My name is Banyana Madi and I am a midwife, studying for a doctorate at the<br />

University of Surrey. I would like to invite you to participate in my study looking<br />

into how pregnant women make decisions about the most appropriate place of<br />

birth. I will therefore ask those wishing to take part in the study to take me<br />

through the process of how they came to decide on where they would like to<br />

deliver. This might include information about:<br />

Your understanding of the process of labour and delivery<br />

What you think the appropriate management should be<br />

Your knowledge of the options available about possible places of birth<br />

Your source of the information about the available options<br />

Where you thought you would like to deliver when you first discovered you were<br />

pregnant<br />

Whether this has changed at all<br />

Whether you have any prior experience of the place where you want to deliver<br />

What you think is the thing that most influenced your decision<br />

What role your partner has on the decision<br />

Whether you think you have adequate information upon which to make your<br />

choice<br />

Whether you think you made a choice about where you want to deliver or whether<br />

you just followed what most people do<br />

What interests you most about your planned place of birth<br />

What you don't like about the alternative place<br />
