after obtaining from the Government servant in writing, a declaration,<br />

that he had, in fact rendered that period of service. He should,<br />

at the foot of the statement, make and subscribe to a declaration as<br />

to the truth of that statement and should in support of such<br />

declaration, produce all documentary evidence and furnish all information<br />

which is in his power to produce or furnish.<br />

(h) In cases where the service register itself is lost or is not traceable,<br />

once the date of appointment is established with reference to<br />

direct independent evidence, an affidavit given by the employee in<br />

respect of his service duly supported by collateral evidence of<br />

contemporary employees shall be accepted.<br />

(i) Heads of Offices in respect of non-gazetted officers and Heads of<br />

Departments in respect of Gazetted Officers shall be personally<br />

held responsible for any omission in the verification of<br />

service or any other entries in the Service register for the entire<br />

period of service of a Government servant.<br />

(j) In the case of sanction of family pension to the families of Government<br />

employees who died on or after reorganisation of the States,<br />

in whose cases records were lost due to floods or otherwise, it<br />

would be sufficient, if an applicant produces collateral evidence<br />

about the deceased employee’s service or if the number of the<br />

P.P.O. of the deceased pensioner is mentioned in the application.<br />

In such cases, family pension has to be sanctioned from the date<br />

of issue of the order based on the affidavit filed by a contemporary<br />

or co-pensioner before the Notary Public. If the number of<br />

P.P.O. is indicated in the application, it can be taken, after due verification<br />

of P.P.O. that the claim is bonafide one. However, if no<br />

particulars are available either in the application of the applicant<br />

or with the concerned department for verification,<br />

the claim may be rejected straightaway.<br />

These instructions are applicable in case of pensioners and family<br />

pensioners and employees who retired/ died before 1964 and also in case of<br />

employees whose service records are destroyed not on account of their fault,<br />

but on account of floods, fire etc., even subsequent to 1964.<br />
