1. Definiti<strong>on</strong><br />

Khat <str<strong>on</strong>g>is</str<strong>on</strong>g> a herbal product c<strong>on</strong>s<str<strong>on</strong>g>is</str<strong>on</strong>g>ting of <str<strong>on</strong>g>the</str<strong>on</strong>g> leaves and shoots of <str<strong>on</strong>g>the</str<strong>on</strong>g> shrub<br />

Catha edul<str<strong>on</strong>g>is</str<strong>on</strong>g>. It <str<strong>on</strong>g>is</str<strong>on</strong>g> cultivated primarily in East Africa and <str<strong>on</strong>g>the</str<strong>on</strong>g> Arabian<br />

Peninsula harvested and <str<strong>on</strong>g>the</str<strong>on</strong>g>n chewed to obtain a stimulant effect. There are<br />

many different varieties of Catha edul<str<strong>on</strong>g>is</str<strong>on</strong>g> depending up<strong>on</strong> <str<strong>on</strong>g>the</str<strong>on</strong>g> area in which it <str<strong>on</strong>g>is</str<strong>on</strong>g><br />

cultivated.<br />

3. Legal Status<br />

Khat <str<strong>on</strong>g>is</str<strong>on</strong>g> currently imported and used legally in <str<strong>on</strong>g>the</str<strong>on</strong>g> UK.<br />

4. Prevalence in UK<br />

One of <str<strong>on</strong>g>the</str<strong>on</strong>g> main epidemiological tools <str<strong>on</strong>g>for</str<strong>on</strong>g> m<strong>on</strong>itoring drug use in England and<br />

Wales, <str<strong>on</strong>g>the</str<strong>on</strong>g> Brit<str<strong>on</strong>g>is</str<strong>on</strong>g>h Crime Survey, does not report <strong>on</strong> khat use. There <str<strong>on</strong>g>is</str<strong>on</strong>g> no<br />

evidence of khat use in <str<strong>on</strong>g>the</str<strong>on</strong>g> general populati<strong>on</strong>, all <str<strong>on</strong>g>the</str<strong>on</strong>g> epidemiological data<br />

available have reported <strong>on</strong> communities in which khat use <str<strong>on</strong>g>is</str<strong>on</strong>g> prevalent, ie <str<strong>on</strong>g>the</str<strong>on</strong>g><br />

Somali, Ethiopian, Kenyan and Yemeni communities.<br />

5. Prevalence in L<strong>on</strong>d<strong>on</strong><br />

The Home Office comm<str<strong>on</strong>g>is</str<strong>on</strong>g>si<strong>on</strong>ed a report by <str<strong>on</strong>g>the</str<strong>on</strong>g> Nati<strong>on</strong>al Associati<strong>on</strong> <str<strong>on</strong>g>for</str<strong>on</strong>g> <str<strong>on</strong>g>the</str<strong>on</strong>g><br />

Care and Resettlement of Offenders (NACRO) to character<str<strong>on</strong>g>is</str<strong>on</strong>g>e khat use in<br />

Somali communities around <str<strong>on</strong>g>the</str<strong>on</strong>g> UK. Researchers c<strong>on</strong>ducted 602 interviews<br />

using privileged access interviewers in fours cities in England. Of <str<strong>on</strong>g>the</str<strong>on</strong>g> total<br />

sample, 39% had used khat at some time, 34% had used khat in <str<strong>on</strong>g>the</str<strong>on</strong>g> m<strong>on</strong>th<br />

prior to interview and less than 4% admitted to using <strong>on</strong> a daily bas<str<strong>on</strong>g>is</str<strong>on</strong>g>.<br />

