6. The Monthly <strong>Bowel</strong> Chart<br />

Some residents require ongoing monitoring of their<br />

bowel elimination pattern to assist you to identify<br />

whether or not aperients or other medications are<br />

required. The Monthly <strong>Bowel</strong> Chart is designed<br />

<strong>for</strong> this purpose. The best time to commence it is<br />

following completion of the resident’s continence<br />

assessment.<br />

By completing the Monthly <strong>Bowel</strong> Chart, you will<br />

obtain the following in<strong>for</strong>mation:<br />

● The day that the<br />

resident has a bowel<br />

motion <strong>and</strong> on AM,<br />

PM or night shift.<br />

● The type of bowel<br />

movement (refer<br />

to the Bristol Stool<br />

Form Scale on the<br />

<strong>Bowel</strong> Chart).<br />

● Whether or not<br />

the resident was<br />

incontinent of faeces.<br />

● Whether or not the<br />

resident was given an<br />

aperients/suppository.<br />
