(b) At least annually as part of preparation <strong>for</strong> a new ISP, the<br />

personal agent must:<br />

(A) Evaluate progress toward achieving the purposes of the<br />

ISP, assessing and revising goals as needed;<br />

(B) Note effectiveness of the use of support services funds<br />

based on personal agent observation as well as individual<br />

satisfaction;<br />

(C) Determine whether changing needs or availability of other<br />

resources has altered the need <strong>for</strong> continued use of support<br />

services funds to purchase supports; and<br />

(D) Record final support services fund costs.<br />

(12) TRANSITION TO ANOTHER BROKERAGE. At the request of an<br />

individual enrolled in brokerage services who has selected another<br />

brokerage, the brokerage must collaborate <strong>with</strong> the receiving brokerage<br />

and the CDDP of the individual's county of residence to transition support<br />

services.<br />

(a) If the Department has designated and contracted funds solely <strong>for</strong><br />

the support of the transitioning individual, the brokerage must notify<br />

the Department to consider transfer of the funds <strong>for</strong> the individual to<br />

the receiving brokerage.<br />

(b) The ISP in place at the time of request <strong>for</strong> transfer may remain in<br />

effect 90 days after entry to the new brokerage while a new ISP is<br />

negotiated and authorized.<br />
