13 the employer within two days to the <strong>of</strong>ficer who issued it with a state-<br />

14 ment <strong>of</strong> the reasons for the termination <strong>of</strong> such employment. A work<br />

15 permit shall be issued only by the superintendent <strong>of</strong> schools or by a<br />

16 person authorized by him in writing, or, where there is no superin-<br />

17 tendent <strong>of</strong> schools, by a person authorized in writing by the local<br />

18 school board in the community where such child resides, upon the<br />

19 application <strong>of</strong> the parent, guardian or custodian <strong>of</strong> the child desiring<br />

20 such permit. The person authorized to issue work permits shall not<br />

21 issue any such permit until he has received, examined, approved and<br />

22 filed the following papers duly executed, namely:<br />

23 1. A written agreement from the person, firm or corporation into<br />

24 whose service the child under sixteen years <strong>of</strong> age is about to enter,<br />

25 promising to give such child employment, describing the work to be<br />

26 performed and agreeing to return the work permit <strong>of</strong> such child to<br />

27 the <strong>of</strong>fice from which it was issued within two days after the termina-<br />

28 tion <strong>of</strong> the employment <strong>of</strong> such child.<br />

29 2. The school record <strong>of</strong> such child filled out and signed by the<br />

30 chief executive <strong>of</strong> the school which such child has last attended certi-<br />

31 fying that the child is able to read intelligently and write legibly<br />

32 simple sentences in the English language and has completed a course<br />

33 <strong>of</strong> study equivalent to six yearly grades in reading, writing, spelling,<br />

34 English language, geography, and arithmetic. Such school record<br />

35 shall give also the name, date <strong>of</strong> birth and residence <strong>of</strong> the child as<br />

36 known on the records <strong>of</strong> the school and also the name <strong>of</strong> its parent,<br />

37 guardian or custodian.<br />

38 3. A certificate signed by a medical inspector <strong>of</strong> schools or if<br />

39 there be no such inspector then by a physician appointed by the board<br />

40 <strong>of</strong> education certifying that the applicant for the work permit has<br />

41 reached the normal development <strong>of</strong> a child <strong>of</strong> its age and is in suf-<br />

42 ficiently sound health and physically able to perform the work for<br />

43 which the permit is sought.<br />

44 4. Evidence <strong>of</strong> age showing that the child is fourteen years old<br />

45 or upwards which shall consist <strong>of</strong> one <strong>of</strong> the following pro<strong>of</strong>s required<br />

46 in the order herein designated as follows :<br />

47 a A transcript <strong>of</strong> the birth certificate filed according to law with<br />

48 a registrar <strong>of</strong> vital statistics or other <strong>of</strong>ficer charged with the duty <strong>of</strong><br />

49 recording births.<br />

50 6 A passport or a transcript <strong>of</strong> a certificate <strong>of</strong> baptism showing<br />

51 the date <strong>of</strong> birth and place <strong>of</strong> baptism <strong>of</strong> such child.<br />

52 c A. school census record.<br />

53 d In cases where none <strong>of</strong> the above named pro<strong>of</strong>s is obtainable,<br />

54 a certificate signed by the local medical inspector <strong>of</strong> schools, or if there<br />

55 be no such inspector then by a physician appointed by the local board<br />

56 <strong>of</strong> education certifying that in his opinion the applicant for the work<br />

57 permit is fourteen years <strong>of</strong> age or upwards.<br />

58 A duplicate <strong>of</strong> every such work permit issued shall be filled out<br />

59 and forwarded to the <strong>of</strong>fice <strong>of</strong> the commissioner <strong>of</strong> labor between the<br />

60 first and the tenth day <strong>of</strong> the month following the month in which it<br />

61 is issued. The blank forms for the work permit, the employer's agree-<br />

62 ment, the school record and the physician's certificate shall be formu-<br />

63 lated by the state superintendent <strong>of</strong> public instruction and furnished<br />

64 by him to the local school authorities. The work permit shall in no<br />

65 case be issued to the applicant or its parent, guardian or custodian,<br />

66 but shall in every case be forwarded to the prospective employer <strong>of</strong>
