Tropical Nurs<strong>in</strong>g<br />

feed<strong>in</strong>g slowly to encourage retention of <strong>the</strong> feed<strong>in</strong>g. Often <strong>the</strong> mo<strong>the</strong>r or a relative<br />

can express breast milk until <strong>the</strong> baby is strong enough, <strong>and</strong> <strong>the</strong> suck<strong>in</strong>g/swallow<strong>in</strong>g<br />

reflexes are coord<strong>in</strong>ated. The neonates of cret<strong>in</strong>s <strong>and</strong> hypothyroid mo<strong>the</strong>rs are very<br />

delayed <strong>in</strong> <strong>the</strong>ir suck<strong>in</strong>g reflexes, <strong>and</strong> <strong>the</strong>se babies need gavage or preemie bottles<br />

with <strong>the</strong> <strong>in</strong>take monitored carefully. The cret<strong>in</strong> mo<strong>the</strong>r’s production of breast milk<br />

is usually delayed by several weeks, necessitat<strong>in</strong>g supplementation to ensure a weight<br />

ga<strong>in</strong> of at least 25 grams per day. She also needs careful tra<strong>in</strong><strong>in</strong>g <strong>in</strong> breastfeed<strong>in</strong>g<br />

skills, as, while <strong>the</strong> milk production is a natural process, <strong>the</strong> skill to coord<strong>in</strong>ate<br />

baby’s mouth to <strong>the</strong> areola usually needs to be taught <strong>and</strong> monitored. The joy that<br />

<strong>the</strong> mo<strong>the</strong>r experiences with hav<strong>in</strong>g a satisfied <strong>and</strong> grow<strong>in</strong>g baby is worth all <strong>the</strong><br />

midnight <strong>and</strong> early morn<strong>in</strong>g hour trips over to maternity to feed <strong>the</strong>se t<strong>in</strong>y newborns!<br />

One significant <strong>in</strong>fluence on hav<strong>in</strong>g full term babies <strong>and</strong> healthier children is <strong>the</strong><br />

level of education of <strong>the</strong> mo<strong>the</strong>r. Accord<strong>in</strong>g to <strong>the</strong> <strong>World</strong> Bank studies <strong>in</strong> 25 countries,<br />

even one to three years of maternal school<strong>in</strong>g reduces child mortality by about<br />

15 percent. Educated women are more likely to access prenatal care, delivery assistance,<br />

<strong>and</strong> <strong>in</strong>formation on health <strong>and</strong> safe dr<strong>in</strong>k<strong>in</strong>g water. For this reason, maternal<br />

education was encouraged dur<strong>in</strong>g <strong>the</strong> prenatal visit classes, well child cl<strong>in</strong>ics, <strong>and</strong><br />

community meet<strong>in</strong>gs. I worked closely with <strong>the</strong> local school teachers to assist <strong>in</strong> <strong>the</strong><br />

health education curriculum, ma<strong>in</strong>ta<strong>in</strong> <strong>the</strong> health of <strong>the</strong> students <strong>and</strong> faculty, <strong>and</strong><br />

obta<strong>in</strong> supplies for <strong>the</strong> school. One of <strong>the</strong> best <strong>in</strong>vestments for health promotion is<br />

education of children, particularly girls as <strong>the</strong>ir education affects <strong>the</strong> lives of <strong>the</strong>ir<br />

offspr<strong>in</strong>g to a greater extent than that of <strong>the</strong> fa<strong>the</strong>rs.<br />

Immunization cl<strong>in</strong>ics are profoundly important to protect <strong>the</strong> lives of <strong>the</strong> babies<br />

<strong>and</strong> children. Often <strong>the</strong>se children are already poorly nourished, anemic,<br />

immunocompromised, <strong>and</strong> a childhood illness which may cause a mild illness for a<br />

child <strong>in</strong> a First <strong>World</strong> environment, often proves fatal for a Third <strong>World</strong> child. The<br />

ever present challenge for vacc<strong>in</strong>es is transport <strong>and</strong> storage which ma<strong>in</strong>ta<strong>in</strong>s <strong>the</strong><br />

cold/freez<strong>in</strong>g temperatures required. This is particularly difficult <strong>in</strong> hot climates.<br />

The refridgerator models with solar energy have worked well if ma<strong>in</strong>ta<strong>in</strong>ed properly.<br />

Immunization, growth <strong>and</strong> development records are vital for <strong>the</strong> parents to protect<br />

<strong>and</strong> present at <strong>the</strong> cl<strong>in</strong>ics. The measles vacc<strong>in</strong>e can be safely adm<strong>in</strong>istered to a baby<br />

of six months of age dur<strong>in</strong>g an epidemic if a second dose to raise <strong>the</strong> antibody level<br />

is given after 12 months of age. This vacc<strong>in</strong>e is highly efficacious, provid<strong>in</strong>g protection<br />

with<strong>in</strong> seven days <strong>and</strong> sav<strong>in</strong>g <strong>the</strong> lives of many, many children.<br />

The nurs<strong>in</strong>g career <strong>in</strong> a Third <strong>World</strong> country is extremely diverse, requir<strong>in</strong>g<br />

creative approaches to time-honored pr<strong>in</strong>ciples of application, constantly assess<strong>in</strong>g<br />

<strong>the</strong> outcomes. The satisfaction <strong>and</strong> joy exceed <strong>the</strong> frustrations when one reflects on<br />

<strong>the</strong> healthier <strong>and</strong> more comfortable lives one has touched.<br />
