CHAPTER ONE: INTRODUCTION<br />

1.1 Global HIV/AIDS Pandemic<br />

HIV/AIDS is an unprecedented pandemic and a public health emergency. Nearly 25<br />

million <strong>people</strong> world-wide have died <strong>of</strong> HIV/AIDS since 1981 when the first case was<br />

recorded and the epidemic is still growing <strong>to</strong> be the biggest challenge <strong>to</strong> public health<br />

(United Nations Program on HIV/AIDS [UNAIDS], 2005). In 2008, 33.4 million <strong>people</strong><br />

were living with HIV/AIDS worldwide (UNAIDS, 2009). 2.7 million <strong>people</strong> were found<br />

<strong>to</strong> be newly infected with HIV in 2008 alone,; 85% <strong>of</strong> <strong>who</strong>m were in the reproductive age<br />

and a <strong>to</strong>tal <strong>of</strong> 2.0 million died due <strong>to</strong> HIV related illnesses (UNAIDS, 2009). Sub-<br />

Saharan Africa remains the region most heavily affected by HIV, accounting for 67% <strong>of</strong><br />

all <strong>people</strong> living with HIV and for 75% <strong>of</strong> AIDS deaths in 2007. (UNAIDS, 2008).<br />

1.2 HIV/AIDS Epidemic in Uganda<br />

In Uganda, HIV/AIDS is the leading cause <strong>of</strong> death <strong>among</strong> adults and the fourth highest<br />

cause <strong>of</strong> death <strong>among</strong> children under five years (Uganda AIDS Commission [UAC],<br />

2006; UNAIDS, 2006). UNAIDS (2008) estimates that 940,000 <strong>people</strong> were living with<br />

HIV in Uganda as <strong>of</strong> December 2007. The 2004-05 Uganda HIV Sero–behavioural<br />

Survey [UHSBS] (Ministry <strong>of</strong> Health [MoH] & Opinion research Corporation [ORC]<br />

Macro, 2006) report an overall national HIV sero-prevalence rate <strong>of</strong> 6.4% <strong>among</strong> men<br />

and women aged 15-49 years. HIV incidence (new infections) reportedly increased from<br />

70,000 in 2002 <strong>to</strong> 132,500 in 2005. The survey reveals that 42% <strong>of</strong> these new infections<br />

occurred within hetero<strong>sexual</strong> stable relationships which accounts for 75-80% <strong>of</strong> all new<br />

infections while mother <strong>to</strong> child HIV transmission accounts for 15-25% (UAC, 2006).<br />

Although the decline in prevalence in the country has been attributed <strong>to</strong> behaviour<br />

change, evidence <strong>of</strong> such change has been uneven with researchers observing no<br />

significant increase in abstinence and fidelity (Opio, Mishra & Hong, 2008). Multiple<br />

<strong>partners</strong>, low condom use, high rates <strong>of</strong> discordance, lack <strong>of</strong> HIV sero-<strong>status</strong> disclosure <strong>to</strong><br />
