9 registrar <strong>of</strong> all physicians, midwives, undertakers or retail casket<br />

10 dealers who have been registered in his district during the whole or<br />

11 any part <strong>of</strong> the preceding calendar years. No fee or other compen-<br />

12 sation shall be charged by local registrars to physicians, midwives,<br />

13 undertakers or casket dealers for registering their names under this<br />

14 section or making returns there<strong>of</strong> to the state registrar.<br />

SEC. 1376-al7. Returns from public and private institutions.<br />

1 All superintendents or managers, or other persons in charge <strong>of</strong><br />

2 hospitals, almshouses, lying-in or other institutions, public or private,<br />

3 to which persons resort for treatment <strong>of</strong> diseases, confinement, or<br />

4 are committed by process <strong>of</strong> law, shall make a record <strong>of</strong> all the per-<br />

5 sonál and statistical particulars relative to the inmates in their insti-<br />

6 tutions which are required in the forms <strong>of</strong> the certificates provided<br />

7 for by this chapter, as directed by the state registrar ; and such record<br />

8 shall be, by them, made for all inmates at the time <strong>of</strong> their admittance ;<br />

9 and in case <strong>of</strong> persons admitted or committed for treatment <strong>of</strong> disease,<br />

10 the physician in charge shall specify for entry in the record the nature<br />

11 <strong>of</strong> the disease, and where, in his opinion, it was contracted. The per-<br />

12 sonal particulars and information required by this section shall be<br />

13 obtained from the individual himself if practicable to do so; and when<br />

14 they can not be so obtained, they shall be obtained in as complete a<br />

15 manner as possible from relatives, friends, or other persons acquainted<br />

16 with the facts.<br />

SEC. 1376-al8. Blanks and instructions—defective certificates—<br />

system <strong>of</strong> preservation—private records—transcripts.<br />

1 The state registrar shall prepare, print and supply to all regis-<br />

2 trars, all blanks and forms used in registering, recording and preserv-<br />

3 ing the returns, or in otherwise carrying out the purpose <strong>of</strong> this<br />

4 chapter ; and shall prepare and issue such detailed instructions as may<br />

5 be required to procure the uniform observance <strong>of</strong> its provisions and<br />

6 the maintenance <strong>of</strong> a perfect system <strong>of</strong> registration.<br />

7 No other blanks shall be used than those supplied by the state<br />

8 registrar. He shall carefully examine the certificates received monthly<br />

9 from the local registrars, and if any such are incomplete or unsatis-<br />

10 factory he shall require such further information to be supplied as<br />

11 may be necessary to make the record complete and satisfactory.<br />

12 All physicians, midwives, informants or undertakers, and all<br />

13 other persons having knowledge <strong>of</strong> the facts, are hereby required to<br />

14 supply such information as they may possess, upon a form provided by<br />

15 the state registrar or upon the original certificate, regarding any<br />

16 birth or death upon demand <strong>of</strong> the state registrar, in person, by mail,<br />

17 or through the local registrar. No certificate <strong>of</strong> birth or death, after<br />

18 its acceptance for registration by the local registrar, and no other<br />

19 record made in pursuance <strong>of</strong> this chapter, shall be altered or changed in<br />

20 any respect otherwise, than by amendments properly dated, signed<br />

21 and witnessed.<br />

22 The state registrar shall further arrange, bind and permanently<br />

23 preserve the certificates in a systematic manner, and shall prepare<br />

24 and maintain a comprehensive and continuous card index <strong>of</strong> all
