The antiseptic<br />
radiopaque inner layer<br />
prevents colonization<br />

of pure silicone. The<br />
antiseptic effect is<br />
achieved by diffusion of<br />
silver ions from the inner<br />
layer. The pure silicone<br />
prevents micro fractures<br />

Product Description<br />
Silverline Ventricular Shunt Catheters<br />
are made of antiseptic radiopaque<br />
implant grade silicone. They have<br />
numbered centimeter marks. A stylet<br />
and a trocar are delivered with the<br />
catheter.<br />
Silverline Peritoneal Shunt Catheters<br />
are made of two layers. The inner<br />
layer is made of antiseptic radiopaque<br />
implant grade silicone. The<br />
outer layer is made of pure silicone.<br />
By this construction prevention of<br />
micro- fractures and calcification is<br />
achieved.<br />
Silverline Shunt Catheters are<br />
compatible with all manufacturers´<br />
shunt valves.<br />

Stylet 8F ‘Heese’<br />
The hollow stylet is equipped with holes<br />
that correspond to the holes in the<br />
catheter. Flow of <strong>CSF</strong> out of the stylet<br />
indicates a correct position in the<br />
ventricle. A turn of 90 degrees of the<br />
stylet shuts off the flow of <strong>CSF</strong>.<br />

External Ventricular <strong>Drainage</strong><br />
Catheter with Cranial Bolt<br />
The ventricular catheter is fixed in<br />
the bone by means of a bolt. After<br />
making a burr hole and opening the<br />
dura the probe is placed in the<br />
ventricle with the bolt in place in the<br />
upper region of the catheter.<br />
The catheter has two<br />
sets of numbered depth<br />
marks. The first set is<br />
related to the tip of the<br />
catheter.<br />
The drill bit has an<br />
adjustable stop.<br />
The Allen wrench ist<br />
used to adjust the stop.<br />
The dura opener is used<br />
to perforate the dura<br />
mater.<br />
Accessories<br />
With the clamping nut<br />
the probe is fixed in the<br />
bolt.<br />
Then the bolt is slid down to the<br />
burr hole and screwed into the<br />
bone. Finally the probe is fixed in the<br />
bolt with the clamping nut. The<br />
Luer-Lock connector is placed in the<br />
drainage tube and connected to a<br />
drainage kit.<br />
The second set of depth<br />
marks is related to the<br />
upper edge of the bolt.<br />
Manometer Tube<br />
The manometer tube is used to<br />
determine the opening pressure. It is<br />
connected to the hollow stylet of a<br />
Heese-Catheter. Opening pressure can<br />
then be observed from the liquid level. The manometer tube is<br />
outfitted with lettered<br />
marks.<br />

The External Ventricular <strong>Drainage</strong> Kit<br />
drains and collects cerebrospinal fluid<br />
for external ventricular drainage.<br />
The kit is comprised of a tube<br />
connector with a Luer-Lock for<br />
connection to the Ventricular<br />
Catheter, a drip chamber, and a<br />
replaceable drainage bag. The tube<br />
connector contains an access port. A<br />
three-way stopcock to connect a proximal<br />
pressure transducer, a threeway<br />
stopcock to connect a distal<br />
pressure transducer, a pump chamber<br />
and a check valve are provided.<br />
The drip chamber is aerated with a<br />
replaceable filter.<br />
All tube connectors except the ones<br />
for the filter and drainage bag are<br />
permanently glued in place. The<br />
diameters of all connecting components<br />
and stopcocks are the same<br />
as that of the ventricular catheter,<br />
thereby avoiding narrowing that can<br />
cause clogging.<br />
A special version for use with the<br />
<strong>Spiegelberg</strong> Compliance-Monitor is<br />
available. In it the tube from the distal<br />
three-way stopcock to the drip<br />
chamber is made of compressible<br />

The External Ventricular <strong>Drainage</strong> Bag<br />
drains and collects cerebrospinal fluid<br />
for external ventricular drainage,<br />
external subdural drainage and<br />
external lumbar drainage. It is<br />
indicated whenever accurate<br />
measuring of drained fluid and exact<br />
levelling are not essential.<br />

