Preparation Time<br />

prom<strong>in</strong>ent official stamp usually makes <strong>the</strong> document more acceptable to <strong>the</strong> bureaucracy<br />

of Third <strong>World</strong> countries.<br />

Immunizations. Generally speak<strong>in</strong>g, <strong>the</strong>re are very few immunization requirements<br />

to enter any country <strong>in</strong> <strong>the</strong> world. One exception is <strong>the</strong> yellow fever vacc<strong>in</strong>ation.<br />

Many countries do require it of all <strong>in</strong>com<strong>in</strong>g travelers. O<strong>the</strong>rs require it only if<br />

a person is travell<strong>in</strong>g from a country where yellow fever is endemic. Yellow Fever is<br />

found <strong>in</strong> much of Africa, (exclud<strong>in</strong>g some parts of both North <strong>and</strong> South Africa) It<br />

is also found <strong>in</strong> many parts of South America. Yellow fever vacc<strong>in</strong>ation is available at<br />

county health departments <strong>and</strong> usually requires an appo<strong>in</strong>tment ra<strong>the</strong>r than walk-<strong>in</strong>.<br />

But, even if <strong>the</strong>re are few requirements for travel, many immunizations may<br />

<strong>in</strong>deed be strongly recommended. First of all, one’s basic immunizations should be<br />

up to date. This <strong>in</strong>cludes tetanus-diph<strong>the</strong>ria, polio, <strong>and</strong> for those persons born after<br />

1957, MMR (mumps-measles-rubella) as well. Tetanus-diph<strong>the</strong>ria is good for 10<br />

years, but if one is com<strong>in</strong>g at all close to <strong>the</strong> 10 year time, it would be a good idea to<br />

update it <strong>and</strong> be relieved of <strong>the</strong> necessity of a booster on <strong>the</strong> field if an <strong>in</strong>jury should<br />

occur. There is a global polio eradication scheme <strong>in</strong> operation, but this has not yet<br />

been achieved. Polio should be boosted <strong>and</strong> IPV (as opposed to OPV) should be<br />

used to bypass <strong>the</strong> very small risk of contract<strong>in</strong>g polio from <strong>the</strong> live virus. MMR too<br />

should be boosted if this has not been done s<strong>in</strong>ce childhood. Hepatitis A vacc<strong>in</strong>e<br />

should be given: <strong>the</strong> series consists of two shots 6-12 mos. apart but <strong>the</strong> first dose<br />

will protect for short-term travels to Third <strong>World</strong> countries if given 3 weeks before<br />

travell<strong>in</strong>g. Do<strong>in</strong>g medical work makes hepatitis B protection m<strong>and</strong>atory <strong>and</strong> most<br />

of us assume that all medical personnel have this protection. However, a recent<br />

CDC report shows that as many as 30% of medical personnel (those fall<strong>in</strong>g outside<br />

of OSHA regulations) have not been immunized. It should be noted too that Hepatitis<br />

B is endemic <strong>in</strong> <strong>the</strong> orient mak<strong>in</strong>g it doubly important to have on board if<br />

work<strong>in</strong>g <strong>in</strong> this part of <strong>the</strong> world. Travell<strong>in</strong>g <strong>and</strong> work<strong>in</strong>g <strong>in</strong> <strong>the</strong> middle belt of<br />

Africa encompasses <strong>the</strong> so called “men<strong>in</strong>gitis belt” <strong>and</strong> <strong>the</strong> polyvalent vacc<strong>in</strong>e<br />

Menomune (Sanofi Pasteur makes this vacc<strong>in</strong>e) should be adm<strong>in</strong>istered. Duration<br />

of protection is not precisely known, but this should be repeated every 3 years or<br />

<strong>the</strong>reabouts if travell<strong>in</strong>g to this area.<br />

Immunization aga<strong>in</strong>st typhoid is available <strong>in</strong> both oral <strong>and</strong> <strong>in</strong>jectable form. The<br />

oral product is manufactured <strong>in</strong> Switzerl<strong>and</strong> <strong>and</strong> is available through Berna Products<br />

Corp located <strong>in</strong> Coral Gables, FL. It is a live vacc<strong>in</strong>e <strong>and</strong> needs to be repeated<br />

only every seven years. Use of <strong>the</strong> oral preparation is preferable. There are two <strong>in</strong>jectable<br />

vacc<strong>in</strong>es available, one manufactured by Pasteur Merieux Connaught is s<strong>in</strong>gle<br />

dose while <strong>the</strong> Wyeth Ayerst product requires two <strong>in</strong>jections separated by one month.<br />

Cholera vacc<strong>in</strong>e gives little protection, is somewhat reactogenic <strong>and</strong> <strong>the</strong>refore its use<br />

is not advised. There have been <strong>in</strong>stances <strong>in</strong> Third <strong>World</strong> countries (<strong>in</strong> response to a<br />

cholera epidemic <strong>in</strong> that country) <strong>in</strong> which cholera immunizations have been suddenly<br />

m<strong>and</strong>ated. It may be wise to carry with you a note from your personal physician<br />

say<strong>in</strong>g that you should not have this immunization for medical reasons. If <strong>the</strong><br />

status of protection aga<strong>in</strong>st hepatitis A <strong>and</strong> B is unclear, antibody titers should be<br />

drawn to clarify this issue.<br />

If work<strong>in</strong>g <strong>in</strong> <strong>the</strong> Far East, one should check with <strong>the</strong> CDC for <strong>the</strong>ir recommendation<br />

regard<strong>in</strong>g Japanese encephalitis vacc<strong>in</strong>ation.<br />

There are a number of simple items that should be carried with you s<strong>in</strong>ce purchas<strong>in</strong>g<br />

<strong>the</strong>m on site may not be easily possible. These <strong>in</strong>clude simple pa<strong>in</strong> medications,<br />

good quality sunglasses, mosquito repellent (should conta<strong>in</strong> DEET), sunscreen<br />
