autoclave tape or o<strong>the</strong>r markers can assure full sterilization. The author spent some<br />

time at a hospital <strong>in</strong> <strong>the</strong> ra<strong>in</strong> forest where all autoclav<strong>in</strong>g was done with a pressure<br />

cooker on a wood-burn<strong>in</strong>g stove.<br />

Gas sterilization is very useful for items that cannot tolerate <strong>the</strong> high pressure<br />

steam of autoclav<strong>in</strong>g. Yet <strong>the</strong> actual equipment for this can be very expensive. And<br />

<strong>the</strong> st<strong>and</strong>ard chemical used for this, ethylene oxide, is a very flammable solution that<br />

cannot be transported by air <strong>and</strong> is unavailable <strong>in</strong> many develop<strong>in</strong>g countries. Every<br />

day <strong>in</strong> many places <strong>in</strong> <strong>the</strong> develop<strong>in</strong>g world disposable equipment is used <strong>and</strong> discarded<br />

after each operation, often at a cost of several hundred dollars (US). However,<br />

equipment such as electrocautery pencils <strong>and</strong> laparoscopic trocars are be<strong>in</strong>g<br />

reused <strong>in</strong> hospitals with severe cost restra<strong>in</strong>ts after sterilization with formaldehyde.<br />

Several studies have confirmed <strong>the</strong> efficacy of re-use. A very economical chamber for<br />

this sterilization is an old refrigerator with a good seal on <strong>the</strong> door. Leav<strong>in</strong>g equipment<br />

overnight <strong>in</strong> a sta<strong>in</strong>less steel pan, exposed to 5% formal<strong>in</strong> fumes or paraformaldehyde<br />

tablets <strong>in</strong> a refrigerator can be a major cost-sav<strong>in</strong>g practice. One protocol<br />

uses 7 g of formal<strong>in</strong> tablets per cubic meter. A light bulb <strong>in</strong> <strong>the</strong> refrigerator helps <strong>the</strong><br />

vaporization process. The area around <strong>the</strong> refrigerator must be well-ventilated for<br />

<strong>the</strong> periods when <strong>the</strong> chamber is emptied <strong>and</strong> <strong>the</strong> sterilized equipment r<strong>in</strong>sed with<br />

sterile water or sal<strong>in</strong>e <strong>and</strong> aerated. A m<strong>in</strong>imum of 12 h is recommended, <strong>and</strong> all<br />

organisms are killed with<strong>in</strong> 24 h, <strong>in</strong>clud<strong>in</strong>g HIV. Some hospitals will use soaks dur<strong>in</strong>g<br />

<strong>the</strong> day for equipment such as laparoscopes, cystoscopes <strong>and</strong> connect<strong>in</strong>g cords<br />

but do a gas sterilization with formal<strong>in</strong> at night. Equipment such as disposable<br />

trocars <strong>and</strong> cautery pencils will be used only once between formal<strong>in</strong> sterilizations.<br />

Most cost-conscious hospitals where labor costs are quite <strong>in</strong>expensive reuse sterile<br />

gloves. The heavier orthopedic gloves are especially useful for this practice. And,<br />

on <strong>the</strong> o<strong>the</strong>r h<strong>and</strong>, good surgical gloves should not be wasted on rout<strong>in</strong>e dress<strong>in</strong>g<br />

changes. Instead, plastic bread bags which are cheap <strong>and</strong> readily available are excellent<br />

for dress<strong>in</strong>g changes, do<strong>in</strong>g rectal exams <strong>and</strong> for generalized protection of <strong>the</strong><br />

exam<strong>in</strong>er for evaluat<strong>in</strong>g such areas as gro<strong>in</strong> <strong>and</strong> axilla or for o<strong>the</strong>r contacts with<br />

bodily secretions. A pocket <strong>in</strong> <strong>the</strong> white coat should be kept full of <strong>the</strong>m.<br />

In addition to strict sterile technique, high quality surgery with good outcomes<br />

requires high quality monitor<strong>in</strong>g <strong>in</strong> <strong>the</strong> <strong>the</strong>ater. Cont<strong>in</strong>uous electrocardiography<br />

may not be available <strong>in</strong> some centers, but all hospitals should <strong>in</strong>vest <strong>in</strong> a cont<strong>in</strong>uous<br />

oximetry monitor. These monitors carry a warn<strong>in</strong>g that <strong>the</strong>y should not be used<br />

with flammable anes<strong>the</strong>tics. If e<strong>the</strong>r is a ma<strong>in</strong>stay <strong>in</strong>halation for a hospital, <strong>the</strong> battery-operated<br />

rechargeable model is probably safe <strong>and</strong> will be one of <strong>the</strong> best<br />

$US1,000 <strong>in</strong>vestments <strong>the</strong> hospital can make. Regular blood pressure monitor<strong>in</strong>g<br />

<strong>and</strong> cont<strong>in</strong>uous auscultation of <strong>the</strong> precordium through a s<strong>in</strong>gle earpiece stethoscope<br />

add to safety.<br />

The cost-conscious hospital can save a great deal on suture <strong>in</strong> a number of ways.<br />

In places where nylon might be used, fish<strong>in</strong>g-l<strong>in</strong>e will substitute nicely. Sk<strong>in</strong> can be<br />

closed with 4 pound test l<strong>in</strong>e. Fascia can be closed with 12 pound test l<strong>in</strong>e. If retention<br />

sutures are desired, 20 pound l<strong>in</strong>e works well (<strong>and</strong> large buttons from <strong>the</strong> market<br />

work well as bridges for ty<strong>in</strong>g <strong>the</strong>se). At least four square knots (5 throws) should<br />

be used for <strong>the</strong> fish-l<strong>in</strong>e. A board 18-<strong>in</strong>ches long with a V-notch cut at each end is<br />

used to wrap <strong>the</strong> nylon or to lay it straight for autoclav<strong>in</strong>g. It is best to autoclave<br />

only that amount of nylon needed for one case so that autoclav<strong>in</strong>g is not repeatedly<br />

done on <strong>the</strong> same nylon. The nylon is threaded onto small cutt<strong>in</strong>g needles for <strong>the</strong><br />

sk<strong>in</strong> <strong>and</strong> round-body needles elsewhere. The author <strong>and</strong> many colleagues have used
