1–14. National Guard soldiers<br />
The Casualty <strong>and</strong> Mortuary Affairs Open Allotment funds may be initially cited for the primary care <strong>and</strong> transportation<br />
for the remains <strong>of</strong> eligible National Guard soldiers. National Guard Bureau funds will be used to reimburse secondary<br />
care expenses <strong>and</strong> transportation expenses occurring after the remains arrive at the receiving funeral home.<br />
a. Primary care.<br />
(1) When the PADD <strong>of</strong> a deceased National Guard soldier elects the <strong>Army</strong> arranged disposition option (see para<br />
4–10) primary care expenses may be initially paid from the Casualty <strong>and</strong> Mortuary Affairs Open Allotment. The<br />
responsible mortuary <strong>of</strong>ficer then will prepare a St<strong>and</strong>ard Form (SF) 1080 (Voucher for Transfer Between Appropriations<br />
<strong>and</strong>/or Funds) <strong>and</strong> send it to the U.S. property <strong>and</strong> fiscal <strong>of</strong>ficer (USPFO) <strong>of</strong> the State in which the individual was<br />
a soldier. The State USPFO will enter the date <strong>of</strong> payment <strong>and</strong> check number on the voucher <strong>and</strong> send an information<br />
copy to CDR, PERSCOM (TAPC–PED–D), Alex<strong>and</strong>ria, VA 22331–0482.<br />
(2) When the PADD <strong>of</strong> a deceased National Guard soldier elects the family arranged disposition option (see para<br />
4–11) primary care expenses are reimbursed by directly submitting, a Department <strong>of</strong> Defense (DD) Form 1375<br />
(Request for Payment <strong>of</strong> Funeral <strong>and</strong>/or Interment Expenses) with a copy <strong>of</strong> the itemized funeral bills, a certified copy<br />
<strong>of</strong> the death certificate, the funeral home’s general price list (GPL) <strong>and</strong> a copy <strong>of</strong> the deceased’s orders or the unit’s<br />
DA Form 1379 (U.S. <strong>Army</strong> Reserve Components Unit Record <strong>of</strong> Reserve Training) to Chief, National Guard Bureau<br />
(NGB–ARL–LS), South George Mason Drive, Arlington, VA 22204–1382. The claim will be evaluated <strong>and</strong> allowance<br />
paid to the claimant.<br />
b. Secondary care. Secondary care expenses are reimbursed by directly submitting, a DD Form 1375 with a copy <strong>of</strong><br />
the itemized funeral bills, a certified copy <strong>of</strong> the death certificate, GPL, <strong>and</strong> a copy <strong>of</strong> the deceased’s orders or the<br />
unit’s DA Form 1379 to Chief, National Guard Bureau (NGB–ARL–LS), South George Mason Drive, Arlington, VA<br />
22204–1382. The claim will be evaluated <strong>and</strong> allowance paid to the claimant. A separate DD Form 1375 is not<br />
required for payment <strong>of</strong> the primary <strong>and</strong> secondary care allowances when the PADD elects the family arranged<br />
disposition option.<br />
c. Transportation <strong>of</strong> remains.<br />
(1) When the PADD <strong>of</strong> a deceased National Guard soldier elects the <strong>Army</strong> arranged disposition option (see para<br />
4–10) transportation expenses to the place designated by the PADD may be initially paid from the Casualty <strong>and</strong><br />
Mortuary Affairs Open Allotment. The responsible mortuary <strong>of</strong>ficer then will prepare an SF 1080 (Voucher for<br />
Transfer Between Appropriations <strong>and</strong>/or Funds) <strong>and</strong> send it to the U.S. property <strong>and</strong> fiscal <strong>of</strong>ficer (USPFO) <strong>of</strong> the State<br />
in which the individual was a soldier. The State USPFO will enter the date <strong>of</strong> payment <strong>and</strong> check number on the<br />