Holder for securing the EVD-Kit to<br />

way that the zero mark is located at the<br />

Monro. A swivel joint adjusts the holder<br />

The Velcro straps are used to fix the<br />

EVD-Holder ‘Freiburg’<br />
In the holder ‘Freiburg’ the rod is<br />
fixed to a plate, which is placed unter<br />
the mattress of the bed. The weight<br />
of the matress and the weight of the<br />
patient fix the holder securely in<br />
place.<br />
The holder is adjusted such that the<br />
zero mark is at the elevation of the<br />
Foramen of Montro of the patient.<br />
When the elevation of the head-part<br />
of the bed is changed, the elevation<br />
of the drip chamber in relation to the<br />
foramen of Monro remains unchanged.<br />
A swivel joint adjusts the holder to a<br />

Scientific Information about Silverline ® Technology<br />
Can we eliminate <strong>CSF</strong>-drainage infection?<br />
Infections are a serious complication of <strong>CSF</strong>drainage<br />
systems. Even with best surgical<br />
technique and utmost aseptic handling measures<br />
infections cannot be avoided completely (1, 2,<br />
4).<br />
Impregnating ventricular catheters with a<br />
combination of Rifampicin and Minocyclin, or<br />
Rifampicin and Clindamycin reduces the<br />
likelihood of catheters to become colonized and<br />
reduces the frequency of positive <strong>CSF</strong> cultures<br />
(6). However, prophylactic use of antibiotics<br />
carries the risk of infection with antibiotic<br />
resistant organisms including opportunistic<br />
infection due to organisms such as candida (8).<br />
Silver as an antimicrobial substance has been<br />
known for centuries (3). It has been used in<br />
central venous catheters for some time (Erlangen<br />
Silver Catheter).<br />
Method<br />
Particles of silver and particles of an an insoluble<br />
silver salt are incorporated in the polyurethane<br />
or silicone material of Silverline Catheters.<br />
Silver ions have a strong antiseptic effect with a<br />
broad spectrum (3, 5). Should a resistancy to<br />
silver develop under the influence of the the<br />
silver ions, the antibiotic therapy will still be<br />
possible.<br />
The particles have a large specific surface<br />
resulting in the release of silver ions in antiseptic<br />
concentration on the surface of the catheter.<br />
There are two phases to the release of the<br />
antiseptic agent: In phase one the silver salt<br />
releases silver ions in antiseptic concentration.<br />
Phase one starts immediately after placement of<br />
the catheter and lasts for several hours. In phase<br />
two silver ions are released from the metallic<br />
silver. Phase two begins some hours aftert<br />
catheter placement and it lasts for a very long<br />
time.<br />
Silver inactivates the enzyme systems with SHgroups<br />
of the cell wall that are active in the<br />
transmembraneous energy metabolism. Silver<br />
ions block the breathing chain of the bacteria<br />
(5).<br />
Results<br />
Using the roll-culture method Silverline<br />
catheters were subjected to different suspensions<br />
of bacteria for various fixed time intervals. After<br />
that, roll-cultures were done. For control<br />
purposes non-antimicrobial catheters were<br />
subjected to the same test (7).<br />
Silverline catheters demonstrated antimicrobial<br />
effects on:<br />
- Staph. aureus,<br />
- Staph. epidermidis,<br />
- Methicillin resistant Staphylococcus,<br />
- E. coli,<br />
- Pseudomonas aeruginosa,<br />
- Clostridium difficile,<br />
- Acinetobacter baumannii,<br />
- Streptococcus intermedius,<br />
- Candida albicans, and<br />
- Candida glabrata.<br />
The figure shows cultures of the Silverline<br />
catheter and of a non-antimicrobial control made<br />
of the same material after subjecting them to a<br />
suspension of Staph. aureus. The top row shows<br />
cultures of the Silverline catheter after 16 hours<br />
and 32 hours. The lower row is the control.<br />
Continuing growth can be seen clearly on the<br />
cultures of the control catheters. The cultures of<br />
Silverline catheters show markedly reduced<br />
growth after 16 hours and sterility after 32<br />
hours.<br />
In clinical studies a marked reduction of the<br />
infection rate was found (9, 10, 11).<br />

Discussion<br />
The antimicrobial effect of Silverline Catheters<br />
could be shown clearly in the in-vitro trial. The<br />
broad spectrum of efficacy and the two phases of<br />
release of the antiseptic agent make the product<br />
efficient and long lasting.<br />
Because of its broad band effect with no danger<br />
of resistancies the Silverline catheters are a new<br />
approach for preventing EVD and shunt<br />
infection.<br />