Of <str<strong>on</strong>g>the</str<strong>on</strong>g> 235 people who identified <str<strong>on</strong>g>the</str<strong>on</strong>g>mselves as having ever used khat, 81%<br />

were male. A similar study interviewed 207 Somal<str<strong>on</strong>g>is</str<strong>on</strong>g> living in L<strong>on</strong>d<strong>on</strong>. In th<str<strong>on</strong>g>is</str<strong>on</strong>g><br />

study<br />

78% of <str<strong>on</strong>g>the</str<strong>on</strong>g> overall sample had ever used khat, 67% had used in <str<strong>on</strong>g>the</str<strong>on</strong>g> week<br />

prior to interview and 6% were using khat <strong>on</strong> a daily bas<str<strong>on</strong>g>is</str<strong>on</strong>g>. The difference in<br />

<str<strong>on</strong>g>the</str<strong>on</strong>g> studies <str<strong>on</strong>g>is</str<strong>on</strong>g> most likely due to <str<strong>on</strong>g>the</str<strong>on</strong>g> sex ratio, with 73% of <str<strong>on</strong>g>the</str<strong>on</strong>g> latter sample<br />

being male compared to 54% in <str<strong>on</strong>g>the</str<strong>on</strong>g> <str<strong>on</strong>g>for</str<strong>on</strong>g>mer study. Both studies report more<br />

use of khat by males.<br />

6. Prevalence in <str<strong>on</strong>g>Brent</str<strong>on</strong>g><br />

There are no stat<str<strong>on</strong>g>is</str<strong>on</strong>g>tics currently available <strong>on</strong> <str<strong>on</strong>g>the</str<strong>on</strong>g> prevalence of Khat in <str<strong>on</strong>g>Brent</str<strong>on</strong>g>.<br />

Anecdotal evidence however provides reports <strong>on</strong> <str<strong>on</strong>g>the</str<strong>on</strong>g> impact <strong>on</strong> <str<strong>on</strong>g>the</str<strong>on</strong>g> family in<br />

terms of domestic violence, family instability and financial pressures.<br />

7. The Use of Khat<br />

In<str<strong>on</strong>g>for</str<strong>on</strong>g>mati<strong>on</strong> about <str<strong>on</strong>g>the</str<strong>on</strong>g> use of khat in <str<strong>on</strong>g>the</str<strong>on</strong>g> UK comes from reports into <str<strong>on</strong>g>the</str<strong>on</strong>g><br />

communities from countries that traditi<strong>on</strong>ally use khat. The<br />

largest epidemiological survey of drug m<str<strong>on</strong>g>is</str<strong>on</strong>g>use in England and Wales,<br />

<str<strong>on</strong>g>the</str<strong>on</strong>g> Brit<str<strong>on</strong>g>is</str<strong>on</strong>g>h Crime Survey, does not include khat as <strong>on</strong>e of its reference<br />

drugs. Most of <str<strong>on</strong>g>the</str<strong>on</strong>g> prevalence data comes from <str<strong>on</strong>g>the</str<strong>on</strong>g> Somali community.<br />

Figures range from 34% to 67% of <str<strong>on</strong>g>the</str<strong>on</strong>g> Somali community who identify<br />

<str<strong>on</strong>g>the</str<strong>on</strong>g>mselves as current users of khat. There are no publ<str<strong>on</strong>g>is</str<strong>on</strong>g>hed reports in <str<strong>on</strong>g>the</str<strong>on</strong>g><br />

o<str<strong>on</strong>g>the</str<strong>on</strong>g>r individual ethnic communities.<br />

There are no reports of khat use in <str<strong>on</strong>g>the</str<strong>on</strong>g> UK outside of <str<strong>on</strong>g>the</str<strong>on</strong>g> communities<br />

that traditi<strong>on</strong>ally use khat.<br />

8. Import, export, d<str<strong>on</strong>g>is</str<strong>on</strong>g>tributi<strong>on</strong> and use of khat in <str<strong>on</strong>g>the</str<strong>on</strong>g> UK<br />

