<strong>Surgery</strong> <strong>in</strong> Develop<strong>in</strong>g Countries<br />

Practical Considerations<br />

F<strong>in</strong>d<strong>in</strong>g <strong>the</strong> Right Opportunity<br />

One of <strong>the</strong> most important aspects of overseas service is f<strong>in</strong>d<strong>in</strong>g <strong>the</strong> right oppor- 2<br />

tunity for <strong>the</strong> physician or surgeon to use his or her abilities optimally. It is important<br />

also for <strong>the</strong> surgeon to be familiar with <strong>the</strong> quality of postoperative care. For<br />

example, one of <strong>the</strong> major problems <strong>in</strong> most develop<strong>in</strong>g countries for a cardiac<br />

surgeon would be to f<strong>in</strong>d <strong>the</strong> cases <strong>and</strong> <strong>the</strong> diagnostic <strong>and</strong> <strong>the</strong>rapeutic technical <strong>and</strong><br />

nurs<strong>in</strong>g resources needed. A pathologist would scarcely be able to f<strong>in</strong>d enough work<br />

to justify his efforts <strong>in</strong> a small remote facility except by immediate conversion to a<br />

nonspecialist general practitioner. Most commonly <strong>the</strong> greatest needs <strong>in</strong> develop<strong>in</strong>g<br />

countries are for primary care physicians <strong>and</strong> pediatricians, <strong>and</strong> certa<strong>in</strong>ly for general<br />

surgeons <strong>and</strong> some surgical specialists such as orthopedists, ob/gyn surgeons, urologic<br />

<strong>and</strong> plastic surgeons, ENT specialists <strong>and</strong> ophthalmologists. Where possible it<br />

may be helpful to br<strong>in</strong>g support personnel if this is acceptable <strong>in</strong> <strong>the</strong> host hospital or<br />

cl<strong>in</strong>ic. Operat<strong>in</strong>g <strong>and</strong> recovery room nurses are very often able to perform yeoman<br />

service which may compensate <strong>in</strong> part for <strong>in</strong>adequate nurs<strong>in</strong>g on <strong>the</strong> part of <strong>the</strong><br />

nationals.<br />

Operat<strong>in</strong>g Room Experience<br />

Lack of materials <strong>and</strong> equipment are almost <strong>in</strong>evitable. With some of <strong>the</strong> largest<br />

<strong>and</strong> most well-established organizations, this is less apt to be true <strong>in</strong>asmuch as <strong>the</strong>y<br />

are able to br<strong>in</strong>g with <strong>the</strong>m materials <strong>and</strong> equipment <strong>the</strong>y expect to be <strong>in</strong> short<br />

supply. Many smaller missions, however, are constantly short of nearly everyth<strong>in</strong>g,<br />

even such commonly used materials as gloves, sutures, gauze, b<strong>and</strong>ages, syr<strong>in</strong>ges,<br />

etc. If possible, it is advisable for <strong>the</strong> surgeon to br<strong>in</strong>g with him not only such<br />

supplies as are listed above, but also any special <strong>in</strong>struments that he may need <strong>in</strong> his<br />

particular type of practice. In my own experience, I have on occasion wished for<br />

what I thought were very basic <strong>in</strong>struments but very important for a certa<strong>in</strong> procedure,<br />

only to f<strong>in</strong>d that <strong>the</strong>y were not available <strong>and</strong> thus had to do with what was<br />

available <strong>and</strong> as a consequence, with less optimal results achieved. The surgeon should<br />

try to anticipate what he is go<strong>in</strong>g to need for any special procedures <strong>and</strong>, if possible,<br />

br<strong>in</strong>g such <strong>in</strong>struments <strong>and</strong> equipment with him. Aga<strong>in</strong>, when possible, if equipment<br />

can be brought which will be left beh<strong>in</strong>d, this is especially helpful not only <strong>in</strong><br />

<strong>the</strong> surgeon’s own practice, but <strong>in</strong> <strong>the</strong> practice of those of <strong>the</strong> national surgeons who<br />

work <strong>the</strong>re on a cont<strong>in</strong>u<strong>in</strong>g basis. It is almost st<strong>and</strong>ard practice <strong>in</strong> develop<strong>in</strong>g countries<br />

to use disposable supplies <strong>and</strong> equipment (e.g., disposable cauteries) over <strong>and</strong><br />

over aga<strong>in</strong>.<br />

If <strong>the</strong> volunteer is br<strong>in</strong>g<strong>in</strong>g drugs, he/she should be aware that <strong>in</strong> some countries<br />

<strong>the</strong>re are strict customs regulations prohibit<strong>in</strong>g <strong>the</strong> <strong>in</strong>troduction of outdated drugs.<br />

S<strong>in</strong>ce many drugs are perfectly safe <strong>and</strong> usable for several months after <strong>the</strong> expiration<br />

date, repackag<strong>in</strong>g may circumvent this problem when it seems appropriate.<br />

An affidavit from a parent organization <strong>in</strong>dicat<strong>in</strong>g that <strong>the</strong> materials are for charitable<br />

use with some sort of official-look<strong>in</strong>g stamp may be very helpful <strong>in</strong> pass<strong>in</strong>g<br />

through customs.<br />

Use of Time <strong>and</strong> O<strong>the</strong>r Concerns <strong>in</strong> <strong>the</strong> Operat<strong>in</strong>g Room<br />

Many who practice surgery <strong>in</strong> <strong>the</strong> U.S. are accustomed to a relatively rapid turnover<br />

between cases or <strong>the</strong> use of more than one operat<strong>in</strong>g room to stagger cases <strong>and</strong><br />
