21 No local registrar shall receive any fee for the issuance of burial
22 or removal permits under this chapter other than the compensation
23 provided in section thirteen hundred seventy-six-a twenty-of this sup-
24 plement.
25 A burial permit shall not be required from the local registrar of
26 the district in which the interment is made, when a body is removed
27 from one district in Iowa to another in the state, for purpose of burial
28 or other disposition.

SEC. 1376-a6. Stillborn children.
1 A stillborn child shall be registered as a birth and also as a death,
2 and a certificate of both the birth and death shall be filed with the
3 local registrar, in the usual form and manner, the certificate of birth
4 to contain, in place of the name of the child, the word "stillbirth". A
5 certificate of birth and a certificate of death shall not be required for
6 a child that has not advanced to the fifth month of uterogestation.
7 The medical certificate of the cause of death shall be signed by the
8 attending physician, if any, and shall state the cause of death as "still-
9 born", with the cause of the stillbirth, if known, whether a premature
10 birth, and, if born prematurely, the period of uterogestation, in
11 months, if known ; and a burial or removal permit of the prescribed
12 form shall be required. Midwives shall not sif?n certificates of death
13 for stillborn children ; but such cases, and stillbirths occurring with-
14 out attendance of either physician or midwife, shall be treated as
15 deaths without medical attendance, as provided for in section thirteen
16 hundred seventy-six-a eight of this supplement.

SEC. 1376-a7. Standard certificates of death—authentication.
1 The certificate of death shall be of a United States standard form,
2 . as approved by the bureau of the census and shall contain the follow-
3 ing items, which are hereby declared necessary for the legal, social
4 and sanitary purposes subserved by registration records :
5 1. Place of death, including state, county, township or incor-
6 porated town or city. If in a city, the ward, street and house num-
7 ber ; if in a hospital or other institution, the name of the same to be
8 given instead of the street and house number. If in an industrial
9 camp, the name of the camp to be given.
10 2. Full name of decedent. If an unnamed child, the surname
11 preceded by "unnamed".
12 3. Sex.
13 4. Color or race, as white, black, mulatto (or other negro
14 descent), Indian, Chinese, Japanese, or other.
15 5. Conjugal condition, as single, married, widowed or divorced.
16 6. Date of birth, including the year, month and day.
17 7. Age, in years, months and days. If less than one day, the
18 hours or minutes.
19 8. Occupation. The occupation to be reported of any person,
20 male or female, who had any remunerative employment, stating (a)
21 trade, profession or particular kind of work; (b) general nature of
22 industry, business or establishment in which employed (or employer).
