commitment should be made to the PADD until eligibility <strong>of</strong> the decedent for burial in a Government cemetery has<br />

been established <strong>and</strong> the cemetery has available grave space.<br />

(1) AR 290–5 for Arlington National Cemetery <strong>and</strong> the U.S. Soldiers’ <strong>and</strong> Airmen’s Home National Cemetery.<br />

(2) AR 210–190 for post cemeteries other than the U.S. Military Academy, West Point.<br />

(3) VA–DMA–IS–1 for interments in national cemeteries.<br />

b. State Veterans’ cemeteries. Numerous State Veterans’ cemeteries are located throughout the United States. If the<br />

PADD is interested, more information can be obtained by contacting the superintendent at the State Veterans’<br />

cemetery, the Office <strong>of</strong> Veterans’ Cemeteries program manager, or the executive director <strong>of</strong> the State Veterans’<br />

Commission.<br />

13–4. Arranging funeral <strong>and</strong> interment services<br />

a. The PADD is responsible for arranging <strong>and</strong> contracting for funeral <strong>and</strong> interment services for remains that are<br />

individually identified. The PADD’s contracted funeral home should make all funeral <strong>and</strong> interment arrangements to<br />

include coordinating burial in a Government cemetery. The CAO should assist the funeral home as requested.<br />

b. The <strong>Army</strong> is responsible for arranging <strong>and</strong> contracting for funeral <strong>and</strong> interment services for group remains, the<br />

unclaimed remains <strong>of</strong> a person who dies on an <strong>Army</strong> installation, <strong>and</strong> when the PADD selects the direct consignment<br />

to a Government cemetery <strong>Army</strong> preparation option.<br />

c. CAC personnel will not enter into a funeral or interment contract for the disposition <strong>of</strong> individually identified<br />

remains without prior approval <strong>of</strong> CDR, PERSCOM (TAPC–PED–D).<br />

13–5. Request for authority to inter in a Government cemetery<br />

a. Request for authority to inter remains in a Government cemetery will be directed to the director or superintendent<br />

<strong>of</strong> the cemetery in which burial is desired. The request may be by telephone <strong>and</strong> confirmed by letter or by message.<br />

Data normally required by cemetery <strong>of</strong>ficials are stated below.<br />

(1) Active duty soldiers: name, rank, SSN, date <strong>and</strong> place entered active duty, date <strong>of</strong> birth, <strong>and</strong> that the individual<br />

was serving in an active, honorable status at time <strong>of</strong> death.<br />

(2) Spouse <strong>and</strong> minor dependents (dependents who are unmarried <strong>and</strong> under the age <strong>of</strong> 21 years) <strong>of</strong> active duty<br />

soldiers: information in (1) above plus name, date <strong>of</strong> birth, <strong>and</strong> date <strong>of</strong> death <strong>of</strong> the dependent. (For Arlington National<br />

Cemetery, DA Form 2386 (Agreement for Interment) will be used.) The DA Form 2386 is available in the back <strong>of</strong> DA<br />

Pam 290–5.<br />

(3) Unmarried adult children <strong>of</strong> active duty soldiers who, after attaining the age <strong>of</strong> 21 years <strong>and</strong> until completion <strong>of</strong><br />

education or training (but not after attaining the age <strong>of</strong> 23 years) are pursuing a course <strong>of</strong> instruction at an approved<br />

educational institution): same data as required in (2) above.<br />

(4) Unmarried adult children <strong>of</strong> active duty soldiers who have become permanently incapable <strong>of</strong> self-support<br />

because <strong>of</strong> a physical or mental disability incurred before attaining the age <strong>of</strong> 21 years: name <strong>of</strong> decedent, date <strong>and</strong><br />

place <strong>of</strong> birth, date <strong>of</strong> death, relationship to the sponsor, statement <strong>of</strong> the decedent’s martial status, statement <strong>of</strong> degree<br />

<strong>of</strong> dependency, <strong>and</strong> statement <strong>of</strong> an attending physician about the nature <strong>and</strong> duration <strong>of</strong> the physical or mental<br />

disability. Sponsor information: name, rank, SSN or service number, branch <strong>of</strong> Service, <strong>and</strong> date <strong>and</strong> place entered<br />

active duty.<br />

(5) Civilian employees who are former members <strong>of</strong> the Armed Forces: name, rank, SSN or service number, branch<br />

<strong>of</strong> service, VA claim number, date <strong>and</strong> place <strong>of</strong> birth, date <strong>of</strong> death, <strong>and</strong> date <strong>and</strong> place <strong>of</strong> enlistment <strong>and</strong> separation<br />

from the Service. Information about qualifying awards received is needed for Arlington National Cemetery only. Pro<strong>of</strong><br />

<strong>of</strong> type <strong>of</strong> discharge will be required by cemetery <strong>of</strong>ficials.<br />

(6) Spouse <strong>and</strong> minor dependents (dependents who are unmarried <strong>and</strong> under the age <strong>of</strong> 21 years) <strong>of</strong> veterans:<br />

Information in (5) above, plus name, date <strong>of</strong> birth <strong>and</strong> date <strong>of</strong> death <strong>of</strong> dependent. (For Arlington National Cemetery,<br />

DA Form 2386 will be used.)<br />

(7) Unmarried adult children <strong>of</strong> veterans who, after attaining the age <strong>of</strong> 21 years <strong>and</strong> until completion <strong>of</strong> education<br />

or training (but not after attaining the age <strong>of</strong> 23 years) are pursuing a course <strong>of</strong> instruction at an approved educational<br />

institution): same data will be submitted as required in (6) above.<br />

(8) Unmarried adult children <strong>of</strong> veterans indicated in (5) above who have become permanently incapable <strong>of</strong> selfsupport<br />

because <strong>of</strong> a physical or mental disability incurred before attaining the age <strong>of</strong> 21 years: name <strong>of</strong> decedent, date<br />

<strong>and</strong> place <strong>of</strong> birth, date <strong>of</strong> death, relationship to the sponsor, statement <strong>of</strong> the decedent’s marital status, statement <strong>of</strong><br />

degree <strong>of</strong> dependency, <strong>and</strong> statement by an attending physician about the nature <strong>and</strong> duration <strong>of</strong> the physical or mental<br />

disability. Sponsor information: name, rank, SSN or service number, branch <strong>of</strong> Service, VA claim number, <strong>and</strong> date<br />

<strong>and</strong> place <strong>of</strong> enlistment <strong>and</strong> separation from the Service. Information on qualifying awards received is needed for<br />

Arlington National Cemetery only.<br />

b. Required supporting documents stated above will accompany remains to the respective cemetery or will be<br />

provided by the PADD.<br />

c. Verification <strong>of</strong> eligibility for interment in a Government cemetery must be accomplished before remains are<br />

shipped when remains will be directly consigned to a Government cemetery.<br />
