I do feel that bed rest and the quality of care at health <str<strong>on</strong>g>in</str<strong>on</strong>g>stituti<strong>on</strong>s<br />
<str<strong>on</strong>g>in</str<strong>on</strong>g> general, <str<strong>on</strong>g>in</str<strong>on</strong>g> the nurs<str<strong>on</strong>g>in</str<strong>on</strong>g>g homes <str<strong>on</strong>g>in</str<strong>on</strong>g> particular, I th<str<strong>on</strong>g>in</str<strong>on</strong>g>k a number of<br />
questi<strong>on</strong>s rema<str<strong>on</strong>g>in</str<strong>on</strong>g> to be answered.<br />
Who works <str<strong>on</strong>g>in</str<strong>on</strong>g> these facilities, or their qualificati<strong>on</strong>s, who evaluates<br />
these facilities, is of vital c<strong>on</strong>cern. They should be given greater<br />
efficiency and fiscal resp<strong>on</strong>sibility, and <str<strong>on</strong>g>in</str<strong>on</strong>g> the adm<str<strong>on</strong>g>in</str<strong>on</strong>g>istrati<strong>on</strong> of these<br />
<str<strong>on</strong>g>in</str<strong>on</strong>g>stituti<strong>on</strong>s, this should be a primary goal.<br />
Senator Moss. Is your report fairly c<strong>on</strong>temporary, or are you still<br />
see<str<strong>on</strong>g>in</str<strong>on</strong>g>g cases like these com<str<strong>on</strong>g>in</str<strong>on</strong>g>g <str<strong>on</strong>g>in</str<strong>on</strong>g>to your hospital?<br />
Dr. DOBEIN. These examples I collected essentially <strong>on</strong> a day's notice,<br />
with very little effort at all.<br />
Senator Moss. They are very c<strong>on</strong>temporary?<br />
Dr. DOBKIN. Yes.<br />
Senator Moss. Is dehydrati<strong>on</strong> someth<str<strong>on</strong>g>in</str<strong>on</strong>g>g that a patient would have,<br />
if he were hav<str<strong>on</strong>g>in</str<strong>on</strong>g>g full nurs<str<strong>on</strong>g>in</str<strong>on</strong>g>g care?<br />
Dr. DOBRKIN. Dehydrati<strong>on</strong> is essentially a very simple situati<strong>on</strong>.<br />
Senator Moss. There is not enough fluid.<br />
Dr. DOBKIN. There is not enough fluid, yes, a state of be<str<strong>on</strong>g>in</str<strong>on</strong>g>g dried out.<br />
It does not occur suddenly. The water does not suddenly fall out of<br />
the body. It reflects l<strong>on</strong>g-term imbalance of more go<str<strong>on</strong>g>in</str<strong>on</strong>g>g out than com<str<strong>on</strong>g>in</str<strong>on</strong>g>g<br />
<str<strong>on</strong>g>in</str<strong>on</strong>g>.<br />
Many of these patients could help to hydrate themselves ord<str<strong>on</strong>g>in</str<strong>on</strong>g>arily,<br />
but when it gets to a severe state, they need <str<strong>on</strong>g>in</str<strong>on</strong>g>travenous fluid.<br />
It is someth<str<strong>on</strong>g>in</str<strong>on</strong>g>g that I th<str<strong>on</strong>g>in</str<strong>on</strong>g>k is amenable to nurs<str<strong>on</strong>g>in</str<strong>on</strong>g>g homes.<br />
Senator Moss. It would occur mostly by not assist<str<strong>on</strong>g>in</str<strong>on</strong>g>g that pers<strong>on</strong> to<br />
take fluids <str<strong>on</strong>g>in</str<strong>on</strong>g>to his body?<br />
Dr. DOBKIN. Not assist<str<strong>on</strong>g>in</str<strong>on</strong>g>g them, and not help<str<strong>on</strong>g>in</str<strong>on</strong>g>g them take <str<strong>on</strong>g>in</str<strong>on</strong>g><br />
adequate fluid.<br />
Senator Moss. I know what the cause of bedsores are, but how do<br />
you prevent bedsores from people who are kept <str<strong>on</strong>g>in</str<strong>on</strong>g> bed?<br />
Dr. DOBEIN. There are a number of th<str<strong>on</strong>g>in</str<strong>on</strong>g>gs that cause bedsores.<br />
Elderly people's sk<str<strong>on</strong>g>in</str<strong>on</strong>g> is not as str<strong>on</strong>g as young people's, and it will be<br />
pr<strong>on</strong>e to break down.<br />
The more important aspect, is the fact that people are immobilized,<br />
they stay <str<strong>on</strong>g>in</str<strong>on</strong>g> <strong>on</strong>e positi<strong>on</strong> for a period of time, the entire width of the<br />
body is <str<strong>on</strong>g>in</str<strong>on</strong>g> <strong>on</strong>e positi<strong>on</strong>, <strong>on</strong> <strong>on</strong>e porti<strong>on</strong> of the sk<str<strong>on</strong>g>in</str<strong>on</strong>g>, this will cause it to<br />
break down.<br />
If they are not assured of ly<str<strong>on</strong>g>in</str<strong>on</strong>g>g <strong>on</strong> different parts of their body,<br />
mov<str<strong>on</strong>g>in</str<strong>on</strong>g>g around as much as possible. this will occur.<br />
Rough sheets, as you heard described, is the other problem.<br />
An important problem is wet sheets, where the patient will sit <str<strong>on</strong>g>in</str<strong>on</strong>g><br />
his ur<str<strong>on</strong>g>in</str<strong>on</strong>g>e, and feces, this will cause great risk for rapid breakdown<br />
of the sk<str<strong>on</strong>g>in</str<strong>on</strong>g>.<br />
Senator Moss. And bedsores usually become <str<strong>on</strong>g>in</str<strong>on</strong>g>fected if they hurt,<br />
is that right?<br />
Dr. DOBRKIN. They frequently do, to the po<str<strong>on</strong>g>in</str<strong>on</strong>g>t of death.<br />
Senator Moss. All right.<br />

You alluded to some <str<strong>on</strong>g>in</str<strong>on</strong>g>specti<strong>on</strong> reports, I would appreciate if you<br />
enter those fully <str<strong>on</strong>g>in</str<strong>on</strong>g> the record.<br />
One of those reports, which you related, went to the Towers Nurs<str<strong>on</strong>g>in</str<strong>on</strong>g>g<br />
Home, which, I understand, went for 5 days without heat for <strong>on</strong>e
