Anticonvulsants or antiepileptic:<br />

General <strong>in</strong>formation: Breastfed <strong>in</strong>fants<br />

<strong>of</strong> mothers who are tak<strong>in</strong>g<br />

anticonvulsants sometimes develop<br />

drows<strong>in</strong>ess. For mothers who need<br />

anticonvulsants there is <strong>of</strong>ten little<br />

alternative. It is essential that they take<br />

their medication and it can be dangerous<br />

to change antiepileptic medic<strong>in</strong>es<br />

suddenly. Breastfeed<strong>in</strong>g is usually<br />

possible, but the <strong>in</strong>fant must be<br />

monitored. It helps if the dose is kept as<br />

low as possible with<strong>in</strong> the effective<br />

therapeutic range. Carbamazep<strong>in</strong>e:<br />

Compatible with breastfeed<strong>in</strong>g. Monitor<br />

<strong>in</strong>fant for side effects (jaundice,<br />

drows<strong>in</strong>ess, poor suckl<strong>in</strong>g, and vomit<strong>in</strong>g<br />

and poor weight ga<strong>in</strong>) diazepam:<br />

Compatible with breastfeed<strong>in</strong>g <strong>in</strong> s<strong>in</strong>gle<br />

dose. Avoid repeated doses, if possible.<br />

Monitor <strong>in</strong>fant for drows<strong>in</strong>ess<br />

ethosuximide: Avoid if possible.<br />

Monitor <strong>in</strong>fant for side-effects<br />

(drows<strong>in</strong>ess, poor suckl<strong>in</strong>g and poor<br />

weight ga<strong>in</strong>) magnesium sulfate<br />

compatible with breastfeed<strong>in</strong>g,<br />

phenobarbital: Compatible with<br />

breastfeed<strong>in</strong>g. Monitor <strong>in</strong>fant for side<br />

effects (drows<strong>in</strong>ess, poor suckl<strong>in</strong>g and<br />

poor weight ga<strong>in</strong>), Phenyto<strong>in</strong> compatible<br />

with breastfeed<strong>in</strong>g. Monitor <strong>in</strong>fant for<br />

side effects (cyanosis and methemoglob<strong>in</strong>emia)<br />

valproic acid: Compatible with<br />

breastfeed<strong>in</strong>g. Monitor <strong>in</strong>fant for side<br />

effects (jaundice).<br />

Complementary drug: Clonazepam:<br />

Compatible with breastfeed<strong>in</strong>g. The<br />

mother should receive only normal dose.<br />

Anti <strong>in</strong>fective drugs, Anthelm<strong>in</strong>tics:<br />

Intest<strong>in</strong>al anthelm<strong>in</strong>tics:<br />

General <strong>in</strong>formation: There are limited<br />

data available on the use <strong>of</strong> drugs <strong>in</strong> this<br />

category. However, they act ma<strong>in</strong>ly <strong>in</strong><br />

the <strong>in</strong>test<strong>in</strong>al system <strong>of</strong> the mother and<br />

little is absorbed <strong>in</strong>to the general system.<br />

They can be considered compatible with<br />

breastfeed<strong>in</strong>g.<br />

Albendazole: compatible with breastfeed<strong>in</strong>g<br />

levamisole: Compatible with<br />

breastfeed<strong>in</strong>g mebendazole: Compatible<br />

with breastfeed<strong>in</strong>g niclosamide: Compatible<br />

with breastfeed<strong>in</strong>g praziquantel.<br />

Compatible with breastfeed<strong>in</strong>g pyrantel:<br />

Compatible with breastfeed<strong>in</strong>g.<br />

Antifilarial:<br />

Diethylcarbamaz<strong>in</strong>e and ivermect<strong>in</strong>.<br />

Complementary drug:<br />

Suram<strong>in</strong> Sodium:<br />

Antischistosomal and other anti<br />

trematode drugs:<br />

Praziquantel: Compatible with breastfeed<strong>in</strong>g,<br />

triclabendazole.<br />

Complementary drug:<br />

Oxamniqu<strong>in</strong>e:<br />

Compatible with breastfeed<strong>in</strong>g<br />

Antibacterial:<br />

General <strong>in</strong>formation: If the drug is<br />

excreted <strong>in</strong> breast milk, there is a<br />

possibility <strong>of</strong> alter<strong>in</strong>g the <strong>in</strong>fant’s<br />

<strong>in</strong>test<strong>in</strong>al flora. Monitor the <strong>in</strong>fant for<br />

gastro<strong>in</strong>test<strong>in</strong>al disturbances, such as<br />

thrush and diarrhoea. If they occur, stop<br />

the drug and choose an alternative if<br />

necessary. Cont<strong>in</strong>ue breastfeed<strong>in</strong>g.<br />

Lactam drugs: General <strong>in</strong>formation:<br />

Breastfeed<strong>in</strong>g is generally safe.
