Diazepam causes an acute phlebitis which can be m<strong>in</strong>imized by giv<strong>in</strong>g <strong>the</strong> drugs<br />

slowly (1 mg every 2 m<strong>in</strong>utes). The dose of drugs depends on <strong>the</strong> cl<strong>in</strong>ical sett<strong>in</strong>g<br />

(size of patient <strong>and</strong> history of prior drug use). In over 15 years’ use of this technique<br />

<strong>the</strong> author has not seen a case of respiratory depression or anaphylactic reaction.<br />

The commonest side effect is that of sk<strong>in</strong> irritation.<br />

Cervical block (10 ml of 1% or 2% lidoca<strong>in</strong>e solution) is performed by direct<br />

<strong>in</strong>jection of <strong>the</strong> drug at <strong>the</strong> fifth <strong>and</strong> seventh o’clock (5th <strong>and</strong> 7th) position on <strong>the</strong><br />

cervix. It is important to ensure direct <strong>in</strong>jection <strong>in</strong>to <strong>the</strong> tissue ra<strong>the</strong>r than <strong>in</strong>travenously.<br />

This can be avoided by <strong>the</strong> usual precaution of <strong>in</strong>tramuscular <strong>in</strong>jections,<br />

pull<strong>in</strong>g back on <strong>the</strong> plunger before <strong>in</strong>ject<strong>in</strong>g. Intravenous lidoca<strong>in</strong>e at this dose can<br />

lead to cardiac arrhythmia with potentially fatal consequences. The uncontrolled<br />

diabetic with an <strong>in</strong>complete miscarriage is a prime example for us<strong>in</strong>g a cervical block<br />

alone.The cervical block is best performed after clean<strong>in</strong>g <strong>and</strong> drap<strong>in</strong>g <strong>the</strong> patient <strong>in</strong><br />

<strong>the</strong> lithotomy position.<br />

Surgical Technique<br />

The usual preoperative assessment for any patient is necessary <strong>in</strong> <strong>the</strong>se situations.<br />

It is helpful if <strong>the</strong> patient can have a shower prior to <strong>the</strong> procedure. This helps to<br />

reduce <strong>the</strong> risk of <strong>in</strong>fection, <strong>the</strong>refore improv<strong>in</strong>g <strong>the</strong> overall success of <strong>the</strong> procedure.<br />

This is <strong>the</strong> st<strong>and</strong>ard set:<br />

Medium trolley—for <strong>in</strong>struments <strong>and</strong> dilatation <strong>and</strong> curettage pack<br />

Prep trolley—for clean<strong>in</strong>g of patient with cleans<strong>in</strong>g lotions<br />

M<strong>in</strong>i bowl set<br />
