Note: wherever possible, <strong>and</strong> if <strong>the</strong> agency considers it appropriate, this record should<br />
be completed by <strong>the</strong> birth fa<strong>the</strong>r in his words. It may be used as an important source of<br />
information for <strong>the</strong> agency in making its decisions <strong>and</strong>, when <strong>and</strong> where appropriate, for<br />
<strong>the</strong> child <strong>and</strong> <strong>the</strong> adoptive parents in underst<strong>and</strong>ing <strong>the</strong> child’s background. It is<br />
important that <strong>the</strong> birth fa<strong>the</strong>r underst<strong>and</strong>s <strong>the</strong> importance of <strong>the</strong>se questions <strong>and</strong> <strong>the</strong><br />
reasons that <strong>the</strong>y are being asked. The form should be used wherever possible within<br />
<strong>the</strong> context of a counselling interview(s) (required in Regulation 14, AA Regulations<br />
2005).<br />
If you do not underst<strong>and</strong> <strong>the</strong> questions you are being asked or need help in answering<br />
<strong>the</strong>m, you should seek advice <strong>and</strong> support from somebody who is experienced in<br />
adoption matters <strong>and</strong> can help you with this. It is also important for you to underst<strong>and</strong><br />
that, whilst <strong>the</strong> adoption agency must take into account your wishes or feelings, <strong>the</strong><br />
adoption agency <strong>and</strong> <strong>the</strong> adoptive parents cannot be required to follow <strong>the</strong>m.<br />

What things would you like your child/children <strong>and</strong> <strong>the</strong> adopters to know about your<br />
childhood <strong>and</strong> <strong>the</strong> way that has influenced you? In time, it will be very important for your<br />
child to have a better underst<strong>and</strong>ing of your life <strong>and</strong> why it has sometimes been difficult<br />
for you to manage. It will help if your child has a picture of both <strong>the</strong> good things <strong>and</strong> <strong>the</strong><br />
difficult experiences you may have had whilst growing up.<br />
Simon has found <strong>the</strong> whole adoption process difficult to accept <strong>and</strong> he has not given<br />
any information about himself, his childhood or his extended family.<br />
From <strong>the</strong> reports <strong>and</strong> assessments completed it is clear that he has problems telling<br />
fantasy from reality <strong>and</strong> he does have some degree of learning disability although he is<br />
thought to have a higher IQ than his wife Tina.<br />
