General Field Trip Permission Form
As part of my educational program, I will take the children in my care on a variety of field trips
away from my home. The typical destinations of these field trips may include, but are not limited
to:
walks through the neighborhood
walks to the local park, library, senior citizen center, etc.
Once a year we will go to the Pumpkin patch and the Zoo by bus
Clients give their consent to these field trips by signing below.
