(9) The brokerage or provider organization may be required to submit to the<br />

Division a plan of improvement <strong>for</strong> any noncompliance found during an<br />

inspection pursuant to section (1)(a) of this rule.<br />

411-340-0060 Complaints in <strong>Support</strong> <strong>Services</strong> Brokerages<br />

(Amended 7/1/2009)<br />

(1) COMPLAINTS. Brokerages must develop and implement written<br />

policies and procedures regarding individual complaints and a <strong>for</strong>mal<br />

complaint process. These policies and procedures must at minimum<br />

address:<br />

(a) Receipt of complaints. If a complaint is associated in any way <strong>with</strong><br />

abuse, the recipient of the complaint must immediately report the<br />

issue to the CDDP and notify the brokerage director and, if<br />

applicable, the provider organization director. The brokerage must<br />

maintain a log of all complaints regarding the brokerage, provider<br />

organization, or independent provider that the brokerage receives<br />

from individuals, others acting on the behalf of individuals, and from<br />

provider organizations acting in accordance <strong>with</strong> OAR 411-340-<br />

0170(2)(a)(C)(v).<br />

(A) The complaint log must, at a minimum, include the<br />

following:<br />

(i) The date the complaint was received;<br />

(ii) The name of the person taking the complaint;<br />

(iii) The nature of the complaint;<br />

(iv) The name of the person making the complaint, if<br />

known; and<br />
