(C) Complete and coordinate the <strong>Support</strong> <strong>Services</strong> Brokerage<br />
Plan of Care Crisis Addendum when an individual in emergent<br />
status requires a short-term, out-of-<strong>home</strong>, residential<br />
placement; and<br />
(D) Monitor the delivery of supports provided, including those<br />
provided through crisis funding.<br />
(i) Monitoring is done through contact <strong>with</strong> the individual,<br />
any service providers, and the individual's family.<br />
(ii) Monitoring is done to collect in<strong>for</strong>mation regarding<br />
supports provided and progress toward outcomes that are<br />
identified as necessary to resolve the crisis.<br />
(iii) The personal agent must document the in<strong>for</strong>mation<br />
described in OAR 411-340-0125(3)(b)(D)(ii) of this section<br />
in the individual's case file and report to the Regional<br />
Crisis Diversion Program or CDDP as required.<br />
(c) <strong>Support</strong> services provided during emergent status are subject to<br />
all requirements of this rule.<br />
(d) All supports authorized in an ISP continue during the crisis unless<br />
prohibited by other rule, policy, or the supports contribute to new or<br />
additional crisis risk factors.<br />
(4) TRANSITION TO COMPREHENSIVE SERVICES. When an individual<br />
eligible <strong>for</strong> crisis supports may have long-term support needs that may not<br />
be met through support services:<br />
(a) The brokerage must immediately notify the CDDP of the<br />
individual’s county of residence;<br />
(b) The brokerage must coordinate <strong>with</strong> the CDDP and the Regional<br />
Crisis Diversion Program to facilitate a timely exit from support<br />
services and entry into appropriate, alternative services; and<br />
