na ženu heterozigota za ALD. U zaključku, metoda<br />

određivanja koncentracije MKVDL u serumu omogućila<br />

je probiranje širokog raspona rizičnih osoba u svrhu otkrivanja<br />

asimptomatskih bolesnika s X-ALD. Naglašava se<br />

potreba mjerenja MKVDL u serumu svih članova ne samo<br />

uže, već i šire obitelji bolesnika s ALD, kako bi se otkrili<br />

i drugi bolesnici i/ili heterozigoti za ALD. Otkrivanje heterozigota<br />

otvara mogućnost za prevenciju bolesti kroz<br />

gensko savjetovanje. Normalne koncentracije MKVDL u<br />

serumu u klinički suspektnih ispitanica ne isključuju heterozigotnost.<br />

U takvim slučajevima potrebno je odrediti<br />

koncentraciju MKVDL u homogenatu kulture kožnih fi -<br />

broblasta te eventualno napraviti mutacijske analize gena<br />

ABCD1.<br />

Usporedba reagensa dvaju proizvođača<br />

za određivanje vrijednosti C-reaktivnog<br />

proteina u nedonoščadi i novorođenčadi<br />

Klinički zavod za kemiju, KB Sestre milosrdnice, Zagreb, Hrvatska<br />

C-reaktivni protein (CRP) je reaktant akutne faze koji je u<br />

zdravih osoba prisutan u niskim koncentracijama. Patološka<br />

stanja kao što su bakterijske infekcije, upale ili razaranje<br />

tkiva praćena su povećanjem vrijednosti CRP uslijed<br />

otpuštanja upalnih citokina. Određivanje vrijednosti CRP<br />

vrlo je važno u dijagnostici i praćenju infekcije u nedonoščadi<br />

i novorođenčadi. U novorođenčadi sepsa može<br />

početi nespecifi čnim simptomima i dijagnosticira se tek<br />

na osnovi patoloških laboratorijskih nalaza. Cilj rada bio je<br />

ispitati analitičku pouzdanost i reproducibilnost reagensa<br />

Full Range CRP tvrtke Randox za određivanje vrijednosti<br />

CRP u serumu/plazmi (mjerno područje 0,1-160,0 mg/L) u<br />

usporedbi s CRP reagensima tvrtke Olympus kao referentnim<br />

reagensima: Olympus CRP reagensom za koncentracijsko<br />

područje 5,0-300,0 mg/L i Olympus CRP Latex<br />

reagensom namijenjenom ranoj dijagnozi infekcije u nedonoščadi<br />

i novorođenčadi. Za Olympus CRP Latex reagens<br />

postoje dvije aplikacije: osjetljiva (SCRP, za koncentracijsko<br />

područje 0,5-20,0 mg/L ) i visoko osjetljiva (HS-<br />

CRP, za koncentracijsko područje 0,05-2,00 mg/L). Za sve<br />

reagense primijenjene su izvorne baždarne i kontrolne<br />

otopine, kao i aplikacije za instrument Olympus AU 640.<br />

Ispitivanja su provedena u skladu s preporukama ECCLS.<br />

Diagnostic tests should be off ered to all at risk relatives<br />

of X-ALD patients and should include members of the<br />

extended family. Identifi cation of heterozygotes provides<br />

an opportunity for disease prevention through genetic<br />

counseling. Normal concentration VLCFAs in clinically<br />

suspected females does not exclude carrier state. In these<br />

cases, the combined use of fi broblast VLCFA level analysis<br />

and possibly ABCD1 gene mutation analysis could detect<br />

X-ALD carriers correctly.<br />

Comparison of reagents from two<br />

manufacturers for C-reactive protein<br />

determination in preterm and term neonates<br />

C-reactive protein (CRP) is an acute phase reactant, which<br />

is found in low concentrations in healthy individuals.<br />

Pathologic states such as bacterial infection, infl ammation<br />

or tissue lesions are associated with an increase in<br />

CRP level due to the release of infl ammatory cytokines.<br />

Determination of CRP is of utmost importance in the diagnosis<br />

and monitoring of infection in preterm and term<br />

neonates. In neonates, sepsis may begin with unspecifi c<br />

symptoms and is only diagnosed on the basis of pathologic<br />

laboratory fi ndings. The aim of the study was to assess<br />

analytical reliability and reproducibility of the Full Range<br />

CRP reagent (Randox) for CRP determination in serum/<br />

plasma (measuring range 0.1-160.0 mg/L) in comparison<br />

with Olympus CRP reagents as reference reagents: Olympus<br />

CRP reagent for the concentration range of 5.0-300.0<br />

mg/L and Olympus CRP Latex reagent intended for early<br />

diagnosis of infection in preterm and term newborns. The<br />

Olympus CRP Latex reagent has two applications: sensitive<br />

(SCRP, for concentration range of 0.5-20.0 mg/L) and<br />

highly sensitive (HSCRP, for concentration range of 0.05-<br />

2.00 mg/L). The original calibrated and control solutions<br />

and applications for the Olympus AU 640 instrument were<br />

used for all reagents. Tests were performed in line with<br />

<strong>Biochemia</strong> <strong>Medica</strong> 2006;16(Suppl 1):S1–S268<br />
