Medical Education (CME) Program for <strong>the</strong> tra<strong>in</strong><strong>in</strong>g <strong>and</strong> retra<strong>in</strong><strong>in</strong>g of core<br />

staff <strong>in</strong> a surgery unit should be an essential feature of <strong>the</strong> curriculum.<br />

Management Skills<br />

O<strong>the</strong>r factors which affect <strong>the</strong> st<strong>and</strong>ards of surgical tra<strong>in</strong><strong>in</strong>g <strong>and</strong> practice particularly<br />

<strong>in</strong> develop<strong>in</strong>g countries <strong>in</strong>clude <strong>the</strong> poor or misguided management of<br />

scarce resources. Management skills should <strong>in</strong>clude strategic plann<strong>in</strong>g <strong>and</strong> evaluation,<br />

personnel man<br />

Manpower<br />

Du Plessis identified a sound economy as “<strong>the</strong> primary requirement” <strong>in</strong> health<br />

development. 6 The need to focus on manpower development is crucial given <strong>the</strong><br />

appall<strong>in</strong>g surgeon/patient ratios <strong>in</strong> nearly all Third <strong>World</strong> countries. Manpower is<br />

more than <strong>the</strong> availability of doctors or surgeons. A critical mass of health workers is<br />

necessary to provide <strong>the</strong> environment for learn<strong>in</strong>g <strong>and</strong> tra<strong>in</strong><strong>in</strong>g. Even <strong>the</strong>n, <strong>the</strong> available<br />

personnel is <strong>in</strong>appropriately distributed between <strong>the</strong> urban <strong>and</strong> rural areas.<br />

Fund<strong>in</strong>g<br />

The persistent underfund<strong>in</strong>g of healthcare affects <strong>the</strong> performance <strong>and</strong> motivation<br />

which can be expected from <strong>the</strong> educator <strong>and</strong> <strong>the</strong> extent to which <strong>the</strong> tra<strong>in</strong>ed<br />

‘core’ staff can be reta<strong>in</strong>ed.<br />

Conceptual Failure<br />

The new emphasis on Primary Health Care (PHC) complicated <strong>the</strong> issue of<br />

resource allocation with<strong>in</strong> <strong>the</strong> dim<strong>in</strong>ish<strong>in</strong>g resources of Third <strong>World</strong> countries. It<br />

brought with it <strong>the</strong> misconception of an <strong>in</strong>expensive, affordable, accessible <strong>and</strong> efficacious<br />

health system. A health system was advocated especially for develop<strong>in</strong>g coun-<br />