Approximately 6 t<strong>on</strong>s of khat arrives in <str<strong>on</strong>g>the</str<strong>on</strong>g> UK per week, mostly by air<br />

from Kenya. The bulk of th<str<strong>on</strong>g>is</str<strong>on</strong>g> <str<strong>on</strong>g>is</str<strong>on</strong>g> in transit <str<strong>on</strong>g>for</str<strong>on</strong>g> supply to <str<strong>on</strong>g>the</str<strong>on</strong>g> United States<br />

of America. The UK <str<strong>on</strong>g>is</str<strong>on</strong>g> a base <str<strong>on</strong>g>for</str<strong>on</strong>g> khat d<str<strong>on</strong>g>is</str<strong>on</strong>g>tributi<strong>on</strong> to many countries,<br />

including <str<strong>on</strong>g>the</str<strong>on</strong>g> US, where <str<strong>on</strong>g>the</str<strong>on</strong>g> plant <str<strong>on</strong>g>is</str<strong>on</strong>g> illegal.<br />

There <str<strong>on</strong>g>is</str<strong>on</strong>g> an efficient d<str<strong>on</strong>g>is</str<strong>on</strong>g>tributi<strong>on</strong> network to <str<strong>on</strong>g>the</str<strong>on</strong>g> khat using communities<br />

across <str<strong>on</strong>g>the</str<strong>on</strong>g> UK. Most users buy khat at <str<strong>on</strong>g>the</str<strong>on</strong>g> mafresh, a meeting place<br />

where khat <str<strong>on</strong>g>is</str<strong>on</strong>g> bought and chewed. Mafreshi proprietors often sell soft<br />

drinks and cigarettes al<strong>on</strong>gside khat. The trade in khat <str<strong>on</strong>g>is</str<strong>on</strong>g> a legitimate<br />

business and <str<strong>on</strong>g>is</str<strong>on</strong>g> quite d<str<strong>on</strong>g>is</str<strong>on</strong>g>tinct from <str<strong>on</strong>g>the</str<strong>on</strong>g> trade in illegal drugs.<br />

Mafreshi are subject to health and safety requirements as <str<strong>on</strong>g>the</str<strong>on</strong>g>y are<br />

public places where a product <str<strong>on</strong>g>is</str<strong>on</strong>g> sold and c<strong>on</strong>sumed, however many<br />

are unknown to <str<strong>on</strong>g>the</str<strong>on</strong>g> local authorities. They are of varying standards of<br />

cleanliness and safety. Alternatively khat <str<strong>on</strong>g>is</str<strong>on</strong>g> bought at local shops, in<br />

markets or via ‘mobile traders’.<br />

Men are more likely to use at <str<strong>on</strong>g>the</str<strong>on</strong>g> mafresh and women are more likely<br />

to use at home, often al<strong>on</strong>e. There <str<strong>on</strong>g>is</str<strong>on</strong>g> under-reporting of women’s use<br />

of khat probably as a result of <str<strong>on</strong>g>the</str<strong>on</strong>g> extra stigma <str<strong>on</strong>g>the</str<strong>on</strong>g>y face.<br />

9. RISKS TO PHYSICAL HEALTH<br />

There <str<strong>on</strong>g>is</str<strong>on</strong>g> evidence that chewing Khat <str<strong>on</strong>g>is</str<strong>on</strong>g> a r<str<strong>on</strong>g>is</str<strong>on</strong>g>k factor <str<strong>on</strong>g>for</str<strong>on</strong>g> <str<strong>on</strong>g>the</str<strong>on</strong>g> development<br />

of oral cancers. Chewing khat leads to an increase in blood pressure and may<br />

precipitate myocardial infarcti<strong>on</strong>. It <str<strong>on</strong>g>is</str<strong>on</strong>g> difficult to tease out <str<strong>on</strong>g>the</str<strong>on</strong>g> specific r<str<strong>on</strong>g>is</str<strong>on</strong>g>k<br />

factor of khat <str<strong>on</strong>g>for</str<strong>on</strong>g> heart d<str<strong>on</strong>g>is</str<strong>on</strong>g>ease as most users also smoke tobacco during a<br />

khat<br />

sessi<strong>on</strong>.<br />

There <str<strong>on</strong>g>is</str<strong>on</strong>g> some evidence that khat affects <str<strong>on</strong>g>the</str<strong>on</strong>g> reproductive health of<br />

