New Teacher Feedback and Evaluation Forms<br />

There are a series of new <strong>teacher</strong> feedback forms. These<br />

forms are aligned to the 8 timeframes that occur each<br />

year and collectively create the process for gathering<br />

baseline data on the new <strong>teacher</strong>’s performance. Each<br />

form specifically lists the quality indicators for the<br />

specified timeframe. Each indicator includes bulleted<br />

statements referencing the particular knowledge and/or<br />

skill to be demonstrated. There is opportunity to provide<br />

comments for each of the listed indicators. A 0 – 7 rating<br />

scale is also provided for each indicator. The rating scale<br />

and a full description of the performance it references are<br />

provided on each specific growth guide for each quality<br />

indicator.<br />

An overall determination on performance uses baseline<br />

and follow-up scores, feedback generated throughout the<br />

year on selected indicators, general feedback generated<br />

periodically through classroom observations and any<br />

other data or information relevant to the <strong>teacher</strong>’s<br />

performance observed or gathered throughout the year.<br />

This information is captured on feedback forms. This<br />

information and data is used to complete Summative<br />

Evaluation Form.<br />
