Important information: How to register:<br />

Reception opening hours:<br />

Mon, Tues, Thurs, Fri 9am-2pm and 3pm-6.30pm<br />

Wednesday 9am-1pm<br />

If you have concerns about your health:<br />

Phone 295 3737 Monday to Friday 8am to 6.30pm, to<br />

book an appointment or arrange to speak to a doctor.<br />

If you have access to the internet please ask reception<br />

about registering for on-line appointment booking.<br />

For urgent problems only attend reception 9am to<br />

10.30am Monday to Friday to be seen by a doctor.<br />

Please note you may be asked to come back for a<br />

routine appointment if appropriate.<br />

To request a home visit phone 295 3737 by 11am<br />

Monday to Friday. A doctor will assess whether a visit<br />

is the best way of managing your problem.<br />

Some pre-booked appointments are available on<br />

Saturday morning for people who find it difficult to<br />

attend appointments during the week.<br />

What to do when the surgery is closed<br />

(OUT-OF-HOURS)<br />

If your condition is life threatening dial 999 or go to the<br />

nearest A&E.<br />

For advice on minor illness see your local pharmacist,<br />

visit Great Charlotte St Walk-In <strong>Centre</strong> (Tel 285 3535),<br />

Smithdown Children’s Minor Injury Unit (Tel 285 4820),<br />

phone NHS direct (Tel 0845 4647) or visit www.nhs.uk<br />

To speak to a doctor phone 295 3737 to be put<br />

through to the on-call service.<br />

The practice area includes all of L2, 3, 4, and 5, and part of<br />

L1, 6, 7 and 20. To register please contact reception to<br />

check you are in our area and if so attend the health centre<br />

to fill in the relevant forms and book a new patient check.<br />

If you move out of our practice area you will need to join<br />

another GP closer to your new home.<br />

Other information<br />

Rights and Responsibilities of Patients<br />

Our patients have access to treatment and appropriate referrals to<br />

specialists on the NHS, or private referrals if requested. We expect<br />

you to attend appointments on time. You can choose whether or<br />

not to take part in medical research and medical student training.<br />

We aim to treat our patients with respect at all times and expect<br />

the same in return from our patients. We operate a zero tolerance<br />

policy to physical or verbal aggression and will remove patients<br />

from our practice list if necessary.<br />

Confidentiality<br />

Information on medical records is strictly confidential and is<br />

kept on a secure computer network. We share information on<br />

a need-to-know basis with <strong>Health</strong> Care staff and other health<br />

professionals when necessary. For more information go to<br />

confidentiality.<br />