both sexes. In women it may be associated with delivery of low birth<br />

weight babies (as with smoking cigarettes), although <str<strong>on</strong>g>the</str<strong>on</strong>g> evidence <str<strong>on</strong>g>for</str<strong>on</strong>g><br />

th<str<strong>on</strong>g>is</str<strong>on</strong>g> <str<strong>on</strong>g>is</str<strong>on</strong>g> not str<strong>on</strong>g. Cathine <str<strong>on</strong>g>is</str<strong>on</strong>g> excreted in breast milk although <str<strong>on</strong>g>the</str<strong>on</strong>g> impact<br />

of th<str<strong>on</strong>g>is</str<strong>on</strong>g> <str<strong>on</strong>g>is</str<strong>on</strong>g> unknown.<br />

In men <str<strong>on</strong>g>the</str<strong>on</strong>g>re <str<strong>on</strong>g>is</str<strong>on</strong>g> some evidence that using khat <str<strong>on</strong>g>is</str<strong>on</strong>g> associated with lower<br />

sperm motility and sperm count. Some studies report an increase in<br />

libido when using khat and o<str<strong>on</strong>g>the</str<strong>on</strong>g>rs have found decreased libido with<br />

chr<strong>on</strong>ic use of khat.<br />

Residual pesticide, dimethoate, has been found <strong>on</strong> khat leaves<br />

produced in Yemen. There <str<strong>on</strong>g>is</str<strong>on</strong>g> no publ<str<strong>on</strong>g>is</str<strong>on</strong>g>hed data <strong>on</strong> khat produced in<br />

o<str<strong>on</strong>g>the</str<strong>on</strong>g>r countries. Chr<strong>on</strong>ic dimethoate po<str<strong>on</strong>g>is</str<strong>on</strong>g><strong>on</strong>ing can lead to weakness,<br />

fatigue, slurred speech and lack of co-ordinati<strong>on</strong>.<br />

10. RISK OF ADDICTION AND TO PSYCHIATRIC HEALTH<br />

There <str<strong>on</strong>g>is</str<strong>on</strong>g> evidence that some individuals use khat in a dependent way.<br />

However, <str<strong>on</strong>g>for</str<strong>on</strong>g> <str<strong>on</strong>g>the</str<strong>on</strong>g> majority of users th<str<strong>on</strong>g>is</str<strong>on</strong>g> does not appear to be <str<strong>on</strong>g>the</str<strong>on</strong>g> case.<br />

11. SOCIAL ISSUES<br />

The partners of khat users often complain that <str<strong>on</strong>g>the</str<strong>on</strong>g>ir partners’ khat use<br />

<str<strong>on</strong>g>is</str<strong>on</strong>g> resp<strong>on</strong>sible <str<strong>on</strong>g>for</str<strong>on</strong>g> lack of input into family life, <str<strong>on</strong>g>for</str<strong>on</strong>g> family arguments, and<br />

leads to excessive expenditure of <str<strong>on</strong>g>the</str<strong>on</strong>g> family budget. It <str<strong>on</strong>g>is</str<strong>on</strong>g> cited as a<br />

reas<strong>on</strong> <str<strong>on</strong>g>for</str<strong>on</strong>g> family breakdown by spouses, and <str<strong>on</strong>g>the</str<strong>on</strong>g>re <str<strong>on</strong>g>is</str<strong>on</strong>g> a fear that men<br />

using excessively (as heads of <str<strong>on</strong>g>the</str<strong>on</strong>g> family unit) lead to <str<strong>on</strong>g>is</str<strong>on</strong>g>olati<strong>on</strong> <str<strong>on</strong>g>for</str<strong>on</strong>g> <str<strong>on</strong>g>the</str<strong>on</strong>g>ir<br />

spouses and children.<br />

12. LOCAL ISSUES-BRENT<br />

Experiences of health v<str<strong>on</strong>g>is</str<strong>on</strong>g>itors- a few cases have ar<str<strong>on</strong>g>is</str<strong>on</strong>g>en where health v<str<strong>on</strong>g>is</str<strong>on</strong>g>itors<br />

