21 No local registrar shall receive any fee for the issuance <strong>of</strong> burial<br />

22 or removal permits under this chapter other than the compensation<br />

23 provided in section thirteen hundred seventy-six-a twenty-<strong>of</strong> this sup-<br />

24 plement.<br />

25 A burial permit shall not be required from the local registrar <strong>of</strong><br />

26 the district in which the interment is made, when a body is removed<br />

27 from one district in <strong>Iowa</strong> to another in the state, for purpose <strong>of</strong> burial<br />

28 or other disposition.<br />

SEC. 1376-a6. Stillborn children.<br />

1 A stillborn child shall be registered as a birth and also as a death,<br />

2 and a certificate <strong>of</strong> both the birth and death shall be filed with the<br />

3 local registrar, in the usual form and manner, the certificate <strong>of</strong> birth<br />

4 to contain, in place <strong>of</strong> the name <strong>of</strong> the child, the word "stillbirth". A<br />

5 certificate <strong>of</strong> birth and a certificate <strong>of</strong> death shall not be required for<br />

6 a child that has not advanced to the fifth month <strong>of</strong> uterogestation.<br />

7 The medical certificate <strong>of</strong> the cause <strong>of</strong> death shall be signed by the<br />

8 attending physician, if any, and shall state the cause <strong>of</strong> death as "still-<br />

9 born", with the cause <strong>of</strong> the stillbirth, if known, whether a premature<br />

10 birth, and, if born prematurely, the period <strong>of</strong> uterogestation, in<br />

11 months, if known ; and a burial or removal permit <strong>of</strong> the prescribed<br />

12 form shall be required. Midwives shall not sif?n certificates <strong>of</strong> death<br />

13 for stillborn children ; but such cases, and stillbirths occurring with-<br />

14 out attendance <strong>of</strong> either physician or midwife, shall be treated as<br />

15 deaths without medical attendance, as provided for in section thirteen<br />

16 hundred seventy-six-a eight <strong>of</strong> this supplement.<br />

SEC. 1376-a7. Standard certificates <strong>of</strong> death—authentication.<br />

1 The certificate <strong>of</strong> death shall be <strong>of</strong> a United <strong>State</strong>s standard form,<br />

2 . as approved by the bureau <strong>of</strong> the census and shall contain the follow-<br />

3 ing items, which are hereby declared necessary for the legal, social<br />

4 and sanitary purposes subserved by registration records :<br />

5 1. Place <strong>of</strong> death, including state, county, township or incor-<br />

6 porated town or city. If in a city, the ward, street and house num-<br />

7 ber ; if in a hospital or other institution, the name <strong>of</strong> the same to be<br />

8 given instead <strong>of</strong> the street and house number. If in an industrial<br />

9 camp, the name <strong>of</strong> the camp to be given.<br />

10 2. Full name <strong>of</strong> decedent. If an unnamed child, the surname<br />

11 preceded by "unnamed".<br />

12 3. Sex.<br />

13 4. Color or race, as white, black, mulatto (or other negro<br />

14 descent), Indian, Chinese, Japanese, or other.<br />

15 5. Conjugal condition, as single, married, widowed or divorced.<br />

16 6. Date <strong>of</strong> birth, including the year, month and day.<br />

17 7. Age, in years, months and days. If less than one day, the<br />

18 hours or minutes.<br />

19 8. Occupation. The occupation to be reported <strong>of</strong> any person,<br />

20 male or female, who had any remunerative employment, stating (a)<br />

21 trade, pr<strong>of</strong>ession or particular kind <strong>of</strong> work; (b) general nature <strong>of</strong><br />

22 industry, business or establishment in which employed (or employer).
