palpation. With an assistant support<strong>in</strong>g <strong>the</strong> thigh <strong>and</strong> assur<strong>in</strong>g a maximal<br />

sk<strong>in</strong> surface presentation to <strong>the</strong> sk<strong>in</strong> graft<strong>in</strong>g knife, <strong>the</strong> <strong>in</strong>strument is brought<br />

up <strong>the</strong> thigh from knee to gro<strong>in</strong> with a smooth back <strong>and</strong> forth movement.<br />

The split thickness sk<strong>in</strong> graft is covered with normal sal<strong>in</strong>e soaked sponges.<br />

Float<strong>in</strong>g <strong>the</strong> grafts <strong>in</strong> normal sal<strong>in</strong>e is thought to remove some important<br />

components of <strong>the</strong> graft. When <strong>the</strong> area for graft<strong>in</strong>g is ready, <strong>the</strong> sk<strong>in</strong> grafts<br />

are laid out atraumatically. Circumferential sutures are placed for stabilization.<br />

Small open<strong>in</strong>gs <strong>in</strong> <strong>the</strong> split thickness graft are made prior to application.<br />

The sk<strong>in</strong> graft is placed aga<strong>in</strong>st a sterile wood block while multiple<br />

small <strong>in</strong>cisions are made across its surface with a sharp knife.<br />

5. A dress<strong>in</strong>g is placed over <strong>the</strong> anterior thigh donor site. Use a nonstick vasel<strong>in</strong>e<br />

gauze covered with dry sterile gauze <strong>and</strong> a wrap around b<strong>and</strong>age for <strong>the</strong><br />

donor site. The outer nonvasel<strong>in</strong>e dress<strong>in</strong>g is removed at 5 days.<br />

6. An <strong>in</strong>cision is made over <strong>the</strong> iliac crest from <strong>the</strong> anterior superior iliac sp<strong>in</strong>e<br />

laterally for 8 cm. The periosteum is elevated on <strong>the</strong> superior <strong>and</strong> <strong>in</strong>ferior<br />

surface of <strong>the</strong> iliac w<strong>in</strong>g. Us<strong>in</strong>g a sharp chisel a 6 cm <strong>and</strong> 3 cm segment of<br />

bone is removed.<br />
