6 in the following section. In each case where a physician, midwife, or<br />

7 person acting as midwife, was in attendance upon the birth, it shall<br />

8 be the duty <strong>of</strong> such physician, midwife, or person acting as midwife,<br />

9 to file in accordance herewith the certificate herein contemplated.<br />

10 In each case where there was no physician, midwife, or person<br />

11 acting as midwife, in attendance upon the birth, it shall be the duty<br />

12 <strong>of</strong> the father or mother <strong>of</strong> the child, the householder or owner <strong>of</strong> the<br />

13 premises where the birth occurred, or the manager or superintendent<br />

14 <strong>of</strong> the public or private institution where the birth occurred, each in<br />

15 the order named, within ten days after the date <strong>of</strong> such birth, to<br />

16 report to the local registrar the fact <strong>of</strong> such birth. In such case and<br />

17 in case the physician, midwife, or person acting as midwife, in attend-<br />

18 anee upon a birth is unable, by diligent inquiry, to obtain any item<br />

19 or items <strong>of</strong> information contemplated in the following section, it shall<br />

20 then be the duty <strong>of</strong> the local registrar to secure from the person so<br />

21 reporting, or from any other person having the required knowledge,<br />

22 such information as will enable him to prepare the certificate <strong>of</strong> birth<br />

23 herein contemplated, and it shall be the duty <strong>of</strong> the person reporting<br />

24 the birth or who may be interrogated in relation thereto to answer<br />

25 correctly and to the best <strong>of</strong> his knowledge all questions put to him<br />

26 by the local registrar which may be calculated to elicit any informa-<br />

27 tion needed to make a complete record <strong>of</strong> the birth as contemplated<br />

28 by said following section, and it shall be the duty <strong>of</strong> the informant<br />

29 as to any statement made in accordance herewith to verify such state-<br />

30 ment by his signature, when requested to do so by the local registrar.<br />

SEC. 1376-al4. Standard certificate <strong>of</strong> birth.<br />

1 The certificate <strong>of</strong> birth shall be <strong>of</strong> the United <strong>State</strong>s standard<br />

2 form, as approved by the bureau <strong>of</strong> the census, and shall contain the<br />

3 following items, which are hereby declared necessary for the legal,<br />

4 social and sanitary use <strong>of</strong> registration record :<br />

5 1. Place <strong>of</strong> birth, including state, county, township or incor-<br />

6 porated town, or city. If in the city, the ward, street, and the house<br />

7 number; if in a hospital or other institution, the name <strong>of</strong> the same<br />

8 to be given, instead <strong>of</strong> the street and house number.<br />

9 2. Full name <strong>of</strong> child. If the child dies without a name, before<br />

10 the certificate is filed, enter the words "died unnamed". If the living<br />

11 child has not yet been named at the date <strong>of</strong> filing certificate <strong>of</strong> birth,<br />

12 the space for "full name <strong>of</strong> child" is to be left blank, to be filled out<br />

13 subsequently by a supplemental report, as hereinafter provided.<br />

14 3. Sex <strong>of</strong> child.<br />

15 4. Whether a twin, triplet, or other plural birth. A separate<br />

16 certificate shall be required for each child in case <strong>of</strong> plural births.<br />

17 5. For plural births, number <strong>of</strong> each child in order <strong>of</strong> birth.<br />

18 6. Whether legitimate or illegitimate.<br />

19 7. Date <strong>of</strong> birth, including the year, month and day.<br />

20 8. Full name <strong>of</strong> father; provided, that if the child is illegiti-<br />

21 mate, the name <strong>of</strong> the putative father shall not be entered without his<br />

22 consent, but the other particulars relating to the putative father (items<br />

23 nine to thirteen inclusive) may be entered if known, otherwise as<br />

24 "Unknown".<br />

25 9. Residence <strong>of</strong> father.<br />

26 10. Color or race <strong>of</strong> father.
