S<strong>in</strong>ce much of <strong>the</strong> population cannot travel to <strong>the</strong> surgical facilities, we have<br />

envisaged <strong>and</strong> developed a program of mobile surgery. Initially we faced objections<br />

<strong>and</strong> criticisms <strong>in</strong>clud<strong>in</strong>g concern about <strong>in</strong>fections, lack of blood transfusions, etc. In<br />

fact, it is recognized that nonhospital bacteria are less resistant to antibiotics than<br />

hospital acquired organisms. Blood transfusions are required extremely seldom. Subsequent<br />

analysis of outcomes have laid to rest <strong>the</strong> objections.<br />

We built an operat<strong>in</strong>g room <strong>in</strong> a 24 foot van. A larger truck could not negotiate<br />

<strong>the</strong> narrow w<strong>in</strong>d<strong>in</strong>g Andean roads <strong>in</strong> Ecuador. It conta<strong>in</strong>s a small operat<strong>in</strong>g room<br />

with adequate light<strong>in</strong>g, anes<strong>the</strong>sia equipment, electrosurgical unit <strong>and</strong> basic monitor<strong>in</strong>g<br />

<strong>in</strong>struments. Ano<strong>the</strong>r room is for <strong>the</strong> surgeons to scrub <strong>and</strong> for stor<strong>in</strong>g medical<br />

supplies. There are two tents, with oxygen, suction <strong>and</strong> monitor<strong>in</strong>g devices: one<br />

for preoperative preparation <strong>and</strong> one for postoperative care.<br />

A local doctor performs <strong>in</strong>itial screen<strong>in</strong>g of patients. A complete history <strong>and</strong> a<br />

physical exam<strong>in</strong>ation are performed <strong>and</strong> <strong>in</strong>formed consent is acquired. On a set<br />

schedule <strong>the</strong> mobile unit sets out to <strong>the</strong> designated villages <strong>and</strong> sets up <strong>in</strong> a suitable<br />

location <strong>in</strong> <strong>the</strong> village to perform <strong>the</strong> scheduled operations as well as any emergency<br />

procedures that are required. Sometimes surgeries are performed <strong>in</strong> small country<br />

hospitals when facilities permit. Locations visited have <strong>in</strong>cluded sites <strong>in</strong> <strong>the</strong> Amazon<br />

jungles, coastal lowl<strong>and</strong>s, <strong>and</strong> high Andean mounta<strong>in</strong> regions. Practical obstacles<br />

have <strong>in</strong>cluded road obstructions due to l<strong>and</strong>slides <strong>and</strong> o<strong>the</strong>r unexpected barriers.<br />

On one occasion an overhang<strong>in</strong>g rock ledge had to be dynamited to permit <strong>the</strong><br />

truck to pass.<br />

Besides <strong>the</strong> provision of essential surgical care that would not o<strong>the</strong>rwise be available,<br />

<strong>the</strong> program <strong>in</strong>cludes research, teach<strong>in</strong>g, <strong>and</strong> telemedic<strong>in</strong>e consultations. The<br />

research has demonstrated <strong>the</strong> effectiveness <strong>and</strong> safety of this approach as well as<br />

acquir<strong>in</strong>g valuable statistical <strong>in</strong>formation. The teach<strong>in</strong>g component has been valuable<br />

for both Ecuadorian <strong>and</strong> foreign students.<br />

Surgeons are concerned about <strong>the</strong> problem of unmet surgical needs <strong>in</strong> our world<br />

as demonstrated by <strong>the</strong> almost 600 Fellows <strong>and</strong> Residents who filled <strong>the</strong> auditorium<br />

for a panel discussion <strong>in</strong> <strong>the</strong> 2000 Congress of <strong>the</strong> American College of Surgeons.<br />

The many comments <strong>and</strong> questions confirmed <strong>the</strong> conviction that surgeons are<br />

“doers” with a great desire to be helpful. Unfortunately, <strong>the</strong>re was <strong>in</strong>sufficient time<br />

to deal with <strong>the</strong> large number of <strong>in</strong>sightful suggestions. Some of <strong>the</strong> <strong>the</strong>mes that<br />

arose dur<strong>in</strong>g <strong>the</strong> discussion period are as follows:<br />

• Establish a list of resources for service <strong>in</strong> less-developed countries. This<br />

could be published periodically <strong>in</strong> <strong>the</strong> Bullet<strong>in</strong> <strong>and</strong> added to <strong>the</strong> ACS<br />

Web page.<br />

• Provide l<strong>in</strong>ks <strong>in</strong> <strong>the</strong> Web site to organizations <strong>in</strong>volved <strong>in</strong> needy regions.<br />

• Establish a list of activities that <strong>the</strong> ACS is do<strong>in</strong>g for underserved regions<br />

<strong>and</strong> populations<br />

• Establish a booth for International <strong>Surgery</strong> <strong>in</strong> <strong>the</strong> Scientific Displays at<br />

<strong>the</strong> annual Congress meet<strong>in</strong>gs. Invite <strong>in</strong>ternational organizations to participate.<br />

• Develop a register of experienced <strong>and</strong> available advisers to help develop or<br />

design susta<strong>in</strong>able tra<strong>in</strong><strong>in</strong>g programs suitable for needy regions.<br />
