DATE: 1011 511 2 I COUNCIL COMMUNICATION SUBJECT: STAFF REPORT Discussion with possible action to consider a second request for an extension of Injury Leave with pay in accordance with 143.073 (b) of the Texas Local Government Code for Police Lieutenant Jaime Ramon. INITIATED BY: I STAFF SOURCE: Horacio De Leon, Jr., Asst. City Manager Carlos Villarreal, City Manager Daniel E. Migura, Jr., Human Resources Director PREVIOUS COUNCIL ACTION: None. I BACKGROUND: 12/18/2010 Lt. Jaime Ramon while at a traffic stop in his unit was rear ended by another vehicle, resulting in a concussion, low back pain extending to leg and calf, L 4 and 5 degenerative disc with annular tear. 12/20/2010 - placed off work until next follow-up on 12/24/10. 1/06/2011-1/07/2011 - placed on modified duty. 111 01201 1 -taken off of work until pending further evaluation. 311 11201 1-04/01 I201 1 - received physical therapy by Orthopedic Rehab & Sports Medicine Center. 10/2612011 -undergoes surgery 11/14/2011 - continues with lower back pain and remains off work until next follow up on 12/2012011 2/16/12 - placed in physical therapy and work conditioning. 08/06/12 - Lt. Ramon is released modified duty for 8 hrs. a day until expired by ordinance. FINANCIAL IMPACT: Funding is available in the 101 -231 0-521-1 1 10 personnel line item. COMMITTEE RECOMMENDATION: None STAFF RECOMMENDATION: Recommendation will be made by staff during this presentation.

Jaime Ramon, while at traffic stop his unit was rear ended by another vehicle, and had pain to neck, head, and lower back, and was transported to ER via ambulance for MVA. 12/18/2010- Lt. Ramon was evaluated by Doctors Hospital ER and was cleared to return to work on 12/22/20 1 1. He was given Tramadol for pain and instructed to follow-up with physician on the following Monday, 12/20/2010. 12/20/2010- Lt. Ramon was seen by Nurse at Risk management and evaluated and was given an appointment with Dr. Francisco J. Gutierrez for fLrther evaluation. 12/20/2010-Lt. Ramon was seen by Dr. F. J. Gutierrez for initial medical visit and was diagnosed with Back Contusion and Back Head Contusion, and kept off work until next follow-up on 12/24/2010. 121241-2010 - Lt. Ramon by Dr. F. J. Gutierrez again for follow-up and the employee continues with Back pain and Headache. Lt. Ramon remains off work until next appointment on1/3/2011 pending further evaluation. 01/3/2011-Lt. Ramon was seen by Dr. F. J. Gutierrez again for follow-up. Lt. Ramon continued with head contusion, back pain, and neck sprain, and the employee is kept off of work until 1/5/2011 and is able to return to work with restrictions to neck and back allowing Lt. Ramon posture restrictions of standing of 4 hours, sitting of 2 hours, bendinglstooping of 2 hours, and pushinglpulling of 4 hours maximum per day, motion restrictions of walking of 4 hours, climbing stairslladders of 2 hours maximum per day, may not