have supported families where Khat <str<strong>on</strong>g>is</str<strong>on</strong>g> used. The social impact <str<strong>on</strong>g>is</str<strong>on</strong>g> that women<br />

bear <str<strong>on</strong>g>the</str<strong>on</strong>g> burden of caring <str<strong>on</strong>g>for</str<strong>on</strong>g> <str<strong>on</strong>g>the</str<strong>on</strong>g> family when men go away <str<strong>on</strong>g>for</str<strong>on</strong>g> l<strong>on</strong>g periods to<br />

c<strong>on</strong>sume khat. The ec<strong>on</strong>omic cost <str<strong>on</strong>g>is</str<strong>on</strong>g> that men use family income to purchase<br />

Khat including child benefit. Domestic violence <str<strong>on</strong>g>is</str<strong>on</strong>g> comm<strong>on</strong> and also causes<br />

fricti<strong>on</strong> in <str<strong>on</strong>g>the</str<strong>on</strong>g> home.<br />

A school nurses experience of a family where use of Khat has been <strong>on</strong>going<br />

since <str<strong>on</strong>g>the</str<strong>on</strong>g>y arrived in <str<strong>on</strong>g>the</str<strong>on</strong>g> UK in <str<strong>on</strong>g>the</str<strong>on</strong>g> late 80’s. Mum has had schizophrenia,<br />

dad also abuses alcohol and four children have been affected and today<br />

placed <strong>on</strong> <str<strong>on</strong>g>the</str<strong>on</strong>g> cp reg<str<strong>on</strong>g>is</str<strong>on</strong>g>ter as a result of neglect. Two of <str<strong>on</strong>g>the</str<strong>on</strong>g> children now have<br />

bipolar d<str<strong>on</strong>g>is</str<strong>on</strong>g>order and are at th<str<strong>on</strong>g>is</str<strong>on</strong>g> time in residential child psychiatric care and<br />

<str<strong>on</strong>g>the</str<strong>on</strong>g>re <str<strong>on</strong>g>is</str<strong>on</strong>g> also a grandchild who <str<strong>on</strong>g>is</str<strong>on</strong>g> now looked after. Two of <str<strong>on</strong>g>the</str<strong>on</strong>g> children now<br />

use cannab<str<strong>on</strong>g>is</str<strong>on</strong>g>.<br />

13. Asian Women’s Resources Centre<br />

They had <strong>on</strong>e case, in 2004, of a Somalia woman who was experiencing<br />

domestic violence. Her husband frequently used khat. The effect was abuse,<br />

violent and aggressive behaviour towards her.<br />

14. Youth and Community Service<br />

Not currently aware of <str<strong>on</strong>g>the</str<strong>on</strong>g> use by young people using <str<strong>on</strong>g>the</str<strong>on</strong>g>ir services, but<br />

aware of <str<strong>on</strong>g>the</str<strong>on</strong>g> availability within <str<strong>on</strong>g>the</str<strong>on</strong>g> community.<br />

ACTION<br />

1. C<strong>on</strong>duct local studies to establ<str<strong>on</strong>g>is</str<strong>on</strong>g>h <str<strong>on</strong>g>the</str<strong>on</strong>g> prevalence of khat use in <str<strong>on</strong>g>Brent</str<strong>on</strong>g>.<br />

2. Involve Somalia community leaders to ra<str<strong>on</strong>g>is</str<strong>on</strong>g>e awareness <strong>on</strong> <str<strong>on</strong>g>the</str<strong>on</strong>g> impact of<br />

khat use <strong>on</strong> <str<strong>on</strong>g>the</str<strong>on</strong>g> individual and family.<br />

3. Public in<str<strong>on</strong>g>for</str<strong>on</strong>g>mati<strong>on</strong> <str<strong>on</strong>g>is</str<strong>on</strong>g> required <strong>on</strong> <str<strong>on</strong>g>the</str<strong>on</strong>g> health impact of khat use.<br />
