North Tisra is 210m as per m<strong>in</strong>e plan. However, as AMP / Authentic <strong>in</strong>formati<strong>on</strong> are not<br />

available the <strong>in</strong>specti<strong>on</strong> <str<strong>on</strong>g>of</str<strong>on</strong>g> the barrier was emphasized to check the flow <str<strong>on</strong>g>of</str<strong>on</strong>g> water.<br />

Status <str<strong>on</strong>g>of</str<strong>on</strong>g> Atmospheric c<strong>on</strong>diti<strong>on</strong> <str<strong>on</strong>g>of</str<strong>on</strong>g> the old work<strong>in</strong>gs:<br />

• O2- 20.6%, CH4- Nil, CO-Nil, Dry Bulb Temp (DBT):41 0 C, Wet bulb Temp<br />

(WBT):41 0 C, Relative Humidity-100%. The said c<strong>on</strong>diti<strong>on</strong>s prevail s<strong>in</strong>ce 2003 as<br />

per records available.<br />

• The cause <str<strong>on</strong>g>of</str<strong>on</strong>g> the high temperature at the Old work<strong>in</strong>gs may be due to the seepage<br />

<str<strong>on</strong>g>of</str<strong>on</strong>g> excessive hot water <str<strong>on</strong>g>of</str<strong>on</strong>g> about 45 0 C all al<strong>on</strong>g the barrier and high relative<br />

humidity. A Scientific study regard<strong>in</strong>g the ventilati<strong>on</strong> <str<strong>on</strong>g>of</str<strong>on</strong>g> Lodna Colliery is be<strong>in</strong>g<br />

c<strong>on</strong>ducted by CIMFR, Dhanbad to f<strong>in</strong>d means for reduc<strong>in</strong>g the high temp. existed<br />

at that old work<strong>in</strong>g.<br />

About the Incident / accident:-<br />

In 1st Shift <str<strong>on</strong>g>of</str<strong>on</strong>g> 05.01.2009 <strong>on</strong>e Sr.OM who was work<strong>in</strong>g as ventilati<strong>on</strong> <str<strong>on</strong>g>of</str<strong>on</strong>g>ficer went to old<br />

work<strong>in</strong>g <str<strong>on</strong>g>of</str<strong>on</strong>g> VII Seam 24 th rise al<strong>on</strong>g with two others for rout<strong>in</strong>e <strong>in</strong>specti<strong>on</strong>. They went to<br />

UG through 7 Pit (land<strong>in</strong>g at IX/X Seam) at about 11.30 pm. The maximum travell<strong>in</strong>g<br />

time from 7 Pit bottom to 24 th rise <str<strong>on</strong>g>of</str<strong>on</strong>g> VII Seam old work<strong>in</strong>g would be 30 to 45 m<strong>in</strong>utes,<br />

therefore, it was expected that the Team might have entered the old work<strong>in</strong>gs at any time<br />

between 12.00 no<strong>on</strong> to 12.30 P.M and not returned after end <str<strong>on</strong>g>of</str<strong>on</strong>g> the shift. A search<br />

operati<strong>on</strong> was started at about 5.30 P.M. with Multigas Detetor, Hygrometer and Flame<br />

<strong>Safety</strong> Lamp. The whole team reached upto the barrier <str<strong>on</strong>g>of</str<strong>on</strong>g> (-)16 th Level <str<strong>on</strong>g>of</str<strong>on</strong>g> 24 th rise and all<br />

al<strong>on</strong>g the barrier pillar, no noxious gases was detected and O2 % was more than 20%,<br />

temperature was 41 0 C, humidity was 100%, then after 30 m<strong>in</strong>ute <str<strong>on</strong>g>of</str<strong>on</strong>g> search operati<strong>on</strong> the<br />

whole team returned to (-) 3 rd Level. At about 6.40 P.M. the body <str<strong>on</strong>g>of</str<strong>on</strong>g> Sr.OM was located<br />

at (-) 14 th Level between 22 nd and 23 rd rise and the sec<strong>on</strong>d body was detected nearby.<br />

The third body was located at about 11.30 A.M. <strong>on</strong> 06.01.2009 by the rescue team about<br />

13 pillars away.<br />

F<strong>in</strong>d<strong>in</strong>gs and Cause <str<strong>on</strong>g>of</str<strong>on</strong>g> the Accident:-<br />

1. The normal route <str<strong>on</strong>g>of</str<strong>on</strong>g> <strong>in</strong>specti<strong>on</strong> upto (-) 16 th Level barrier is through 24 th rise and<br />

return back al<strong>on</strong>g 24th dip upto (-)3 rd Level. Generally, after tak<strong>in</strong>g some rest<br />

<strong>in</strong>specti<strong>on</strong> team used to <strong>in</strong>spect VII to V/VI Seam drift al<strong>on</strong>g (-) 4 th Level upto 21 st<br />

rise <str<strong>on</strong>g>of</str<strong>on</strong>g> VII Seam and back al<strong>on</strong>g with same route.<br />

2. After reach<strong>in</strong>g the barrier they <strong>in</strong>spected the same probably while return<strong>in</strong>g they<br />

followed a different route through the cross-cut and <strong>on</strong>e <str<strong>on</strong>g>of</str<strong>on</strong>g> team member might have<br />

been separated and fell down after gett<strong>in</strong>g exhausted.<br />

3. Other members <str<strong>on</strong>g>of</str<strong>on</strong>g> the team <strong>in</strong> search <str<strong>on</strong>g>of</str<strong>on</strong>g> separated member too got exhausted and laid<br />

down <strong>in</strong> between <strong>in</strong> (-)14 th Level keep<strong>in</strong>g their Cap lamp and Flame <strong>Safety</strong> Lamp<br />

burn<strong>in</strong>g with a hope that separated member might locate them <strong>on</strong> see<strong>in</strong>g burn<strong>in</strong>g<br />

lamps.<br />

4. The postmortem report clearly po<strong>in</strong>ted out the cause <str<strong>on</strong>g>of</str<strong>on</strong>g> their death is “due to heat<br />

exhausti<strong>on</strong> as a result <str<strong>on</strong>g>of</str<strong>on</strong>g> work<strong>in</strong>g <strong>in</strong> high atmospheric temperature”. The cause<br />

<str<strong>on</strong>g>of</str<strong>on</strong>g> death was simply due to overexposure <strong>in</strong> the above stated c<strong>on</strong>diti<strong>on</strong> for more than<br />

2 to 3 hours.<br />

5. The old work<strong>in</strong>g was <strong>in</strong>spectable with<strong>in</strong> limited span <str<strong>on</strong>g>of</str<strong>on</strong>g> time, as the area was not<br />

adequately ventilated with high DBT, WBT and relative humidity. The temperature<br />
