Tropical Nurs<strong>in</strong>g<br />
Language acquisition is imperative, as communicat<strong>in</strong>g effectively is vital to provid<strong>in</strong>g<br />
medical <strong>and</strong> nurs<strong>in</strong>g care. The books Language Acquisition Made Practical<br />
(LAMP) or Language Learn<strong>in</strong>g <strong>and</strong> Mission by Elizabeth PhD <strong>and</strong> (<strong>the</strong> late) Thomas<br />
Brewster, PhD are extremely practical <strong>and</strong> helpful guides <strong>in</strong> learn<strong>in</strong>g <strong>and</strong> us<strong>in</strong>g languages.<br />
The Wycliffe Summer Institutes of L<strong>in</strong>guistics are superb <strong>in</strong> language tra<strong>in</strong><strong>in</strong>g.<br />
Tapes of some of <strong>the</strong> more common languages are also helpful. On site, a fearless<br />
language coach is <strong>in</strong>valuable for correct<strong>in</strong>g pronounciation, <strong>in</strong>flection <strong>and</strong> idiom. In<br />
<strong>the</strong> beg<strong>in</strong>n<strong>in</strong>g, a list of helpful common medical phrases enables one to memorize<br />
quickly <strong>and</strong> get started with patient care. We all have our horror stories of mispronounc<strong>in</strong>g<br />
a word lead<strong>in</strong>g to un<strong>in</strong>tended mean<strong>in</strong>gs! Practice giv<strong>in</strong>g health <strong>in</strong>structions<br />
<strong>and</strong> tak<strong>in</strong>g histories with someone who won’t m<strong>in</strong>d.<br />
A library for personal reference, teach<strong>in</strong>g, mental refreshment <strong>and</strong> stimulation is<br />
valuable. Have available great texts on subjects such as surgery (I appreciated Dr S.<br />
Swartz’ surgical textbook), medic<strong>in</strong>e, pediatrics, a PDR, Manson’s Tropical Diseases,<br />
CPR by American Red Cross, Midwifery, public health, nurs<strong>in</strong>g, account<strong>in</strong>g <strong>and</strong><br />
economics (how to write grant applications!), immunizations (Epidemiology <strong>and</strong><br />
Prevention of Vacc<strong>in</strong>e-Preventable Diseases by CDC), anthropology, children’s books<br />
for young visitors. To rema<strong>in</strong> current <strong>in</strong> <strong>the</strong> field of medic<strong>in</strong>e, subscriptions to medical<br />
journals such as <strong>the</strong> British Journal of Tropical Medic<strong>in</strong>e, are <strong>in</strong>valuable. For teach<strong>in</strong>g,<br />
it is essential to have a stock of visual aids <strong>and</strong> charts on nutrition, childbirth, transmission<br />
of disease, hematology <strong>and</strong> parasites, oral rehydration, sk<strong>in</strong> <strong>and</strong> eye diseases,<br />
etc. Also books for recreation are appreciated: spiritual <strong>and</strong> historical bios, poetry,<br />
art, sports, h<strong>and</strong>iman’s journal, encyclopedias <strong>and</strong> world atlas, history of music or<br />
whatever is of <strong>in</strong>terest to <strong>the</strong> <strong>in</strong>dividual.<br />
Provid<strong>in</strong>g Equipment <strong>and</strong> Supplies<br />
A tremendous challenge <strong>in</strong> tropical nurs<strong>in</strong>g is to rema<strong>in</strong> with<strong>in</strong> budget yet viable<br />
as a healthcare <strong>in</strong>stitution. Hav<strong>in</strong>g a team of friends ever on <strong>the</strong> lookout for used<br />
equipment <strong>in</strong> good repair is a great asset. A quick look around our O.R. showed<br />
used tables (nonelectric), sta<strong>in</strong>less steel bas<strong>in</strong>s <strong>and</strong> bedpans that had been replaced<br />
with plastic supplies elsewhere, glass syr<strong>in</strong>ges no longer used, glass <strong>the</strong>rmometers.<br />
As hospitals merge or downsize, often this equipment is given away. In a Stateside<br />
O.R. or labor <strong>and</strong> delivery, sterile equipment that is opened but unused is discarded.<br />
A collection box <strong>in</strong> <strong>the</strong> supply room can amass enough equipment to supply a Third<br />
<strong>World</strong> hospital for a year or more, as labor costs for resteriliz<strong>in</strong>g are cheaper than<br />
purchas<strong>in</strong>g it. This method is great for collect<strong>in</strong>g suture, ca<strong>the</strong>ters, sponges, ace<br />
b<strong>and</strong>ages, gloves, dra<strong>in</strong>s. Inexpensive wraps for reautoclav<strong>in</strong>g equipment can be sewn<br />
from musl<strong>in</strong> or cotton sheets. I used fabric of various colors to color-code <strong>the</strong> wraps<br />
<strong>and</strong> covers to make repackag<strong>in</strong>g easier.<br />
A treadle sew<strong>in</strong>g mach<strong>in</strong>e can be a nurse’s best (almost) friend <strong>in</strong> sett<strong>in</strong>g up a<br />
hospital by sew<strong>in</strong>g <strong>the</strong> patient <strong>and</strong> fenestrated drapes, equipment <strong>and</strong> <strong>in</strong>strument<br />
wraps for autoclav<strong>in</strong>g, Mayo st<strong>and</strong> covers, glove wrappers, curta<strong>in</strong>s. For maternity, I<br />
enjoyed sew<strong>in</strong>g baby blankets <strong>and</strong> shirts. I had a carpenter make little cribs to accommodate<br />
<strong>the</strong> buggy-sized mattresses I had brought from <strong>the</strong> States, <strong>the</strong>n sewed<br />
soft flannel fitted sheets.<br />
One elderly gentleman whose chest was burned <strong>in</strong> a flareup while huddled <strong>in</strong><br />
front of an even<strong>in</strong>g fire, rout<strong>in</strong>ely pulled off his b<strong>and</strong>ages <strong>and</strong> picked at <strong>the</strong> wounds.<br />
I sewed several cotton vests with ties that rema<strong>in</strong>ed secure between dress<strong>in</strong>g changes<br />
until <strong>the</strong> burns had healed. For mobile village cl<strong>in</strong>ics, keep<strong>in</strong>g equipment h<strong>and</strong>y<br />
