eceiving funeral, ground transportation arrangements will be made by the preparing CAC within the United States or<br />

the Director, Mortuary Affairs outside the United States. Transportation will be paid by the CAC to the contractor.<br />

b. When commercial air transportation is used to deliver the remains to the commercial airport serving the receiving<br />

funeral home with subsequent ground transportation to the receiving funeral home.<br />

(1) Commercial air transportation arrangements will be made by the preparing CAC within the United States or the<br />

Director, Mortuary Affairs outside the United States. Transportation expenses will be prepaid by the CAC<br />

(2) Ground transportation arrangements from the commercial airport to the receiving funeral will be made by the<br />

receiving funeral home. The preparing CAC within the United States or the Director, Mortuary Affairs outside the<br />

United States must coordinate commercial air carrier schedules with the receiving funeral home. Transportation<br />

expenses will be reimbursed to the PADD as part <strong>of</strong> the funeral <strong>and</strong> interment claim.<br />

11–12. Transportation <strong>of</strong> remains <strong>of</strong> dependents <strong>of</strong> military personnel<br />

The remains <strong>of</strong> dependents <strong>of</strong> military personnel (para 2–14) may be transported to decedent’s home, or any other<br />

place determined to be the appropriate place <strong>of</strong> interment. The shipment <strong>of</strong> remains <strong>of</strong> dependents <strong>of</strong> soldiers is<br />

arranged as follows:<br />

a. Within the United States. All transportation arrangements will be made by the PADD’s contracted funeral home.<br />

The CAC will not arrange transportation <strong>of</strong> remains using Casualty <strong>and</strong> Mortuary Affairs Open Allotment funds<br />

without the prior approval <strong>of</strong> CDR, PERSCOM (TAPC–PED–D).<br />

b. Outside the United States.<br />

(1) When ground transportation is the sole method used to transfer the remains from the preparing mortuary to the<br />

receiving funeral, ground transportation arrangements will be made by the Director, Mortuary Affairs. Transportation<br />

will be paid by the mortuary to the contractor.<br />

(2) When commercial air transportation is used to deliver the remains to the commercial airport serving the<br />

receiving funeral home with subsequent ground transportation to the receiving funeral home—<br />

(a) Commercial air transportation arrangements will be made by the Director, Mortuary Affairs preparing the<br />

remains. Transportation will be paid by the mortuary to the commercial carrier.<br />

(b) Ground transportation arrangements from the commercial airport to the receiving funeral will be made by the<br />

receiving funeral home. The Director, Mortuary Affairs preparing the remains must coordinate commercial air carrier<br />

schedules with the receiving funeral home <strong>and</strong> the CAC responsible for the geographic area <strong>of</strong> the receiving funeral<br />

home. Transportation expenses will be reimbursed to the PADD as part <strong>of</strong> the funeral <strong>and</strong> interment claim .<br />

11–13. Payment <strong>of</strong> transportation <strong>of</strong> deceased dependents <strong>of</strong> active duty soldier<br />

a. The CAC will initiate a DD Form 1375 <strong>and</strong> provide the form to the soldier or other interested person, when the<br />

soldier is not available, to submit with the claim for reimbursement <strong>of</strong> transportation <strong>of</strong> remains expenses.<br />

(1) The CAC completes items 1, 2, 3, 4, 6, 7, 8, 9, <strong>and</strong> 10.<br />

(2) The soldier completes items 12, 13, 16, <strong>and</strong> 18.<br />

(3) Items 5, 11, 14, 15, <strong>and</strong> 17 are left blank<br />

b. The soldier will submit the DD Form 1375 with a copy <strong>of</strong> the signed service contract with the funeral home,<br />

GPL, CPL (as appropriate), a certificate <strong>of</strong> death, <strong>and</strong> DA Form 5327.<br />

c. The claim will be submitted to either the CAC providing assistance to the soldier at home station or place <strong>of</strong><br />

interment. Prior to paying the claim, the CAC must verify the claim has not been previously paid. Claim payment<br />

information can be obtain from DCIPS.<br />

11–14. <strong>Remains</strong> <strong>of</strong> Retired military <strong>and</strong> their dependents<br />

a. Retired military personnel, continuously hospitalized. The transportation <strong>of</strong> remains for retired military personnel<br />

who are continuously hospitalized from the date <strong>of</strong> retirement until the date <strong>of</strong> death (see para 2–9a) are arranged in<br />

the same manner as soldiers who die on active duty.<br />

b. Within the United States. <strong>Remains</strong> <strong>of</strong> military retirees <strong>and</strong> their dependents (see para 2–9 <strong>and</strong> 2–16) may be<br />

transported to a place <strong>of</strong> burial that is no further distance than the decedent’s last place <strong>of</strong> residence, nearest<br />

Government cemetery accepting new interments, or Government cemetery where the retiree or dependent <strong>of</strong> a retiree<br />

has an interment agreement. Amount paid for transportation may not exceed cost <strong>of</strong> transportation from place <strong>of</strong> death<br />

to decedent’s last place <strong>of</strong> residence unless the remains are transported to the nearest Government cemetery accepting<br />

new interments, or Government cemetery where the retiree or dependent <strong>of</strong> a retiree has an interment agreement. When<br />

place <strong>of</strong> death <strong>and</strong> place <strong>of</strong> burial are local to the residence, transportation is authorized for removal <strong>of</strong> remains from<br />

place <strong>of</strong> death to a local funeral home <strong>and</strong> from a local funeral home to a local cemetery. Transportation may not be to<br />

a place outside the United States.<br />

(1) All transportation arrangements will be made by the PADD’s contracted funeral home. The CAC will not<br />

arrange transportation <strong>of</strong> remains or prepay transportation expenses with Casualty <strong>and</strong> Mortuary Affairs Open Allotment<br />

funds without the prior approval <strong>of</strong> CDR, PERSCOM (TAPC–PED–D).<br />

(2) When retiree qualifies for transportation by the VA under chapter 23, title 38, United States Code, the retiree’s<br />
