Food was not covered or <str<strong>on</strong>g>in</str<strong>on</strong>g>adequately covered, with trays that<br />
were be<str<strong>on</strong>g>in</str<strong>on</strong>g>g set up for lunch.<br />
The <str<strong>on</strong>g>in</str<strong>on</strong>g>terior of the refrigerator was splattered with an accumulati<strong>on</strong><br />
ui. food.<br />
I am just go<str<strong>on</strong>g>in</str<strong>on</strong>g>g down and pick<str<strong>on</strong>g>in</str<strong>on</strong>g>g out these th<str<strong>on</strong>g>in</str<strong>on</strong>g>gs. I am not go<str<strong>on</strong>g>in</str<strong>on</strong>g>g<br />
<strong>on</strong>e after the other. It would take forever.<br />
Food from the kitchen was cold when it reached the patient.<br />
Patients hav<str<strong>on</strong>g>in</str<strong>on</strong>g>g orders for low-salt diets, salt-free diets and regular<br />
diets are be<str<strong>on</strong>g>in</str<strong>on</strong>g>g served the same food.<br />
There was old hardened feces <strong>on</strong> a toilet stall wall, around the base<br />
of the commode and under the commode seat. Feces was smeared <strong>on</strong><br />
walls <str<strong>on</strong>g>in</str<strong>on</strong>g> several other areas of the home.<br />
Lumps of feces were <strong>on</strong> the baseboard ledge <str<strong>on</strong>g>in</str<strong>on</strong>g> a patient's room.<br />
Dried, hard, crusty, dirt l<str<strong>on</strong>g>in</str<strong>on</strong>g>ed two bathtubs.<br />
A str<strong>on</strong>g odor of ur<str<strong>on</strong>g>in</str<strong>on</strong>g>e and feces was present <str<strong>on</strong>g>in</str<strong>on</strong>g> several areas of<br />
the home.<br />
Several chairs and wheelchairs were food sta<str<strong>on</strong>g>in</str<strong>on</strong>g>ed and encrusted<br />
with dirt, and God knows what else.<br />
Unbagged soiled l<str<strong>on</strong>g>in</str<strong>on</strong>g>en filled the soiled l<str<strong>on</strong>g>in</str<strong>on</strong>g>en room and spilled over<br />
<str<strong>on</strong>g>in</str<strong>on</strong>g>to the corridor. The room was foul smell<str<strong>on</strong>g>in</str<strong>on</strong>g>g, and flies were swarm<str<strong>on</strong>g>in</str<strong>on</strong>g>g.<br />
Barrels filled with powder, which appeared to be soap, was dirt<br />
speckled, clumped together from dampness, and was unlabeled.<br />
Patients' pers<strong>on</strong>al cloth<str<strong>on</strong>g>in</str<strong>on</strong>g>g is sent to the floor unir<strong>on</strong>ed.<br />
There are two more pages of violati<strong>on</strong>s, but they do not deal directly<br />
with patient care. They deal with ma<str<strong>on</strong>g>in</str<strong>on</strong>g>tenance of the facility and<br />
several other th<str<strong>on</strong>g>in</str<strong>on</strong>g>gs, and these are the th<str<strong>on</strong>g>in</str<strong>on</strong>g>gs we found.<br />
These are not uncomm<strong>on</strong> th<str<strong>on</strong>g>in</str<strong>on</strong>g>gs.<br />
Senator Moss. I want to ask you this, this is a particularly bad<br />
<strong>on</strong>e, but were there others like this?<br />
Ms. JARVIs. Yes. Perhaps they would not have received a letter<br />
of this magnitude. This is just <strong>on</strong>e letter. Perhaps they would not<br />
have received a letter that l<strong>on</strong>g, but we have found these th<str<strong>on</strong>g>in</str<strong>on</strong>g>gs <str<strong>on</strong>g>in</str<strong>on</strong>g><br />
other homes.<br />
It is <strong>on</strong> a smaller scale, but the same th<str<strong>on</strong>g>in</str<strong>on</strong>g>g.<br />
Senator Moss. This was d<strong>on</strong>e dur<str<strong>on</strong>g>in</str<strong>on</strong>g>g the time you were mak<str<strong>on</strong>g>in</str<strong>on</strong>g>g<br />
your <str<strong>on</strong>g>in</str<strong>on</strong>g>specti<strong>on</strong>s for the New York City Department of Health?<br />
Ms. JARVIS. The New York City Department of Health.<br />
I went over to the Federal programs evaluat<str<strong>on</strong>g>in</str<strong>on</strong>g>g nurs<str<strong>on</strong>g>in</str<strong>on</strong>g>g homes for<br />
participati<strong>on</strong> <str<strong>on</strong>g>in</str<strong>on</strong>g> the Federal medicare program, but this was d<strong>on</strong>e<br />
while I was do<str<strong>on</strong>g>in</str<strong>on</strong>g>g <str<strong>on</strong>g>in</str<strong>on</strong>g>specti<strong>on</strong>s for the New York City Department of<br />
Health.<br />
Senator Moss. When you made a report like that, what became of<br />
it, what happened?<br />
Ms. JARvIS. What becomes of the report, or what becomes of the<br />
whole survey?<br />
Senator Moss. Well, what becomes of the report, and then if anyth<str<strong>on</strong>g>in</str<strong>on</strong>g>g<br />
follows from it go<str<strong>on</strong>g>in</str<strong>on</strong>g>g <str<strong>on</strong>g>in</str<strong>on</strong>g>.<br />
MS. JARVIS. Well, we would get back to the office, and we would<br />
write up the report <str<strong>on</strong>g>in</str<strong>on</strong>g> detail.<br />
We will go back <str<strong>on</strong>g>in</str<strong>on</strong>g>to the facility <str<strong>on</strong>g>in</str<strong>on</strong>g> about 3 weeks to see just<br />
how much of this has been cleared up.<br />
