong>the</strong> treatment group,<br />

with goiters reduced <strong>in</strong> most <strong>in</strong>stances (with regrowth exhibited <strong>in</strong> some who escaped<br />

control) <strong>and</strong> some symptomatic goiter patients were offered surgical treatment<br />

for respiratory obstruction. Individual patient benefits were remarkable <strong>in</strong><br />

development expressed <strong>in</strong> improved strength <strong>and</strong> energy output.<br />

The social <strong>and</strong> developmental consequences observed with<strong>in</strong> <strong>the</strong> collective groups<br />

of treated patients were remarkable for an <strong>in</strong>crease <strong>in</strong> caloric requirement <strong>and</strong> a<br />

dramatic <strong>in</strong>crease <strong>in</strong> fertility which led to quantitative as well as qualitative <strong>in</strong>crease<br />

<strong>in</strong> resource consumption. Micronutrient iod<strong>in</strong>e repletion was not accompanied by<br />

any concomitant <strong>in</strong>crease <strong>in</strong> macronutrient supply, <strong>and</strong> hunger <strong>and</strong> environmental<br />

degradation resulted.<br />

It may be that <strong>the</strong> highly prevalent disease of hypothyroidism is conserved <strong>in</strong><br />

areas of greatest resource constra<strong>in</strong>t, <strong>in</strong> which it happens to be found <strong>in</strong> highest<br />

<strong>in</strong>cidence, because it may confer adaptive advantage <strong>in</strong> such marg<strong>in</strong>al environments<br />

as an effect as well as cause of underdevelopment. It may limit energy requirements,<br />

fertility, <strong>and</strong> consumer population pressure <strong>in</strong> closed ecosystems that could o<strong>the</strong>rwise<br />

be outstripped. S<strong>in</strong>gle factor <strong>in</strong>tervention <strong>in</strong> a vertical health care program not<br />

sensitive to <strong>the</strong> fragile biologic balance <strong>and</strong> not part of a culture-sensitive development<br />

program might result <strong>in</strong> medical maladaptation.<br />
