EDITORIAL<br />

We are proud to present to you the 3 edition of our newsletter 'Nayan'. A lot has been<br />

achieved since the last newsletter was published.<br />

We are very pleased to announce the opening of our new venture at Pushpanjali<br />

Crosslays Hospital, a 400 bedded multispecialty hospital in Ghaziabad, NCR. The new<br />

centre was inaugurated on 16 August. It features a comprehensive diagnostic and<br />

treatment facility for all ophthalmic conditions.<br />

We have also started an exclusive tie up for retinal services at Asian institute of medical<br />

sciences in Faridabad. The team of retina specialists will cater to all spectrums of<br />

retinal diseases of patients coming to Asian institute of medical sciences. We are also<br />

participating in ROP diagnosis treatment in close association with major hospitals in<br />

Delhi and NCR like Mata Chanan Devi Hospital and Pushpanjali Crosslays Hospital.<br />

Our academic highlights were an invitation by the National Board of Examinations to<br />

prepare teaching modules for ophthalmology DNB program. Cataract, cornea and<br />

vitreo-retina modules have already been telecast live on national television under the<br />

aegis of the IGNOU DNB program. The next module on Glaucoma will be telecast in<br />

October.<br />

We also presented our various clinical and research papers at prestigious international<br />

conferences of the European society of cataract and refractive surgery (ESCRS) in<br />

Paris as well as at the Asia Pacific academy of Ophthalmolgy (APAO) at Beijing.<br />

To further add to the world class vitreo-retina surgery setup that we have, the<br />

Constellation vision system-the latest vitreous surgery system in the world has been<br />

installed at SEC.<br />

The Carl Zeiss Lumera 700, which is latest state of the art operating microscope with<br />

'resight inverting system' has also been installed at SEC this microscope offers<br />

unparralled optical clarity and is excellent for both anterior as well as posterior segment<br />

surgeries.<br />

As we humbly share our achievements with you through this issue of Nayan, it will be a<br />

privilege to get your valued responses and suggestions to us in our quest for a 'vision of<br />

excellence'.<br />

The technology for safe vitreous surgery<br />

has evolved rapidly since Machemer<br />

introduced the first commercial<br />

vitrectomy system in 1972.<br />

In the fall of 2008, Alcon surgical<br />

launched the CONSTELLATION vision<br />

system.<br />

The new system boasts of several<br />

advantages and advances over conventional vitrectomy systems.<br />

It can be used for conventional 20 Gauge as well as latest 23 and<br />

25 gauge vitrectomies.<br />

DUTY CYCLE- in the past surgeons had control over 2<br />

surgical parameters: cut rate and vacuum. Surgeons could<br />

not determine the duration for which the vitrectomy port<br />

would remain open or closed. Now this too is possible. The<br />

constellation system has 3 duty cycle setting options:<br />

Shave- a closed bias mode where the port is closed a<br />

majority of the time.<br />

Core-an open bias mode, where it is open most of the<br />

time.<br />

50-50 cycle.<br />

Thus with the closed priority cycle, the probe is closed<br />

between each cut and flow is absent, hence cutting on the<br />

retinal surface can be very controlled making retinal<br />

surgery safer and more controlled.<br />

The ULTRAVIT PROBE used with the Constellation vision<br />

system is an advanced pneumatic dual drive guillotine<br />

cutter.<br />

IOP CONTROL Integrated<br />

pressurized infusion and servo control of intraocular<br />

pressure is a unique and invaluable feature. IOP control<br />

reduces sudden hypotony intraoperatively.<br />

This control would enable lower average infusion<br />

pressures, which make it safer in patients with<br />

compromised optic nerves and diabetics.<br />

IMPROVED FLUIDICS: a sensor is provided that<br />

measures infusion fluid flow through the cassette and on<br />

delivery to the patient.<br />

Automated safeguards built into the system prevent<br />

excessive flow rates during cutting and extrusion, there by<br />

reducing the occurrence of a soft eye during surgery.<br />

ILLUMINATION- The system includes 2 dual- port xenon<br />

illumination sources. It uses mirrors rather than beam<br />

splitting to deliver light through the system's illumination<br />

ports.<br />

The illumination provided with the xenon source is far<br />

superior to the traditional halogen light source.<br />

To conclude the CONSTELLATION vision system offers many<br />

new and innovative additions to current vitreous surgery<br />

techniques. These advantages should lead to better surgical<br />

techniques and safer vitrectomy, leading to better results for<br />

patients.

A 24 year old patient presented with progressive change in glasses power. He was using RGP lenses but had poor compliance. He<br />

underwent corneal collagen crosslinkage with riboflavin followed by another corneal procedure. The schiempflug image of the cornea is<br />
