Communication <strong>in</strong> <strong>the</strong> Third <strong>World</strong>: A One-Way Street<br />

program eventually had to be term<strong>in</strong>ated. While work<strong>in</strong>g with <strong>the</strong> universities <strong>the</strong>re<br />

<strong>and</strong> <strong>in</strong> Romania it was becom<strong>in</strong>g <strong>in</strong>creas<strong>in</strong>gly evident that <strong>the</strong>re was huge <strong>in</strong>novation<br />

<strong>and</strong> brilliant adaptation on <strong>the</strong> part of <strong>the</strong> professionals to cope with socioeconomic<br />

problems. Civil war claimed Dr. Yusuf B. Ishaki, Rector of <strong>the</strong> Tojik Medical<br />

University, as a victim of assass<strong>in</strong>ation. These severe circumstances were stifl<strong>in</strong>g any<br />

efforts to cont<strong>in</strong>ue deliver<strong>in</strong>g health care to <strong>the</strong> population. To some extent <strong>the</strong>y had<br />

been <strong>in</strong>novat<strong>in</strong>g for years with<strong>in</strong> a system of health care that was <strong>in</strong>tent on limit<strong>in</strong>g<br />

services <strong>and</strong> cost <strong>in</strong> <strong>in</strong>humane ways. There was no recognition of <strong>the</strong> brilliance of<br />

<strong>the</strong>se efforts or discussion of legitimiz<strong>in</strong>g <strong>the</strong> <strong>in</strong>novations with M<strong>in</strong>istries of Health<br />

that were still emulat<strong>in</strong>g <strong>the</strong> past. I tried serveral mechanisms to surface <strong>the</strong> good<br />

efforts without success. Indeed <strong>the</strong> notion of surfac<strong>in</strong>g <strong>the</strong>se efforts was<br />

anxiety-produc<strong>in</strong>g given <strong>the</strong> firm h<strong>and</strong> of <strong>the</strong> government still present.<br />

In 1998 I <strong>in</strong>itiated efforts to publish a journal of <strong>in</strong>ternational health that has its<br />

goal <strong>the</strong> <strong>in</strong>troduction of a body of literature that was derived from <strong>the</strong> “Third <strong>World</strong>”<br />

with an emphasis on surfac<strong>in</strong>g <strong>the</strong>se unique <strong>and</strong> worthy efforts to deliver <strong>the</strong> highest<br />

level of care <strong>in</strong> <strong>the</strong> milieu of social, political, <strong>and</strong> economic depression <strong>and</strong> often<br />

chaos. This effort is ongo<strong>in</strong>g but fraught with attitudes of want<strong>in</strong>g to replicate <strong>the</strong><br />

research <strong>and</strong> reports of <strong>the</strong> Western literature <strong>and</strong> a dim<strong>in</strong>ished sense of worth regard<strong>in</strong>g<br />

<strong>the</strong> local efforts to cope. I have s<strong>in</strong>ce exp<strong>and</strong>ed <strong>the</strong> communication <strong>and</strong><br />

participation of develop<strong>in</strong>g areas to <strong>in</strong>clude Indonesia. The representation of Indonesia<br />

as <strong>the</strong> largest Islamic country <strong>in</strong> <strong>the</strong> world <strong>and</strong> regard<strong>in</strong>g population <strong>the</strong> fourth<br />

largest country <strong>in</strong> <strong>the</strong> world is m<strong>in</strong>iscule <strong>in</strong> <strong>the</strong> world literature. Hard times have<br />

fallen on Indonesia, <strong>and</strong> although it is recover<strong>in</strong>g it has a long way to go. Our efforts<br />

to generate literature for <strong>the</strong> journal have resulted <strong>in</strong> some response. More importantly<br />

is <strong>the</strong> notion of an electronic (Internet-based) journal has caught <strong>the</strong> imag<strong>in</strong>ation<br />

of <strong>the</strong> Indonesian Medical Association that is start<strong>in</strong>g its own journal, JIMA<br />

(Journal of <strong>the</strong> Indonesian Medical Association) on <strong>the</strong> <strong>World</strong> Wide Web <strong>in</strong> year,<br />

2000. Success has been realized <strong>in</strong> a little different way than anticipated.<br />

This saga is related to illustrate an evolution <strong>in</strong> “develop<strong>in</strong>g country” communication.<br />

Priorities over <strong>the</strong> last 15 years have been shift<strong>in</strong>g very quickly driven by <strong>the</strong><br />

technology of computers <strong>and</strong> <strong>the</strong> Internet. We started with deliver<strong>in</strong>g Western orig<strong>in</strong>ated<br />

<strong>in</strong>formation on paper <strong>and</strong> progressed to CD-ROM, <strong>and</strong> <strong>the</strong>n to <strong>the</strong> Internet.<br />

The next step <strong>in</strong> that journey is beg<strong>in</strong>n<strong>in</strong>g to occur which is <strong>the</strong> legitimization of<br />

presentations of journals from <strong>the</strong> develop<strong>in</strong>g areas so long excluded from <strong>the</strong> world<br />

literature. The University of Norway is host<strong>in</strong>g <strong>the</strong> publication of <strong>the</strong> Ethiopian<br />

Medical Journal <strong>and</strong> o<strong>the</strong>r <strong>in</strong>itiatives are be<strong>in</strong>g planned for Zambia that has tried<br />

<strong>and</strong>, for economic reasons, repeatedly failed to publish <strong>the</strong>ir national medical journal.<br />

The <strong>in</strong>itiative to publish reality <strong>in</strong> cancer <strong>the</strong>rapy for India is just start<strong>in</strong>g with<br />

<strong>the</strong> Austral-Asian Journal of Cancer that is discussed fur<strong>the</strong>r on <strong>in</strong> this chapter. Both<br />

<strong>the</strong> British Medical Journal <strong>and</strong> Lancet have been especially supportive of <strong>in</strong>itiatives<br />

to support publication of material from develop<strong>in</strong>g areas. Pediatrics, <strong>the</strong> official journal<br />

of <strong>the</strong> American Academy of Pediatrics, has along with o<strong>the</strong>r US based journals,<br />

been leaders <strong>in</strong> <strong>in</strong>novation from be<strong>in</strong>g <strong>the</strong> first to offer a CD-ROM-based archive of<br />

its journal to publish<strong>in</strong>g its journal <strong>in</strong> o<strong>the</strong>r languages <strong>and</strong> now offer<strong>in</strong>g both a<br />

paper <strong>and</strong> Internet-based journal.<br />

There is still a role for <strong>the</strong> paper based journal, but it is difficult to def<strong>in</strong>e <strong>the</strong> role<br />

<strong>in</strong> a context any wider than local or national distribution. Each country is unique <strong>in</strong><br />

computer accessibility <strong>and</strong> stages of development. The fact rema<strong>in</strong>s that paper publish<strong>in</strong>g<br />

is relatively expensive, difficult to store <strong>and</strong> cumbersome to move about.<br />
