PART A: OFFENCE CHARACTERISTICS AND HISTORY<br />

Index Offence<br />

The index <strong>of</strong>fence is <strong>the</strong> <strong>of</strong>fence(s) that led to <strong>the</strong> most recent referral to Youth Forensic Psychiatric Services. The following codes should be given<br />

based on what <strong>the</strong> youth was originally charged with (i.e., not <strong>the</strong> <strong>of</strong>fence <strong>the</strong>y were convicted for):<br />

1 = Violent<br />

2 = Non-violent<br />

3 = Sexual<br />

4 = Violent <strong>an</strong>d Sexual<br />

Classify <strong>the</strong> <strong>of</strong>fence using <strong>the</strong> BC Corrections Coding Sheet. The index <strong>of</strong>fence doesn’t necessarily have to be only one <strong>of</strong>fence. The youth may<br />

have committed more <strong>of</strong>fences for which he/she is assessed, <strong>an</strong>d all <strong>of</strong> <strong>the</strong>m should be treated as index <strong>of</strong>fences. It is <strong>of</strong> crucial import<strong>an</strong>ce to<br />

accurately classify <strong>the</strong> index <strong>of</strong>fence.<br />

Age at Index Offence<br />

Refers to <strong>the</strong> date when index <strong>of</strong>fence actually happened – not when it was investigated, or charged, etc. This item is coded in months. If <strong>the</strong> index<br />

<strong>of</strong>fence happened over a longer period <strong>of</strong> time (e.g., a couple <strong>of</strong> years), code <strong>the</strong> youth’s age at <strong>the</strong> beginning <strong>of</strong> <strong>the</strong> <strong>of</strong>fending behaviour (<strong>the</strong><br />

beginning <strong>of</strong> that period).<br />

Victim-Offender Relationship (if 2 or more victims, code highest)<br />

5 = Very close relationship (immediate family member, rom<strong>an</strong>tic partner)<br />

4 = Close relationship (friend, relative, dating partner, etc.)<br />

3 = Specific relationship (teacher, babysitter, etc.) or between friend <strong>an</strong>d acquaint<strong>an</strong>ce<br />

2 = Acquaint<strong>an</strong>ce<br />

1 = Str<strong>an</strong>ger<br />

Severity <strong>of</strong> violence (First Column; consider actual harm to victim, not youth's intention)<br />

7 - Extreme homicide (e.g., multiple victims or multiple fatalities, mutilation)<br />

6 - Homicide<br />

5 - Severe injury (e.g., lasting impairment or life-threatening injury, some rapes)<br />

4 - Serious injury, requiring subst<strong>an</strong>tial hospital treatment (e.g., broken limb, rape, gunshot)<br />

3 - Minor injury (e.g., bruises, minor medical treatment, attempted rape)<br />

2 - Assault without injury<br />

1 - No assault (e.g., threatened with weapon)<br />

Severity <strong>of</strong> violence (Second Column)<br />

2 - Physical violence with weapon use 1 -Physical violence used, no weapon use 0 -No physical violence used<br />
