Theoretically, penicill<strong>in</strong>s can cause an<br />

allergic reaction <strong>in</strong> the <strong>in</strong>fant. If the<br />

<strong>in</strong>fant develops a rash, it could be a sign<br />

<strong>of</strong> allergy. Stop the drug and choose an<br />

alternative if necessary. Cont<strong>in</strong>ue<br />

breastfeed<strong>in</strong>g. Warn the mother that the<br />

<strong>in</strong>fant should not be given the drug <strong>in</strong> the<br />

future. Amoxicill<strong>in</strong>: Compatible with<br />

breastfeed<strong>in</strong>g ampicill<strong>in</strong>: Compatible<br />

with breastfeed<strong>in</strong>g benzath<strong>in</strong>e<br />

benzylpenicill<strong>in</strong>: Compatible with<br />

breastfeed<strong>in</strong>g benzylpenicill<strong>in</strong>,<br />

cloxacill<strong>in</strong>: Compatible with<br />

breastfeed<strong>in</strong>g, phenoxymethylpenicill<strong>in</strong>,<br />

proca<strong>in</strong>e benzylpenicill<strong>in</strong> compatible<br />

with breastfeed<strong>in</strong>g.<br />

Restricted <strong>in</strong>dications:<br />

Amoxicill<strong>in</strong> clavulanic acid, ceftazidime,<br />

ceftriaxone, imipenem cilastat<strong>in</strong>:<br />

Compatible with breast-feed<strong>in</strong>g.<br />

Other antibacterial:<br />

Chloramphenicol: Avoid if possible,<br />

especially if the <strong>in</strong>fant is less than 1<br />

month old. Monitor the <strong>in</strong>fant for side<br />

effects (hemolysis, and jaundice).<br />

Theoretically, there is a risk <strong>of</strong> bone<br />

marrow depression, but this has never<br />

been reported, cipr<strong>of</strong>loxac<strong>in</strong>: Avoid if<br />

possible, until more data are available,<br />

doxycycl<strong>in</strong>e: Avoid if possible.<br />

Possibility <strong>of</strong> sta<strong>in</strong><strong>in</strong>g the <strong>in</strong>fant’s teeth.<br />

S<strong>in</strong>gle dose is probably safe<br />

erythromyc<strong>in</strong>: Compatible with<br />

breastfeed<strong>in</strong>g. Gentamic<strong>in</strong>: Compatible<br />

with breast feed<strong>in</strong>g. Monitor <strong>in</strong>fant for<br />

thrush and diarrhoea. Metronidazole:<br />

Avoid if possible.<br />

Animal data suggest it may be<br />

carc<strong>in</strong>ogenic. If given <strong>in</strong> s<strong>in</strong>gle dose <strong>of</strong> 2<br />

grams, discont<strong>in</strong>ue breastfeed<strong>in</strong>g for 12<br />

hours. Help the mother to express her<br />

breast milk <strong>in</strong> advance and store it <strong>in</strong> a<br />

refrigerator so that her baby can be fed<br />

by cup dur<strong>in</strong>g that time nalidixic acid.<br />

Avoid if possible, especially if the <strong>in</strong>fant<br />

is premature or less than 1 month old.<br />

Monitor the <strong>in</strong>fant for side effects<br />

(hemolysis and jaundice).<br />

Avoid <strong>in</strong> G-6-PD deficient <strong>in</strong>fants<br />

nitr<strong>of</strong>uranto<strong>in</strong>: Compatible with breastfeed<strong>in</strong>g<br />

for healthy full- term <strong>in</strong>fants.<br />

Avoid if possible if the <strong>in</strong>fant is<br />

premature or less than 1 month old.<br />

Monitor the <strong>in</strong>fant for side-effects<br />

(hemolysis and jaundice). Avoid <strong>in</strong><br />

<strong>in</strong>fants with G-6-PD deficiency<br />

spect<strong>in</strong>omyc<strong>in</strong>: sulfadiaz<strong>in</strong>e. Avoid if<br />

possible, especially if the <strong>in</strong>fant is<br />

premature or less than 1 month old.<br />

(bloody diarrhea, hemolysis and<br />

jaundice). Avoid <strong>in</strong> G-6-PD deficient<br />

<strong>in</strong>fants sulfamethoxazole trimethoprim<br />

(Cotrimoxazole): Compatible with<br />

breastfeed<strong>in</strong>g for older, healthy full term<br />

<strong>in</strong>fants. Avoid if possible if the <strong>in</strong>fant is<br />

premature or less than 1 month old.<br />

Monitor the <strong>in</strong>fant for side effects<br />

(Hemolysis and jaundice).<br />

Deficiency: Trimethoprim: Compatible<br />

with breast-feed<strong>in</strong>g.<br />
